      Abstract:This article focused on the important meaning of the humanistic spirit of traditional Chinese culture to the reconstruction of medical ethics. The humanistic spirit of traditional Chinese culture was analyzed from three aspects: the “benevolence” in Confucius,the “compassion” in Buddhism,and the “dialectics” in Taoism,with a purpose of seeking its matching point to medical ethics. At last,five ways of how to cultivate medical humanistic spirit through traditional Chinese culture had been discussed.

According to the analysis of the present investigation,health education in urban community health service had already founded in some way,and also had many problems that need to solve in an effective way,such as rules,multiple cooperation,funds,personnel and the work efficiency,so as to give full play to the urban community health services in health education.

      Abstract:Objective:To provide the evidence to optimize the management of hospital charges reasonably and to solve medical-treatment problems of rural residents through analyzing the characteristics and the influencing factors of inpatients’ hospitalization expenditure in township hospitals of Jiangsu province. Methods:Cluster sampling method was used to investigate objective data of inpatients of 4 township hospitals. Statistic descriptions of hospitalization expenditure,curve estimates of hospitalization period and expenditure,ANOVA and Kruskal-Wallis test of influencing factors were handled by SPSS 13.0 soft for windows. Results:Large parts of hospital charges were medicine fee. Admission day got straight linear correlation with hospitalization expenditure. Age and the treatment effect had significant influence on the hospitalization expenditure and gender or section had not. Conclusion:Improving medical quality of township hospitals,shortening hospitalization time,regulating medicine charge,and establishing a perfect medical insurance system should be engaged to reduce hospitalization expenditure.

      Abstract:Objective:To investigate and analyze the human resources of Shuyang township hospitals,discuss the responsibility and measures of the government in improving,cultivating and importing human resources. Methods:The questionnaire was used to integrate the total amount,education and professional title of human resources. Results:The basic situation of Shuyang township hospitals showed that the amount of the human resources was inadequate and the quality was also unsatisfied after reform. Conclusion:There was less effective improvement in situation of human resources of Shuyang township hospitals after reform. Government and relevant departments should design a health human resources planning of township hospitals,construct a platform of developing talents,improve the existing health personnel in township hospitals,and urge the township hospitals to formulate a good program of orientation and training health workforce.

      Abstract:Objective: To investigate the positive role which played by the intraoperative music therapy that combined with psychological intervention to reduce psychological stress,relief the surgical pain and cut down the body’s stress station. Methods: Sixty cases of selective cesarean section were randomly divided into two groups:the cases of control group were received routine observation and nursing care,the intervention group cases were provided psychological care interventions and intraoperative music therapy besides the measures of the control group. Anxiety scores(SAS),the divergence of plasma cortisol(Cort),glucose(Glu),blood pressure,heart rate,pain scores(VAS)levels were observed before and after the intervention. Results: SAS(self anxiety scores)in both groups were no significant differences before the intervention and the intervention group reduced anxiety,lower than the control group(P < 0.05). In the level of Cort,Glu and VAS,the intervention group was more positive than the control group(P < 0.05). Heart rate,blood pressure and Apgar score showed no significant difference between the two groups. Conclusion: The intraoperative music therapy combined with psychological intervention can relieve stress in patients with cesarean section.

      Abstract:Objective: To investigate the mode of blood purification inservice training for specialized nurses,and cultivate clinical nursing specialist in blood purification. Methods: For the qualifications of trainee and training objectives,training bases designed curricula specially combining the reported experience home and abroad and expert’s argumentation,and trained 25 participants enrolled in 2008,2009 according to the curriculum. Results: 22 of 25 trainees well assessed by the experts evaluation with passing rate of 88%. Conclusion: Through the training,our hospital blood purification centre has achieved good results and futher accumulated the experience of training for specialist nurses.

    • GIT1 RNA hairpin interferes with MLC distributing and decreases MLC phosporylation in osteoblasts

      Abstract:Objective:To study the effect of G protein coupled receptor kinase interacting protein 1(GIT1) RNA hairpin (GIT1-RNAh) on Myosin light chain(MLC) distribution and phosphorylation in osteoblasts. Methods:The sixth passage osteoblasts were divided into two groups infected with GIT1-RNAh (experimental group) and Green Fluoresence protein RNA hairpin (GFP-RNAh) (control group) adenovirus for 12 hours respectively. Each group was further classfied into two groups according to PDGF stimulation or not. MLC distribution and phosphorylation were analysized by immunoflurescence stain and western blot respectively. Results:Immunoflurescence stain results showed that the GIT1-RNAh significantly interfered with MLC distribution. Western Blot results showed that MLC phosporylation was dramatically inhibited in osteoblasts infected with GIT1-RNAh adenovirus after PGDF stimulation (P < 0.05). Conclusion:GIT1-RNAh overexpression interfered with MLC distribution and decreased MLC phosphorylation.

