      Abstract:Objective:To evaluate the effects of superfusion of ethanol or ethanol plus dexamethasone on corticostriatal long-term potentiation(LTP) in rat striatum in vitro. Methods:Extracellular field potential recording method was used in the study. Results:Ethanol could enhance the amplitude of LTP,while dexamethasone could reverse the impact induced by ethanol partly. Conclusion:The inebriant dose ethanol could enhance the amplitude of corticostriatal LTP through the way of activating immediate early gene.

    • Effects of sodium ferulate on the ultrarapid delayed rectifier K+

      Abstract:Objective:To study the effects of sodium ferulate on the ultrarapid delayed rectifier K+ current(IKur) in human atrial myocytes. Methods:Human atrial myocytes were isolated by enzyme dispersion method. IKur in human atrial myocytes were recorded by using the whole cell patch clamp. The changes of IKur were compared in the absence and the presence of sodium ferulate. Results:There was no effect of 0.4 g/L sodium ferulate on I-V relation of IKur. However, 0.4 g/L sodium ferulate inhibited IKur to some degrees at each test pulse. The current densities of IKur at +60 mV decreased from 4.997±0.35 PA/PF to 3.331±0.26 PA/PF(n = 6, P < 0.05). The inhibitory effect was concentration-dependent. IC50 was(0.41±0.03)g/L and the Hill coefficient was 0.95±0.05. Conclusion:Sodium ferulate as a potassium channel blocker can inhibit IKur in human atrial myocytes effectively.

    • ?Effect of pharmaceutical intervention on AT1R, AT2R, ERK and JNK activity in chronic hibernating myocardium in rabbits

      Abstract:Objective: To investigate in chronic hibernating myocardium in rabbits and the influence and significance of captopril, betaloc, valsartan in angiotensinⅡsubtype 1 receptor(AT1R), angiotensinⅡsubtype 2 receptor(AT2R), extracellular signal regulated kinase 1/2 (ERK1/2), c-Jun N-terminal kinase(JNK). Methods: The model of chronic hibernating myocardium(CHM) was established. The changes of AT1R, AT2R, ERK1/2, JNK in different groups were assessed by western blotting and immunohistochemistry. Results: The amount of AT1R decreased while AT2R increased in the CON group compared with in sham group, and both AT1R and AT2R decreased in drug groups compared with the CON group. The content of ERK had no change in each group, while that of “expression” p-ERK increased in CON group compared with in sham group, and was lower in drug intervention groups than in CON and sham groups. The contents of JNK and p-JNK decreased in CON and drug intervention groups compared with in sham group. The protein levels of JNK, p-JNK in drug intervention groups were lower than in the CON group. Three drugs can inhibit interstitial fibrosis and reduce apoptotic cells. The expression levels in the groups(with different doses) had statistical difference as well as between groups of captopril and other drugs; however the results between betaloc and valsartan had no significant difference. Conclusion: AT1R, AT2R may be the upper stream receptor of ERK and JNK and may participate in generation and evolution of CHM. Captopril, valsartan and betaloc may preserve CHM by inhibiting AT1R, AT2R and JNK activity.

      Abstract:medical conducts, especially surgeries, were invasive procedures with potential possibility of injury. the positive values of medical conducts called for non-criminalization. to that aim, it was necessary to design “medical exception” in criminal law. the theoretical basis of “medical exception” was “consent” defense theory in criminal law. “medical exception” was only applicable to conduct which was beneficial to patients and had a therapeutic purpose. with social values updated, “medical exception” would apply beyond therapeutic medical procedures.

    • The cytoskeleton reorganization of NRK52E cells was associated with the redistribution of ERM during ATP depletion

      Abstract:Objective:To investigate the cytoskeleton reorganization of NRK52E during cellular ATP depletion and its mechanisms. Methods:We established a cellular ATP depletion model was established in vitro with 0.1 μmol/L antimycin A diluted in depletion buffer;F-actin was examined with fluorescence of FITC-phalloidin by flow cytometric analyses;the expression of ERM proteins was detected through Western Blotting and immunoblotting technologies. Results:The cellular ATP depletion model was established successfully,and the cellular ATP concentration declined in a time-dependent manner(P < 0.05); The F-actin of NRK52E was increasing during cellular ATP depletion,and these changes were paralleled with the severity of cellular ATP depletion(P < 0.05). When ATP was depleted,the ERM protein detached from the cytoskeleton into the cytoplasma. Conclusion:The cytoskeleton reorganization of NRK52E cells might be associated with the redistribution of ERM during ATP depletion.

      Abstract:Objective:To investigate the effects of genestin on the actin cytoskeleton and its mechanisms involeved in p38 kinase-HSP27 signaling pathway. Methods:The breast cancer cells(MDA-MB-435 cells) were treated with different concentrations of genistein(12.5,25.0,50.0 and 100.0 -滋mol/L) for 24 h. The expressions of p38 kinase and HSP27 and the phosphorylation of p38 and HSP27 were evaluated by Western blotting analysis. The expressions of actin and HSP27 in cytosolic fraction and cytoskeletal fraction were also detected by Western blotting. Results:After treated with genistein for 24 h,cellular F-actin was depolymerized and the phosphorylation of p38 kinase and HSP27 was significantly attenuated compared with the control cells. HSP27 translocated from the cytosolic fraction to the cytoskeletal fraction simultaneously. Conclusion:These findings suggested that the reorganization of actin cytoskeleton induced by genistein should probably be mediated by translocation of HSP27 in cells via p38 kinase signaling pathway.

