      目的:探究乳化异氟烷后处理对离体大鼠肺缺血再灌注损伤的影响?方法:雄性SD大鼠随机分成4组(n=6):空白对照组(S组)?缺血再灌注组(IR组)?乳化异氟烷组(EI组)?脂肪乳组(IL组),建立离体大鼠肺缺血再灌注损伤模型?分别于平衡末(T0)?缺血缺氧前即刻(T1)?再灌注30 min(T2)和再灌注60 min(T3)时,记录潮气量(tidal volume,VT)?肺静脉氧分压(oxygen partial,PaO2)?肺顺应性(compliance,CL)和气道阻力(airway resistance,R)的数值;测定肺湿/干重比(W/D)并观察肺组织HE染色情况;检测肺组织中SOD和MDA的表达水平?结果:与T0时刻比较,4组生理数据在T3时刻VT?CL和PaO2下降而R上升(P < 0.05或0.01)?与S组比较,IR组和IL组在T2?T3时刻VT?CL和PaO2下降而R上升(P < 0.05或0.01);与IR组比较,EI组和IL组在T2?T3时刻VT?CL和PaO2上升而R下降(P < 0.05或0.01);与EI组比较,IL组在T2时刻PaO2下降,在T3时刻VT?CL和PaO2下降而R上升(P < 0.05)?与S组比较,IR组?IL组W/D和MDA表达水平上升而SOD表达水平下降(P < 0.05或0.01),EI组SOD表达水平下降(P < 0.01);与IR组比较,EI组W/D和MDA表达水平下降而SOD表达水平上升(P < 0.05或0.01),IL组W/D下降而SOD表达水平上升(P < 0.05);与EI组比较,IL组SOD表达水平下降而MDA表达水平上升(P < 0.05)?病理显示EI组肺损伤轻于IR组和IL组?结论:8%乳化异氟烷后处理对大鼠肺缺血再灌注损伤有保护作用,其机制可能与减轻肺缺血再灌注损伤后氧化应激有关?

      Objective:To investigate the emulsified isoflurane post-conditioning on isolated lung ischemia-reperfusion(I/R)injury in rats. Methods:Sprague-Dawley rats were randomly divided into four groups(n=6 each):control group(group S),ischemia-reperfusion group(group IR),emulsified isoflurane group(group EI)and intralipid group(group IL). Lung ischemia-reperfusion injury model was established. Recorded tidal volume(Tidal volume,VT),pulmonary venous partial pressure of oxygen(Oxygen partial,PaO2),lung compliance(Compliance,CL)and airway resistance(Airway resistance,R)values at the end of the balance(T0),immediately prior to ischemia and hypoxia(T1),reperfusion 30 min(T2)and reperfusion 60 min (T3);measured lung wet / dry weight ratio (W/D);observed lung tissue HE staining and detected the expression levels of SOD and MDA. Results:Compared with T0 moment,VT,CL and PaO2 decreased but R increased at T3 moment in the four groups(P < 0.05 or 0.01). Compared with S group,VT,CL and PaO2 significantly decreased but R obviously increased at T2-3 moment in IR group and IL group(P < 0.05 or 0.01). Compared with IR group,VT,CL and PaO22 increased but R decreased at T2-3 moment in EI group and IL group(P < 0.05 or 0.01). Compared with EI group,PaO2 decreased at T2-3 moment,VT and CL decreased but R increased at T3 moment in IL group(P < 0.05). Compared with S group,W/D,the expression level of MDA increased and the expression level of SOD decreased in IR group and IL group(P < 0.05 or 0.01),the expression level of SOD decreased in EI group(P < 0.01). Compared with IR group,W/D,the expression level of MDA decreased and the expression level of SOD increased in EI group(P < 0.05 or 0.01),W/D decreased and the expression level of SOD increased in IL group(P < 0.05). Compared with EI group,the expression level of SOD decreased and the expression level of MDA increased in IL group(P < 0.05). Pathology showed that the damage of lung tissue of group IL or group EI was lighter than that in group IR,the damage of lung tissue of group EI was lighter than that in group IL. Conclusion:8% emulsified isoflurane post-conditioning can prevent isolated lung ischemia-reperfusion injury in rats,and the mechanism may be related to alleviating lung oxidative stress caused by ischemia-reperfusion injury.