    • Effects of ischemic postconditioning on expression of liver nitric oxide synthase in rat and its significance

      Abstract:Objective:To investigate the effects of ischemic postconditioning on expression of liver nitric oxide synthase(NOS) in rat. Methods:The model of 70% hepatic ischemia reperfusion was constructed in SD male rat. Thirty two SD rats were randomly divided into three groups:sham group (SO group,8 rats),ischemia reperfusion group (IR group,12 rats) and ischemic postconditioning group (IPO group,12 rats). In group IPO,rats were treated as IR group,but at the onset of reperfusion,reflow was initiated with 10 s of full blood flow,followed by 10 s of re-occlusion,and repeated six times. After reperfusion for 3 hours,changes of ALT,AST and NO in different groups were compared. Reverse transcription-polymerase chain reaction and western blot were used to detct the expressions of NOS mRNA and protein respectively. Histopathological changes were observed by light microscope. Results:Compared with the SO group,ALT,AST and NO in serum were significantly higher (P < 0.01) than those in the other groups,and the mRNA and protein expressions of NOS were also obviously increased. Compared to the group IR,the levels of ALT,AST were significantly lower (P < 0.01),while NO was significantly higher(P < 0.01) in the IPO group. The expressions of NOS mRNA and protein were increased in the IPO group. Under light microscope,the group IR and IPO both showed typical pathological changes of ischemic-reperfusion injury,but the injury was extenuated in the IPO group. Conclusion:Ischemic postconditioning can protect liver from ischemic-reperfusion injury. The mechanism might be related with the induction of eNOS in tissue and the increase of NO in serum.

    • Effects of high glucose on apoptosis and expression of apoptosis-related genes in neonatal rat cardiomyocytes

      Abstract:Objective:To investigate the effects of high glucose on apoptosis and expression of apoptosis-related genes in neonatal rat cardiomyocytes. Methods:Cultured cardiomyocytes were exposed to high glucose (20 mmol/L) for 48 h. Laser confocal microscopy, TUNEL assay and flow cytometry were utilized to detect cardiomyocyte apoptosis. Real-time PCR was performed to identify mRNA expression of apoptosis-related P53,Pax3, Bmp4,Slug and Msx1 genes in the glucose group and the control group. Results:Cardiomyocyte apoptosis was obviously observed in the glucose group under the laser confocal microscope. TUNEL assay indicated that cardiomyocyte apoptotic index in the glucose group was significantly higher than that in the control group (P < 0.05). Flow cytometry revealed that cardiomyocyte apoptotic rate in the glucose group was significantly higher than that in the control group (P < 0.05). Real-time PCR Showed that the expressions of P53 and Msx were up-regulated in the glucose group while the expressions of Pax3, Bmp4 and Slug were down-regulated. Conclusion:High glucose can disrupt apoptosis-related genes’ expression, including P53,Pax3, Bmp4,Slug and Msx1, which may be related to the cardiomyocyte apoptosis.

    • Effects of curcumine on expression of MCP-1 induced by H2O2 in myocardial cell

      Abstract:Objective:To investigate the effects of curcumine on expression of monocyte chemoattractant protein-1(MCP-1) induced by H2O2 in myocardial cell. To explore the possible mechanism of antioxygenation of curcumine in oxidative stress. Methods:In order to gain the oxidative injure cell model,cells myocardial with H2O2. Myocardial cells were incubated with curcumin and H2O2,then the expression of MCP-1 at different time points during the oxidative injure was detected by western blotting. Results:Western blotting showed that MCP-1 expressed in all groups. Higher level of MCP-1 expression was observed in H2O2 stimulated group compared with that in control group(P < 0.05). After treated with curcumin,the expression of MCP-l in H2O2+ curcumine group was reduced(P < 0.05). Conclusion:The expressions of MCP-1 induced by H2O2 can be inhibited by curcumine,and it may be one of the mechanisms of antioxygenation effect of curcumine.

    • Study on the signal pathway of EGF and TGF-α modulating the distribution of cell cycle in tongue squamous cell

      Abstract:Objective:To investigate the signal pathway of EGF and TGF-α modulating the distribution of cell cycle in tongue squamous cell. Methods:Flow cytometric analysis and immunohistochemistry were employed to investigate the cell cycle distribution,and expressed levels of p38-MAP kinase,c-myc,c-fos and NF-κB respectively. Results:EGF and TGF-α showed similar effect on tongue squamous cell,but the dose-effect relationships were different. In comparison with the control,the proportion of cells in G1/G0 declined and that in S phase ascended. The levels of p38-MAP kinase,c-myc and c-fos were down-regulated and NF-κB p65 up-regulated. EGF and TGF-α up- and down-regulated the expressed levels of NF-κB p50 respectively. Conclusion:The distinct expression of NF-κB may be one of the reasons that results in the difference of EGF and TGF-α on the cell cycle distribution. NF-κB is closely related to the occurrence of tongue squamous carcinoma.