      Abstract:Objective: To study the effects and mechanism of homocysteine(Hcy) on Parkinson’s disease(PD) induced by 6-hydroxydopamine(6-OHDA) in vivo. Methods:Forty rats were divided into 4 groups. 6-OHDA or the solvent of 6-OHDA was focally administrated to induce PD, 2 h later Hcy or 0.9% sodium chloride was administrated in the ipsolateral substantial nigra(SN). We used behavioral testing, Immolunohistochemical techniques, biochemistry techniques to detect the injury of SN. Results:The rotary turns of PD rats induced by 6-OHDA showed significant increase after treatment with Hcy compared with the controls(P < 0.05). Also the numbers of tyrosine hydroxylase(TH)-stained neurons were decreased, and dendrites were fragmented and truncated. Free radicals were increased and antioxidant enzymes decreased. Conclusion:Focal infusion of Hcy into the SN increased the vulnerability of the dopaminergic neurons to 6-OHDA-induced degeneration, it seems that the endangering effect of Hcy is due to exacerbating oxidative stress.

    • Establishment of human breast carcinoma model in ovariectomized nude mice

      Abstract:Objective:To establish a model of human breast carcinoma in ovariectomized nude mice and study their biological features. Methods:MCF-7 cells(estrogen-receptor positive,ER+) were implanted into the flank of ovariectomized nude mice. The tumor was stimulated by estradiol E2 transdermal patches. Tumor size and body weight were monitored weekly during the study. The tumor-bearing mice were killed on day 30 and tumor masses were collected for analysis. Tumor tissues were pathologically examined by the light microscope. The ER protein was detected by immunohistochemistry. Results:The tumor tubercles could be formed on day 4. The tumor-take rate was 75%(12/16),and the mean tumor surface areas was(10.0 ± 2.1)mm2. All of the specimens were infiltrating ductal carcinoma,and ER expression was positive. Conclusion:Human breast carcinoma model was established successfully in ovariectomized nude mice,which reserved certain biological features of human breast carcinoma,and it was convenient to use transdermal patches,therefore it might be an available model for further research on breast carcinoma.

    • “Water-cooled circulation” microwave coagulation in pig breast:an experiment study

      Abstract:Objective:To study the feasibility and effectiveness of “water-cooled circulation” microwave coagulation(MC) in the treatment of breast tumor. Methods:The breast of pigs underwent coagulation with “water-cooled” microwave coagulation equipment-ECO100 type. After MC procedure,the volume of target area were calculated;routine pathological examination were done in the MC lesion and surrounding tissue.Adverse effects on surrounding breast tissue and skin were carefully examined during and after MC ablation. Results:Coagulation volume was postivly correlated with the power and time of MC. The correlation coefficient between MC volume and cross product of MC power with time is 0.904,regression equation is y=0.117x-2.608. Target breast cells showed coagulative-necrosis changes by HE staining after MC,no clear change were obsered in surrounding tissue by HE staining. There was not skin burn and adjacent tissue damage outside targeted tissue in the treatment of“water-cooled circulation”. Conclusion:“Water-cooled circulation” microwave coagulation could make breast cell coagulative-necrosis,and the coagulation circumscription was clear.The therapy was microinvasive,and no serious adverse effect was founded. The experiment provided a basis for the clinical application of microwave.

    • Apoptotic effect and mechanisms of AHPN on human skin malignant melanoma cell A375

      Abstract:Objective: To study apoptotic effects of synthetic retinoic acid 6-[3-(1-adamantyl)-4-hydroxyphenyl]-2-naphthalene carboxylic acid(AHPN) on human skin malignant melanoma A375 cells in comparison with the natural ligand all-trans-retinoic acid(ATRA) in vitro and the mechanisms related to the actions of AHPN. Methods:MTT assay was used to determine the anti-proliferative effects of AHPN and ATRA on A375 cells. Flow cytometry was performed to investigate the influence of AHPN and ATRA on cell cycle and cell apoptosis. In addition, transfection and luciferase activity assays were employed to explore the mechanisms of how AHPN executes its proapoptotic function. Results:Firstly, AHPN promoted apoptosis and G1 arrest in A375 cells compared with ATRA. Secondly, the activity of NF-κB in A375 cells treated with AHPN increased 2-3 times compared with solvent DMSO treatment. Conclusion:AHPN, in comparison with ATRA, is a more effective alternative for therapy of malignant melanoma. The potentially proapoptotic function of AHPN requires activation of NF-κB.

The article also brought forward measures and proposals for pathologic autopsies by means of analysis of 63 samples of autopsies involved in medical tangles in Wuxi area.

    • The comparative study of treatment in experimental spinal cord injury by FTY720 with FK506

      Abstract:Objective:To compare the differences of neuroprotective effect between FTY720 and FK506 on spinal cord injury(SCI). Methods:A total of 48 male rats were divided randomly into 4 groups:①the test group treated with FTY720;②the test group treated with FK506;③the injuried group and ④the control group. While A-C group were enrolled to establish SCI models using Allen method before treated. The neurological function score and immunohistochemical examination were assessed at certain time after injury. Results:The group of FTY720 was better than the group of FK506 in the detection of Inclined Plate Test scores,BBB scales and SEP test. In injury group,a large number of neuronal death could be seen,and pathological changes in treatment group were better than those in injuried group. The pathological changes in the test group treated with FTY720 group was lest. Conclusion:Early using both of FTY720 and FK506 might be capable of treating SCI,and the effect of FTY720 was superior to FK506.

    • Evaluation of mandibular reconstruction with free vascularized fibular flap

      Abstract:Objective: To evaluates mandible reconstruction by free vascularized fibular flap with special emphasis on the immediate complications. Methods:The clinical data of 63 patients were reviewed, 35 male and 28 female(age range 16 to 73 years). Aesthetic and functional outcomes were evaluated by follow-up and/or telephone conversation. Results:Among the 63 fibular flaps, 29 were osteocutaneous flaps with one or two skin islands. All the flaps were surviving well and the success rate was 100%. Long term donor site disability was not recorded for any patient. Thirty-one patients responded for aesthetic and functional evaluation, 20 cases(64.5%) reported their facial appearance as excellent or good, 11 (35.48%) felt fair; 14 cases (45%) could eat unrestricted diet, 17(55%) could eat soft diet; 21 cases were(67.67%) speech normal, 10(22.33%) speech intelligible. Conclusion:Free fibular flap reflects good functional and esthetic results with a high degree of consistency, and acceptable level of complications, and we strongly believe the vascularized fibular flap is the first choice for mandibular reconstruction.

