Clinical evaluation of contrast-enhanced ultrɑsonogrɑphy with low acoustic pressure in the differential diagnosis of focal liver lesion

    Abstract:

    Objective: To evaluate the value of contrast-enhanced ultrasonography with low acoustic pressure in differential diagnosis of focal liver lesion. Methods:181 cases with 209 focal liver lesions underwent contrast-enhanced ultrasonography(CEUS) and the results were analyzed retrospectively. The lesion was confirmed by pathology or follow-up for 6-12 months. Results:144 cases of HCC, 1 case of angiosarcoma, 1 case of cholangiocellular carcinoma(CCC) and 1 case of hepatic tuberculosis(HTB) showes an enhancement pattern of “fast-in and fast-out”, 25 cases of haemangioma showing “slow-in and slow-out”, 21 nodules of adenomatous hyperplasia in cirrhotic patients showing no enhancement in the arterial phase. In the portal phase, the echogenicity increased and the lesion became isoechoic compared to the surrounding liver parenchyma. Five cases of metastasis showed a pattern of “fast out”, 1 case of focal nodular hyperplasia(FNH) and 1 case of inflammatory pseudotumor(IPT) showing“fast-in and slow-out”. Five cases of liver abscess showed quick indiscrete haphazard enhancement, and 3 cases of IPT and 2 cases of HTB showing no enhancement. Conclusion:CEUS is a choice for characterization of liver focal lesions. Compared to CDFI, CEUS, can dramatically improve the ability to find and characterize focal liver lesions.
