      目的:探究血清adropin蛋白与2型糖尿病(type 2 diabetes mellitus,T2DM)及颈动脉粥样硬化(carotid atherosclerosis,CAS)的关系。方法:选取住院T2DM患者108例,依据颈动脉彩色多普勒超声检查结果分为T2DM+CAS组68例和单纯T2DM组40例;同期选取健康体检者60例作为正常对照(normal control,NC)组。采集受试者的一般资料并测定其临床代谢指标;血清adropin浓度通过酶联免疫吸附法测定。结果:T2DM+CAS组与单纯T2DM组的adropin浓度均低于NC组(P<0.01);而T2DM+CAS组与单纯T2DM组比较,adropin浓度差异无统计学意义。相关性分析显示,血清adropin水平与体重指数、甘油三酯、空腹血浆葡萄糖、空腹胰岛素、稳态模型胰岛素抵抗指数、糖化血红蛋白、高敏C 反应蛋白呈负相关(均P<0.01);与高密度脂蛋白胆固醇呈正相关(P<0.01)。多元线性回归提示空腹胰岛素、糖化血红蛋白是adropin浓度的独立影响因素(均P<0.01)。结论:血清adropin 水平在T2DM患者中显著降低,其可能通过调节胰岛素敏感性、脂质代谢和炎症反应等作用与T2DM的病理过程密切相关。

      Objective:To investigate the relationship between serum adropin and carotid atherosclerosis in patients with type 2 diabetes mellitus(T2DM). Methods:A total of 108 T2DM patients were divided into two groups based on the results of carotid color Doppler ultrasound: 68 with carotid atherosclerosis(T2DM+CAS group)and 40 without carotid atherosclerosis(simple T2DM group),and 60 healthy volunteers were enrolled as normal control(NC group). General information was gathered. Biochemistry,glycosylated hemoglobin A1c(HbA1c) and fasting insulin(FIns) were detected by clinical laboratory. Serum adropin concentration was measured by enzyme?linked immunosorbent assay. Results:The levels of adropin in T2DM+CAS and simple T2DM were significantly lower than that in NC(P<0.01).There was no significant difference of the adropin concentration between T2DM+CAS group and simple T2DM group. Adropin level was negatively correlated with body mass index,triglycerides,fasting plasma glucose,FIns,homeostasis model assessment of insulin resistance(HOMA?IR),HbA1c,high?sensitive C reactive protein and positively correlated with high?density lipoprotein cholesterol(all P<0.01). Multiple linear regression analysis revealed that FIns and HbA1c were independent influencing factors of serum adropin(all P<0.01). Conclusion:Serum adropin levels were significantly reduced in patients with T2DM,which may be closely related to the pathological process of T2DM by regulating insulin sensitivity,lipid metabolism and inflammatory responses.