    • The preliminary research of NGF packed with liposome crossing the blood-brain barrier

      Abstract:Objective:To research the penetration to blood brain barrier(BBB) after nerve growth factor(NGF) packed with liposome. Methods:The 4.0mCi of 99mTc-NGF packed with liposomeA,4.0mCi of 99mTc-NGF packed with liposomeB and free 99mTc-NGF were injected into the rabbit by the vein of ear border respectively. SPECT was used to detect the distribution of 99mTc-NGF. And then the ratio of radioactivity counts were calculated to describe the character of distribution in brain;The NGF concentration in CSF was detected by ELISA after the NGF was injected into the rabbit at 30,60,90,120 min. Results:The results of imaging with SPECT showed that 99mTc-NGF packed with liposomeA was aggregated in brain; while 99mTc-NGF was evacuated through urinary system. In addition,99mTc-NGF packed with liposomeB was aggregated in liver;The NGF concentration in CSF showed that the NGF packed with liposome could penetrate the BBB partly. Conclusion:The ratio of brain tissue signal intensity to body signal intensity of NGF packed with liposomeA obviously exceeded that of NGF and liposomeB from beginning to terminal. The NGF packed with liposomeA could help the NGF penetration the BBB.

    • The protective effects of QiShenYiQiJinGao on isoproterenol-induced heart failure in SD rats

      Abstract:Objective:To investigate the protective effects of QiShenYiQiJinGao on isoproterenol-induced chronic heart failure(CHF) and its effects on the myocardial energy metabolism in SD rats. Methods:142 male SD rats were randomly divided into two groups:control group and CHF group. Rats in CHF group were injected subcutaneously with isopreteronol(ISO). Four weeks later,the rats in CHF group were randomly re-divided into five groups:CHF model group,trimetazidine treated group,QiShenYiQiJinGao high,middle and low dose group. After an average treatment for five weeks,echocardiogram was performed to assess the cardiac function changes of these rats. The myocardial high energy phosphates and sarcoplasmic reticulum Ca2+-ATPase(SERCA) activity were determined by enzymatic analyses. Results:Compare with the model group,FS was significantly increased(31.48 ± 5.98 vs 36.35 ± 3.80,P < 0.05) in QiShenYiQiJinGao group. Meanwhile,EF also had a trend to be improved in QiShenYiQiJinGao group. Furthermore,the level of ATP and TAN(TAN=ATP+ADP+AMP) was increased by 19% and 31%,which was similar to the control group(P > 0.05). Compare with the model group,the SERCA activity was significantly increased in QiShenYiQiJinGao and trimetazidine treated group. Conclusion:QiShenYiQiJinGao could improve the cardiac function of ISO-induced CHF in SD rats via enhancing myocardial energy metabolism.

    • Preliminary study on Herpes simplex virus type 1 infection of human oral epithelial cell in vitro

      Abstract:Objective: To explore the functions and mechanisms of herpes simplex virus type 1(HSV-1) while infecting human oral epithelial cells in vitro(being similar to the infection in vivo). Methods:An abundance of HSV-1 strains amplified in Vero cells were used to infect human oral epithelial cells. The culture supernatant was collected to infect Vero cells again. Morphology of HSV-1 was identified by inverted microscope and transmission electron microscope. Nucleic acid of the virus was detected by PCR. Results:The infected human oral epithelial cells didn’t display an obvious cytopathic effect(CPE) under inverted microscope(while Vero cells which were infected by the culture supernatant showed typical(CPE). The virus particles were not observed in the cytoplasm nor in nucleus of human oral epithelial cells, however under transmission electron microscope in the cytoplasm of Vero cells, the nucleic acid of HSV-1 could be detected in infected human oral epithelial cells, by PCR. Conclusion:HSV-1 can successfully infect human oral epithelial cells. This model may provide a useful approach for studying the pathogenesis of herpes virus-associated periodontal disease.

    • Expression,purification,and characterization of VEGF in E. Coli

      Abstract:Objective:To make recombinant vascular endothelial growth factor(VEGF) in E.Coli. Methods:The VEGF cDNA was cloned and put into the bacterial expression system PET32a(Novagen) in frame with histidine tag. After introducing into the BL21 (DE3) E coli host,recombinant VEGF165 protein was produced and purified by denaturing condition on the His-bind resin. Results:The recombinant protein was confirmed by AA sequencing. The reconstituted VEGF165 was recognized by its receptor Neuropilin-1 and could obstruct the expression of Neuropilin-1 in cell curface. Conclusion:pET32a/human VEGF165 expression vector was transfected into E.coli BL21(DE3) and produced a lot of functional recombinant human VEGF165.

    • Pathogenic effects of biofilm with chronic pseudomonas aeruginosa lung infection in rats

      Abstract:Objective: To establish an animal model of P.aeruginosa biofilm associated with chronic pulmonary infection and investigate the pathogenic effects of biofilm. Methods: Experiments in vitro, measuring the MICS, MBCS of levofloxacin(LFX), ceftazidime(CAZ) in PAO579 in alginate beads and planktonic PAO579. Rats were challenged with 0.1 ml of PAO579(109CFU/ml) in alginate beads or 0.1 ml of planktonic PAO579(109 CFU/ml), 3,7,14 days after challenging, bacteriological, pathological features were observed. Results: The MICS, MBCS of LFX, CAZ in PAO579 in alginate beads were higher than those in planktonic PAO579 in vitro. CFU/lung in alginate beads group was significantly higher than that in planktonic bacteria group(P = 0.002, P = 0.004, P = 0.002, respectively); macroscopic lung pathology and the inflammation in alginate beads group were significantly more severe compared to those in planktonic bacteria group in vivo. Conclusion: P.aeruginosa biofilm protected bacterium from killing of antibiotics and might mediate the host immune damage in the lung tissue and made bacterium evade the host immune defense.

