      目的:探讨血清脂蛋白相关磷脂酶A2(lipoprotein?associated phospholipase A2,LP?PLA2)与冠状动脉粥样硬化之间的相关性。方法:选取南京医科大学第二附属医院心内科住院患者共119例纳入试验,其中冠脉造影显示有冠脉病变的83例纳入冠脉病变组,无冠脉病变的36例纳入冠脉造影阴性组。采用酶动力学方法检测LP?PLA2,酶联免疫吸附试验(enzyme linked immunosorbent assay,ELISA)法检测高敏C反应蛋白(high sensitive C?reaction protein,hs?CRP),免疫比浊法检测D?二聚体水平。将患者根据冠脉病变支数分为1支病变、2支或2支+左主干病变、3支或3支+左主干病变3组,并使用Gensini评分对冠脉病变程度进行评分。结果:冠脉病变组血清LP?PLA2、hs?CRP及D?二聚体水平较冠脉造影阴性组明显升高(P<0.05)。不同冠脉病变程度患者之间LP?PLA2和hs?CRP具有显著差异(P<0.05)。进一步的Pearson相关性分析显示LP?PLA2和hs?CRP与冠脉病变Gensini评分具有较低相关度的正相关关系(r=0.418,r=0.365,P<0.05)。结论:血清LP?PLA2、hs?CRP及D?二聚体水平与冠状动脉粥样硬化具有相关性。对于冠脉病变患者,血清LP?PLA2和hs?CRP水平在评估冠脉病变程度上仅能提供有限的参考价值。

      Objective:To investigate the association between serum lipoprotein?associated phospholipase A2(LP?PLA2)and coronary atherosclerosis. Methods:A total of 119 inpatients from the Second Affiliated Hospital of NMU were selected in this study. Eighty?three patients with coronary atherosclerosis were included in the coronary lesion group,and 36 people without coronary atherosclerosis were included in the negative group. LP?PLA2 was detected by enzyme kinetic method,high sensitive C?reaction protein(hs?CRP)was detected by enzyme linked immunosorbent assay(ELISA),and the level of D?Dimer was detected by immune turbidimetry. The patients in the case group were divided into three groups including 1 branches,2 or 2 branches lesion + left main lesion,and 3 or 3 branches lesion according to the number of diseased coronary artery. The severity of coronary artery lesion was evaluated by the Gensini integrations. Results:The serum levels of LP?PLA2,hs?CRP and D? Dimer in the case group were significantly higher than those in the control group(P < 0.05). The levels of LP?PLA2 and hs?CRP were significantly different between patients with different severity of coronary lesions(P < 0.05). Furthermore,Pearson correlation analysis showed that LP?PLA2 and hs?CRP had a low?level positive correlation with Gensini score(r=0.418,r=0.365,P < 0.05). Conclusion:The levels of serum LP?PLA2,hs?CRP and D?Dimer were correlated with coronary atherosclerosis. For patients with coronary atherosclerosis,serum levels of LP?PLA2 and hs?CRP can only provide a limited reference value in assessing the extent of coronary artery lesions.
