    目的:探讨Gadd45a和p53在乳腺癌中的表达情况及其与临床病理参数和预后的关系-方法:采用免疫组化法检测78例乳腺癌组织及其癌旁正常组织中Gadd45a和p53的表达-结果:Gadd45a在乳腺癌组织中的阳性表达率为38.4%,明显低于正常乳腺组织,p53在乳腺癌组织中的阳性表达率为65.4%,明显高于正常乳腺组织,差异均具有统计意义 (P < 0.05);Gadd45a在乳腺癌组织学Ⅰ级组-Ⅱ级组-Ⅲ级组的阳性表达率依次下降,差异均有统计学意义(P < 0.05);p53在伴有淋巴结转移组中的阳性表达率高于无淋巴结转移组,差异均有统计意义(P < 0.05);p53表达与Gadd45a表达呈负相关(P < 0.05);Gadd45a阳性组的生存期长于阴性组,生存率高于阴性组,存在统计学差异;p53阳性组的生存期短于阴性组,生存率低于阴性组,存在统计学差异(P < 0.05);p53(+)Gadd45a(-)组(即阳性组)中位生存时间短于阴性组,生存率低于阴性组-结论:Gadd45a和p53蛋白的检测可作为乳腺癌恶性程度的检测指标;p53的表达与乳腺癌的转移相关,Gadd45a的表达与乳腺癌的分化分级相关;p53高表达及Gadd45a低表达与乳腺癌的不良预后相关;p53(+)Gadd45a(-)表型与乳腺癌的不良预后相关;Gadd45a蛋白有望成为乳腺癌治疗的新的靶点-

    Abstract:

    Objective: To study the expression of Gadd45a and p53 protein in breast cancer and its correlation with clinical pathologic features in breast cancer. Methods: The expressions of Gadd45a and p53 proteins in 78 cases of breast cancer and mammary gland tissues were determined by the immunohistochemical staining. Results: The expression of Gadd45a in patients was lower than that in the control mammary gland tissue,while the expression of p53 in cancer samples was higher than that in the adjacent mammary gland tissue (P < 0.05). The positive rate of Gadd45a correlated differentiation level of cancer (Grade I,83.3%,Grade II,46.7%,Grade III,16.7%,P < 0.05),meanwhile,the positive rate of p53 in patients with lymph node metastasis was 76.6%,which was much higher than that in no metastasis patient (48.4%,P < 0.05). p53 correlated with Gadd45a negatively. Survival analyses showed significant differences between the positive class and negative class of these three proteins[MST(median survival time):Gadd45a:57.3m,46.3m;p53:47.7m,58.6m,5-year survival rate:Gadd45a:89.6%,60.0%;p53:70.6%,92.6%]. Survival analyses shown significant differences between the positive class and negative class of p53(+)Gadd45a(-)[MST:p53(+)Gadd45a(-):37.5m;58.3m,5-year survival rate:p53 (+)Gadd45a (-):50.0%,89.2%]. Conclusion: Gadd45a and p53 testing could predict the invasive behavior of breast cancer. The expression of Gadd45a correlates with a favorable prognosis,and the expression of p53 correlates with unfavorable prognosis,p53 (+)Gadd45a (-)predict poor prognosis. Gadd45a and p53 could be developed to be a new breast cancer therapeutic target.