    • Identification of class 1 integron carrying novel gene cassettes in Pseudomonas aeruginosa isolates

      Abstract:Objective:To investigate the distribution,characterization of class 1 integron and its action in multi-resistance in Pseudomonas aeruginosa. Methods:PCR and DNA sequencing methods were undertaken to clarify the type and context of variable region of class 1 integron;the susceptibility of class 1 integron positive strains to five represented antibiotics was determined by agar dilution method;conjugation experiment was carried out to disclose its transferable property;and lastly,pulsed-field gel electrophoresis pattern was performed to explain the genetic relationship of those class 1 integron positive Pseudomonas aeruginosa. Results:Of 47 isolates,29 strains of Pseudomonas aeruginosa were found containing class 1 integron,and no class 3 integron was detected. A length of 4.300 bp variable region was found in all the class 1 integron positive strains by PCR amplification. The resulting DNA sequences revealed that it was comprised of a new array of aac(6’)-Ⅱ-aadA13-cmlA8-oxa-10 gene cassettes,and the genbank number is EU182575. According to the database in genbank,aadA13 was previously unreported in Pseudomonas aeruginonsa; chloramphenicol efflux pumping determinant,named as cmlA8 shares 95% nucleotide similarity to cmlA5 gene. All those class 1 integron positive isolates showed high homology by PFGE pattern analysis after digestion with SpeⅠ. Conclusion:Class 1 integrons disseminated among correlated Pseudomonas aeruginosa in our hospital possessing novel gene cassettes and mediated the resistance and multi-drug resistance.

      Abstract:Objective:To investigate the effects of calcium-activated chloride (ClCa) channels on proliferation of pulmonary artery smooth muscle cells(PASMCs) in rats under chronic hypoxic condition. Methods:The cultured PASMCs were placed under normoxic and chronic hypoxic conditions:The cells were observed by light and electron microscope; The cell cycles were observed by flow-cytometry; Immunocytochemistry staining was used to detect the expressions of PCNA, c-fos and c-jun of PASMCs; Cytoplasmic free Ca2+ con-centration ([Ca2+]i) in PASMCs was investigated by fluorescent quantitation using fluorospectrophotometer. Results:The PASMCs were contractile phenotype under normoxic conditions. Observation by transmission electron microscope: In kytoplasm of contractile phenotype cells, myofilament bundles were abundant and the content of cell organs such as Golgi’s bodies were rare. The PASMCs were synthetic phenotype under chronic hypoxic condition. There were increased free ribosomes, dilated rough endoplasmic reticulums, highly developed Golgi complexes, decreased or disappeared thick filaments and dense body in kytoplasm of synthetic phenotype cells. After NFA and IAA-94, the situations were reversed The number of S+G2M PASMCs were significantly increased in chronic hypoxic condition; The NFA and IAA-94 were shown to significantly decrease them from (28.6±1.0)% to (16.0±1.6)% and the number of G0G1 PASMCs significantly increased from (71.4±1.9)% to (83.9±1.6)% (P < 0.01). In chronic hypoxic conditions, the expression of proliferating cell nucleus antigen was significantly increased; The NFA and IAA-94 were shown to significantly decrease it from (81±6)% to (27±7)%(P < 0.01). The expression of c-fos and c-jun were significantly increased in chronic hypoxic conditions; The NFA and IAA-94 were shown to significantly decrease them from 0.15±0.02, 0.32±0.05 to 0.05±0.01, 0.12±0.05, respectively (P < 0.01); Under chronic hypoxic conditions, [Ca2+]i was increased; The NFA and IAA-94 decreased it from (281.8±16.5)nmol/L to (117.7±15.4)nmol/L(P < 0.01). Conclusion:Hypoxia initiated the change of PASMCs from contractile to synthetic phenotype and increased proliferation of PASMCs. NFA and IAA-94 depressed cell proliferation by blocking ClCa channels in hypoxic condition. These may play an important role in proliferation of PASMCs under chronic hypoxic conditions.

    • Effect of rat proinsulin gene therapy by intramuscular injection on blood glucose in streptozotocin-induced diabetic rats

      Abstract:Objective:To study the effect of rat proinsulin gene therapy by intramuscular injection on blood glucose and insulin secretion in streptozotocin-induced diabetic rats. Methods:Recombinant eukaryotic cell expression plasmid of rat proinsulin gene pCMV/proinsulin was transferred into streptozotocin-induced diabetic rats by intramuscular injection. The treatment group(Group A) was injected with pCMV/proinsulin naked plasmid DNA 100 μg,whereas the corresponding control group(Group B) was treated with similar amount of pCMV DNA. Normal group and DM group were also observed at the same time. Blood glucose levels and plasma insulin levels were determined by hypersensitive radioimmunoassay. RT-PCR was used to determine the alteration of proinsulin mRNA expression in skeletal muscles and pancreas. Sections of skeletal muscles and pancreas in each group were also observed. Results:①The blood glucose level in Group A was decreased about 4 mmol/L and maintained for 1~2 weeks. The serum insulin level increased significantly after pCMV/proinsulin gene therapy. The blood glucose in Group A was significantly lower than Group B and DM Group(P < 0.05),while serum insulin was higher than that of two groups(P < 0.05). ②Proinsulin mRNA expression could be detected in skeletal muscle cells of Group A,but no detected in that of Group B and DM Group. It was also showed that proinsulin mRNA expression was higher in pancreas of Group A than Group B and DM Group. ③Immunohistochemistry studies showed that insulin staining was positive in the sections of skeletal muscles of Group A,whereas the same results were not obtained in Group B and DM Group. And insulin staining was more intensive in pancreas of Group A than that of Group B. Conclusion:Direct injection of naked plasmids encoding rat proinsulin gene intramuscularly led to the reduction of blood glucose level,suggesting that proinsulin gene therapy offers a potential approach to diabetes.

      Abstract:Objective:To study the Cryptosporidium infection among young children in Anhui province, and to explore the best way to diagnose the disease. Methods: Stool specimens of 1 204 children were collected; oocysts of Cryptosporidium were identified with auramine O-modified staining, acid-fast staining, safranine T and methylene blue staining, and auramine O-modified acid-fast staining. Results: The detectable rate of Cryptosporidium in four stainings were respectively 2.46%, 1.50%, 1.98% and 3.46%, and there was a higher significant difference in the rate between auramine O-modified acid-fast staining and the others(P < 0.005). The detectable rate was significantly lower in urban children(2.14%, 15/684) than in rural ones(5.19%, 27/520). Boys and girls had similar detectable rate (1.99%, 24/1 204 vs. 1.50%, 18/1 204). Cryptosporidium infection was usually subclinical, and its major clinical features included benign diarrhea, mild abdominal pain and nausea. Conclusion:Cryptosporidium infection was relatively common in kindergartens and a higher infection rate was found in rural children. As the majority of the Cryptosporidium infections were subclinical, diagnosis is important although difficult.

      Abstract:Objective: To observe the effects of controlled hypotension on gastric intramucosal pH and post-operational gastrointestinal functions using two specific anesthesia methods. Methods: Thirty patients(ASAⅡ)scheduled for ectomy of hepatocarcinoma, were randomly assigned to two groups: epidural block combined with intravenous anesthesia group(E group) and inhalation anesthesia group(G group). Gastric PgCO2 and pHi were monitored at different time points, before theintravenous induction of controlled hypotension, after 1 h and 2 h, and 1 h after the termination of controlled hypotension. In the meanwhile, the artery blood gas was analyzed. Results: There was no significant difference in blood gas indexes between E group and G group. However, pHi decreased significantly after 1 h and 2 h of controlled hypotension(P <0.05), and during the same periods PgCO2 increased significantly(P <0.05 or P <0.01), the time of bowel movement and defecating deferred significantly shorter in G group patients, when compared with E group patients. Conclusion:Epidural block in combination with general anesthesia can improve gastrointestinal blood flow during controlled hypotension and facilitates post-operational recovery of gastrointestinal functions.

      Abstract:Objective:To construct and certificate eukaryotic expression plasmid carrying human HO-1 gene. Methods:By molecular biological technique,total RNA was extracted from human liver. After RT-PCR and TA cloning,the plasmid of pMD/19-HO-1 was successfully constructed. Then the plasmid was digested with restriction enzymes BamHⅠand EcoRⅠ. Then the human HO-1 gene fragment was cloned into plasmid pcDNA3.1 to construct the recombinant pcDNA3.1-HO-1 plasmid. pcDNA3.1-HO-1 was verified by restriction digestion and gene sequencing,The constructed plasmid were transfected into ECV304 cell line and certificated by Western blot analysis. Results:Identification of pcDNA3.1-HO-1 by restriction digestion and sequencing showed that the target gene was inserted into pcDNA3.1 correctly; the sequence of the target gene by DNA sequencing was identical with that of NCBI gene bank; the plasmid could increase the expression of human HO-1 obviously. Conclusion:pcDNA3.1-HO-1 was constructed successfully,which will help to study the effect of human HO-1 on vasculopathy in diabetes mellitus.

      Abstract:Objective:To investigate the effect of glucose and glucocorticoid on the expression of 11β-HSD1,11β-HSD2 gene transcriptions in PIEC in vitro,and the relationship between 11β-HSD1 and transforming growth factor-β1 and Endothelin-1. Meanwhile,to observe the effect of 11β-HSD1 and 11β-HSD2 antisense oligonucleotides on the expression of 11β-HSD1 and 11β-HSD2 by PIEC under hyperglycemia. Methods:PIEC were cultured in medium in vitro under different concentration of glucose(5.6 mmol/L,10.0 mmol/L,20.0 mmol/L,30.0 mmol/L)and in different concentration of glucocorticoid and antisense oligonucleotides under hyperglycemia. RT-PCR was applied to determine the expression of 11β-HSD1,11β-HSD2,TGF-β1 and ET-1 mRNA. Western-blot was performed to measurer the expression of 11β-HSD1 and 11β-HSD2. Results:After treatment of different concentration of glucose for 48 h,the expression of 11β-HSD1 and TGF-β1 mRNA under high glucose condition were significantly increased and represented a certain correlation;high glucose can increase the expression of 11β-HSD1 protein. Both mRNA and protein of 11β-HSD1 and 11β-HSD2 were blocked by 11β-HSD1 and 11β-HSD2 antisense oligonucleotides;meanwhile,the expression of TGF-β1 mRNA was decreased. Conclusion:High glucose and glucocorticoid could increase the expression of 11β-HSD1 mRNA and protein,there was a certain linear correlation between the expression of 11β-HSD1 and TGF-β1 mRNA. 11β-HSD1 might have a correlation with diabetes mellitus.

      Abstract:Objective: To construct the recombinant adenovirus vector carrying rat vascular endothelial growth factor(VEGF), as preparation for genetic transfection that follows. Methods:Rat VEGF was obtained by using RT-PCR amplification and then cloned into the shutter plasmid pDC316. Subsequently, this newly constructed plasmid pDC316-VEGF, after identification by nuclease digestion analysis and sequencing analysis, was transfected into human embryonic kidney cells HEK293 by Lipofectamine 2000 mediation, together with adenovirus-packaging plasmid pBHGE3. Based on the homologous recombination of the two plasmids within HEK293 cells, the recombinant adenovirus vector carrying VEGF and VDC316-VEGF was created. VDC316-VEGF was subsequently identified using PCR, purified using repeated plaque passages, proliferated using freezing and melting within HEK293 cells, and titrated using 50% Tissue Culture Infective Dose(TCID50) assay. Results:The newly constructed recombinant adenovirus was confirmed to carry rat VEGF based on PCR results, and its titration value determined based on TCID50 assay was 3×109 pfu/ml. Conclusion:The recombinant adenovirus carrying rat VEGF was successfully constructed. The newly constructed adenovirus can produce a sufficiently high titration value within HEK293 cells, providing a reliable tool for genetic transfection in further gene therapy researches.

    • A investigation of insulin sensitivity and β cell functions in subjects with or without MS according to different diagnosis standards

      Abstract:Objective:To investigate the correlation between metabolic syndrome(MS) and insulin resistance in Chinese,discuss the applicability of ATPⅢ criteria and IDF criteria for MS in Chinese people. Methods:Given 75 g oral glucose tolerance challenge,110 Chinese were divided into normal glucose tolerance(NGT) group, impaired glucose tolerance(IGT) group and type 2 diabetes (DM) group according to 1999 WHO criteria. Homeostasis model assessment (HOMA) was applied to assess the status of insulin resistance(HOMA-IR) and basic function of pancreatic -茁 cell(HOMA--茁 cell). Results:In subgroups with and without MS according to the ATPⅢ criteria, no significant difference was found in HOMA-IR(1.50 ± 1.84,2.15 ± 2.50,P = 0.13) and HOMA--茁 cell(0.90 ± 0.56,0.96 ± 0.94,P = 0.68). However,in subgroups according to IDF criteria,significant difference was found in HOMA-IR(1.25 ± 0.83,2.22 ± 2.81,P = 0.01). Conclusion:There was no distinct insulin resistance in the metabolic syndrome group defined by the 2001 ATPⅢ criteria. The ATPⅢ criteria for MS was not coherent with the 1999 WHO criteria. The ATPⅢ criteria for MS might not have enough applicability in Chinese patients, while the IDF criteria might be more applicable in Chinese people.

      Abstract:Objective:To study the relationship between perifollicular blood flow and follicule development, oocyte maturing rate, fertilizing rate, cleaving rate, embryo quality and the outcomes of embryo transfer. Methods:The samples were selected from 66 suffers who underwent in vitro fertilization(IVF)or intracytoplasmic sperm injection(ICSI). Eeach patients perifollicular blood flow(diameter ≥12mm )was estimated on the day of human chorionic gonadotropin(HCG)administration. Results:Among 66 cycles,26(39.4%) cycles resulted in pregnancy. perifollicular blood flow resistance index(RI), peak systolic velocity/end diastasis velocity(S/D) of non-pregnant group was significantly higher than that of the pregnant group(P < 0.004). When RI<0.49, the pregnancy rates, fecundation rates, fertilization rates, metaphase numbers for the of second meiosis oocytes increased evidently(P < 0.05),but there were no statistical difference in gonadotropin dosage, cycle frequency, infertility years, ages, estradiol(E2)on the day of HCG administration, numbers of oocyet retrieved and high-quality embryo rates(P > 0.05). There were no statistical difference between non-pregnant group and pregnant group in S and D(P > 0.05). There was no correlation between periFollicular blood flow RI and follicular diameter by linear regression analysis. Conclusion:Our study shows that perifollicular blood flow RI and S/D are effective indices of predicting the pregnancy outcome of IVF-ET.

    • The relationship among apolipoprotein E genetic polymorphism, carotid atherosclerosis and the related inflammatory factors

      Abstract:Objective:To explore the relationship among apolipoprotein E(ApoE) genetic polymorphism,the carotid atherosclerosis(CAS) and the produce of related inflammatory factors. Methods:Total 83 patients were examined by B-mode ultrasound to measure intima-media thickness(IMT) of the common carotid artery and plaque formation. According to the results of B-mode ultrasound examination,the patients were divided into the carotid atherosclerosis group and control group. The polymerase chain reaction restriction fragment length polymorphism technology(PCR-RFLP) was used to detect ApoE genotype; Serum triglyceride(TG),total cholesterol (TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C) and inflammatory factors were measured. Results:The frequency of ε3/4 in CAS group was higher than that in control group(P < O.05). The values of TC,LDL-C,TG,carotid IMT,Hs-CRP and fibrinogen among different groups of ApoE allele were significantly different(P < O.05),and the value of these parameters in the group carrying ε4 allele ranked the highest among all genetic polymorphism. Conclusion:ApoE genetic polymorphism was one of the important genetic factors affecting serum total cholesterol and LDL-C level. Meanwhile correlating with the state of inflammation in vivo,ApoE genetic polymorphism also affected CAS process in a certain extent.

      Abstract:Objective:To evaluate the clinical value with positron emission tomography/computerized tomography(PET/CT) imaging for the detection of vulnerable plaque in atherosclerotic lesions. Methods:Sixty people with a age of over 60[mean age (69.2±7.1)years] underwent three dimension(3D) whole-body fluorine-18-2-fluoro-2-deoxy-D-glucose(18F-FDG) PET/CT imaging and were evaluated retrospectively, including 6 cases assessed as normal and 54 cases with active atherosclerotic plaque. Fifty-four cases with SUVs and CT values in the aortic wall of high-FDG-uptake were measured retrospectively. These high-FDG-uptake cases in the aortic wall were divided into three groups according their CT value. Cases in group 1 had high uptake in atherosclerotic lesions of the aortic wall with CT value of less than 60 Hu(soft plaque). Cases in group 2 had high uptake with CT value between 60-100 Hu (intermediate plaque). Cases in group 3 had high uptake with CT value more than 100 Hu(calcified plaque). Group 4 was normal. Results: In group 1, there were 42 high-FDG-uptake sites (average SUV 1.553±0.486). In group 2, there were 30 high-FDG-uptake sites(average SUV 1.393±0.296). In group 3, there were 36 high-FDG-uptake sites(average SUV 1.354±0.189). In group 4, there were 33 normal-FDG-uptake sites (average SUV was 1.102±0.141). The SUVs showed significant difference among the four groups(F = 678.909, P = 0.000). There were also significant difference found between the normal-FDG-uptake group and the high-FDG-uptake groups(P = 0.000, 0.000, 0.001, respectively). Conclusion:Different degrees of 18F-FDG uptake in active large atherosclerotic plaque were shown in different stages of atherosclerotic plaque formation. The soft plaque had the highest FDG uptake in this study. This suggested that 18F-FDG PET/CT imaging may be of great potential value in early diagnosis and monitoring of vulnerable soft plaque in atherosclerotic lesions.

    • Current state of treatment and management of diabetic patients in the outpatient clinic

      Abstract:Objective:To investigate the current state of treatment and management of diabetic patients in the outpatient of our hospital. Methods:Outpatients treated for 12 months or longer were enrolled in our study. Demographic data,treatment information,routine tests,and chronic complications were collected through outpatient interview and case history investigation. Results:Total 257 patients were enlisted,whose average age was(61.8 ± 12.3) years and HbAlc average value was(7.3 ± 1.5)%. The standard-meeting rate of blood glucose,blood pressure and lipid profile was higher than those of investigation results in 2003 China(P < 0.05). Cataract,diabetic neuropathy and microalbuminuria ranked top three complications among all ones. Major factors affecting the standard-meeting rate of blood glucose were smoking,diet,exercise,education,duration of diabetes and level of literacy. Conclusion:The standard-meeting rate of blood glucose,blood pressure and lipid profile among diabetic patients in our outpatients in 2005 was higher than the national average level in 2003. 72.7% of diabetic patients failed in achieving optimal glycemic control. Thus,it is still important to promote education on diabetes to prevent the advanced complications.

      Abstract:Objective: To make intensified analysis upon the existing difficulties in early detection for the infectious diseases outbreak in China and put forward some effective measures to improve it. Methods: Field investigation and in-depth interview were applied in 23 interviewees from different-level CDC. Results:The study findings suggest that although the timeliness of outbreak detection has been improved since SARS epidemic in China, some problems still exist, such as: ①lacking syndrome surveillance; ②report quality of infectious disease should be improved; ③difficulties of identifying the cases at lower social economic status; ④a definite financial compensation mechanism for the operation of web-based report system is absent; ⑤insufficient information exchange among different branches. Conclusion:Some effective measures must be taken including developing syndrome surveillance system; affording more education and training for related staff; increasing payout on surveillance and establishing an information exchange platform for early detection of disease outbreak.

    • The expression of CD28 on T-lymphocyts of the peripheral blood in the elderly patients with lung cancer

      Abstract:Objective:To investigate the change of the expression of CD28 on T-lymphocyts of the peripheral blood in the elderly patients with lung cancer. Methods:Three-color fluorescent flow cytometry were used to analyze the percentage of peripheral blood T-lymphocyts and T cell subsets in both of 35 normal elderly persons and 32 elderly patients with lung cancer. Results:CD3+-CD4+-CD8+-CD4+CD28+-CD8+CD28+T cells in the peripheral blood of the elderly patients with lung cancer were significantly lower than those in the normal elderly persons(P < 0.01);CD8+CD28-(P < 0.01) T cells in the peripheral blood of the elderly patients with lung cancer were significantly higher than those in the normal elderly persons.However there was no significant difference between two groups of CD4+CD28- T cells. Conclusion:Elderly group patients with lung cancer had a significantly lower cellular immunity dysfunction and anticancer ability than those in normal elderly persons,which led to their illness worse.

      Abstract:While facing the current situation of supply and demand contradiction of teaching equipment and lecturing method, the authous intended to give some suggestion of establishing opening laboratory for students after-class usage, which was just based on interesting of students and need of experiments, introducing PBL model in experiment teaching to establish the problem and medical record based learning modality. The article attempted to educate and train medical talents with creativity and all-round development by establishing opening laboratory and introducing of PBL teaching model.

      Abstract:Objective:To observe effects of salviamiltiorr hizabge on inflammatory cytokine in acute necrotic pancreatitis(ANP) in rats. Methods:The acute necrotic pancreatitis rat model was constructed while the Salviamiltiorrhizabge was administrated synchronously. ELISA was then applied to test the concentration of TNF-α and IL-1β and TGF-β in the serum. Results:On the onset of ANP(3 h),TNF-α and IL-1β were richly produced while TGF-β was obviously secreted until the 6 h after the model construction. Salviamiltiorrhizabge inhibited the production of TNF-α and IL-1β,and the expression of TGF-β at 12 h. Conclusion:Salviamiltiorrhizabge could significantly inhibit the secretion of the pro-inflammation cytokine of TNF-α and IL-1β,and the production of TGF-β. It might be involved in the therapeutic mechanism of Salviamiltiorrhizabge in acute necrotic pancreatitis in rats.

      Abstract:In order to comply with the internationalization trend of medical education, and establish student training criteria and complete supporting facilities with the purpose of guaranteeing and improving the quality of medical education in China, the article made a comparison on cultivating objectives and target student selection of eight-year-system medical education between the US and China. Through analysis of advantages and disadvantages, the authors provided some advices on reform and development of eight-year-system medical education from perspectives of conception change, target student selection, identification and further detail of cultivating objectives and etc.

    • The construction of eukaryotic expression vector carrying CD2AP and its cellular expressing localization in COS-7 cells

      Abstract:Objective:To construct the eukaryotic expression vector of pEGFP-CD2AP and detect whether it could express in COS-7 cells. Methods:The coding sequence of CD2AP was obtained by PCR,then the PCR product was insert into pGEM T-Easy vector. After DNA sequence analysis,CD2AP was subcloned into pEGFP-C2 vector. Finally,a eukaryotic expression vector was obtained. The pEGFP-CD2AP expression vector was transfected with lipofectamine 2000 to COS-7 cells,and the expression of pEGFP-CD2AP was determined by GFP fluorescence. Results:The pEGFP-CD2AP expression vector could express both EGFP reporter gene and CD2AP in COS-7 cells. Conclusion:The pEGFP-CD2AP expression vector was constructed successfully,and it could be expressed in COS-7 cells. This work provided foundations for further study on the function of CD2AP.

    • Effects of antenatal dexamethasone on rat fetal lung morphogenesis

      Abstract:Objective:To compare the effects of different dosage of dexamethasone(DEX) on rat fetal lung morphogenesis. Methods:Fifteen pregnant rats were divided into five groups with 3 rats in each group randomly:small dosage DEX single and multiple course groups,large dosage DEX single and multiple course groups and control group. On the 19th day of gestation,cesareans were taken out,and the histologic structures of 6 fetal rat lungs in each pregnant rat were observed with light microscope and electronic microscope. Results:Under the light microscope,fetal rat lung in the four treatment groups had fewer alveolar numbers,larger alveolar space,and thinner alveolar septum compared with control group(P < 0.01). With the increased dosages and times of the DEX,the fetal rat lungs have fewer alveolar numbers,larger alveolar space,and thinner alveolar septum(P < 0.01). Under the electronic microscope,lamellar body and cellular organs such as bioblast in endochylema of the four treatment groups were more obvious compared with the control group. But basal membrane of lung epithelial cells turned out to be discontinuous and different thickness.Conclusion:Using DEX antepartum could promote fetal lung morphogenesis obviously. But with the dose and course increased,the development alveoli was inhibited and the basal membrane was damaged.

    • Research on immune status of people infected by HIV without clinical therapy and AIDS patients in Jiangsu province

      Abstract:Objective:To discuss the variety of T-lymphocytes and the combining TB infection of the HIV infected people in Jiangsu province to provide the reference for anti-virus treatment. Methods:The T-lymphocytes subsets from the samples of 140 normal people and 116 HIV infected people were analyzed by flow cytometry. The combining TB infection situation of the HIV infected people were detected by X-ray and sputum examination. Results:The total count of CD4+ T-lymphocytes was lower in HIV infected people than that of normal people(P < 0.001). CD4+/CD8+ ratio of all infected people was obviously lower than that of normal people(P < 0.001). 37.07% infected people had CD4+ T-lymphocytes count lower than 200 -滋l. The CD8+ T-lymphocytes counts and the Th/Ts of the two groups had significant difference. 90 of the 116(77.59%) HIV infected people didn’t have clinical symptoms and the others appeared related clinical symptoms. The CD4+ T-lymphocyte count and the ratio of the Th/Ts between two groups had significant difference. 11 HIV infected people were also infected by TB. Conclusion:When the object was infected by HIV, the number of CD4+ T-lymphocyte count and CD8+ T-lymphocyte count were firmly correlated with the course of disease. Both CD8+ and CD4+ testing was superior to single testing.

      Abstract:Reproductive Medicine was one of the integrative subjects, which developed rapidly in recent years. This subject was greatly hopeful in this century. Seven-year-system medical students were senior medical talents with master degree. Nanjing Medical Unicersity set up reprodutive medicine as a required course for them. Through utilizing mutlimedia courseware and PBL (problem based learning) method, considering timely supplement and renewal of related knowledges, emphasizing the training of thinking ability,research ability and humanity quality, a good teaching effect had been achieved.

    • Isolation and genotyping of wild-type measles in Jiangsu province

      Abstract:Objective:To survey the current prevalence strain of wild measles virus in Jiangsu Province. Methods:In May 2005,the throat swab samples of the suspected measles patients in hospital and measles outbreak spot were collected,and inoculated into B95a cell derived from EB virus. Strains of wild measles virus were gottern. Results:RT-PCR and the sequence analysis of its product suggested that these 58 strains of measles viruses should belong to the H1 genotype of the eighth genetic group of wild measles virus. Conclusion:The genotype of wild measles virus isolated from Jiangsu province was the same as the dominant genotype prevalence in China.

    • Evaluation of ultrasonography,CT and MRI in diagnosing orbit diseases

      Abstract:Objective:To evaluate the role of ultrasonography(US) in diagnosing orbit diseases,compared with CT and MRI.Methods:The image datum of 84 cases were retrospectively analyzed and correlated with postoperative pathology results. Results:On physical diagnosis,US and CT/MRI showed no significant difference. When correlated with pathology,the coincidences of US and CT/MRI were:solid lesion,92% and 89.3%;cystic lesion,100% and 90%;inflammatory lesion,100% and 95.7%;vascular lesion,100% and 77.8%. Conclusion:On intra-orbit lesions,US is superior to CT/MRI,because it is cheap,safe,sensitive and real-time.However,CT/MRI might be better,if the lesion intrudes the eye sockets.

    • The correlate study of the gestational age and the fetal cerebellum volume measured by virtual organ computer aided analysis of three-dimensional ultrasound

      Abstract:Objective:To measure fetal cerebellum volume by using virtual organ computer aided analysis(VOCALTM) of three-dimensional ultrasound. Methods:176 healthy pregnant women with gestational age between 19 to 38 weeks were enrolled in this study. Using two-dimensional mode to find the plane of transverse cerebellum diameter,then changed to the three-dimensional mode.The fetal cerebellum was scaned by three-dimentional ultrasound,and the volume would measured by the program of VOCALTM. Results:The fetal cerebellum volume changed with gestational age. The cerebellum volume increased from 1.258 cm3 at 19 weeks to 18.913 cm3 at 38 weeks in average,R2=0.9938. Conclusion:The fetal cerebellum volume could be measured by virtual organ computer aided analysis of three-dimensional ultrasound. The fetal cerebellum volume changed with the gestational age.