    • Effects of prone position ventilation combined with recruitment maneuvers on oxygenation and lung recruited volume in canines with acute respiratory distress syndrome

      Abstract:Objective:To evaluate the effects of prone position ventilation combined with recruitment maneuvers(RM) on oxygenation and lung recruited volume(RV) in oleic acid-induced acute respiratory distress syndrome in canines. Methods:Twenty-four healthy canines were randomly divided into four groups after oleic acid-induced lung injury:supine position group(SC),supine position+RM group(SS),prone position group(PC) and prone position+RM group(PS). Oxygenation index and RV were measured for five hours after ARDS was reproduced. Results:(1)Compared to the SC group,oxygenation index in SS group was increased only at 2 h after onset of ARDS(P < 0.05),while that of PC and PS group improved both at 2 h and 5 h after onset of ARDS(all P < 0.05). Compared to the SC,SS,and PC group,PS group oxygenation index was much higher at 2 h and 5 h after onset of ARDS(all P < 0.05). (2)There was no significant difference of RV among the four group at 0 h after onset of ARDS(all P > 0.05). Compared to the SC group,RV in SS,PC and PS group was much higher at 2 h after onset of ARDS(all P < 0.05). At 5 h after onset of ARDS,the RV of the SS group had a tendency to decrease while the RV of the PC and PS group had a tendency to increase. Conclusion:Prone position ventilation combined with RM improved oxygenation significantly. The increase of RV may be one of the most important mechanisms of its positire effect.

    • Effects of Astragalus Danshen on rat submandibular gland in Sjogren's syndrome model:an intervention study

      Abstract:Objective:To observe the interventive effects of Astragalus Danshen compound on rat submandibular gland in a small model of Sjogren’s syndrome. Methods:Submandibular gland tissue was extracted from Wistar rat,and emulsificated with complete Freund’s adjuvant (CFA)after blending,then subcutaneously injected into the rats on the hind feet and both sides of the groin. After that,subcutaneous injection of DTP vaccine was performed on the back to enhance immunity,thus producing pathological changes and clinical manifestations of the salivary gland similar to human Sjogren’s syndrome. Traditional Chinese medicine Astragalus particles combined with Danshen were applied. Results:The rat model was treated for 7 weeks. Salivary flow was reduce(P < 0.05),significant weight loss was found(P < 0.05),and submandibular gland index increased(P < 0.05). Partial parameters were imroved on rat model treated with Astragalus Danshen. The Chinese herbal compound significantly increased salivary flow(P < 0.05),both in the low and high dose groups(P < 0.01). However,there was no significance in water drinking between the rat model and the saline control group(P > 0.05). Conclusion:Astragalus Danshen can increase body weight and saliva secretion in the rat model mimicking Sjogren’s syndrome.

    • Large sample case-control study of association between adiponectin and susceptibility to type 2 diabetes mellitus

      Abstract:Objective:To survey the relationship between serum adiponectin level and type 2 diabetes mellitus (T2DM). Methods:348 newly diagnosed patients of T2DM and 574 controls from Jiangsu area were investigated,and the serum adiponectin levels were determined with ELISA. The information of healthy condition and behavior was gotten by examination and inquiry. Results:The serum adiponectin of T2DM group(6.01±2.43 mg/L)was significantly lower than that of control group(8.59±2.88 mg/L);In stratified analysis,compared with the group with high level of serum adiponectin,the low level groups(less than median level) had more risk of susceptibility to T2DM,and the OR values of age≤50[OR (95% CI):13.80(5.48~34.74)],smoking[10.74(5.58~20.68)]and abdomen obesity[7.27(4.55~11.62)] were top three; By multiple Logistic regression analysis,the T2DM risk for the low level of serum adiponectin,was increased[OR(95%CI):2.22(1.91~2.58)] compared with the higher level of serum adiponctin(graded by quartile range). Conclusion:There was an association between low level of adiponectin and high risk of susceptibility to T2DM.

    • Epidemiologic study of thyroid dysfunction in community population of Nanjing

      Abstract:Objective:To investigate the prevalence of thyroid dysfunction in Nanjing community population. Methods:The residents who had lived in Nanjing for at least five years and were more than 20 years old were selected by a stratified cluster sampling technique. 1 266 subjects were selected for questionnaire,physical examination and serum thyroid function test. Results:① Prevalences of overt hyperthyroidism,subclinical hyperthyroidism,overt hypothyroidism and subclinical hypothyroidism were 0.55%,1.74%,0.32% and 4.82% respectively in Nanjing city. ②The prevalences of overt hyperthyroidism,subclinical hyperthyroidism,overt hypothyroidism and subclinical hypothyroidism in females there all higher than those in male,however,the difference was not significant. ③ The prevalence of hypothyroidism increased significantly with age(ρ=0.0971,P=0.0005). Conclusion:Thyroid dysfunction,especially subclinical thyroid dysfunction is common in Nanjing. Screening,evaluation and intervention for thyroid dysfunction is necessary.

    • Estimation of HIV-1 incidence of MSM in Jiangsu Province with BED-CEIA assay

      Abstract:Objective:To understand HIV infection of men who have sex with men(MSMs) in Jangsu Province,and estimate the annual HIV incidence of MSMs. Methods:710 samples from national pilot work of HIV/AIDS comprehensive prevention and care among MSMs in 2008 were tested to find HIV-1 seropositive samples.BED HIV-1 IgG capture enzyme immunoassay(BED-CEIA) was used to find recent HIV-1 infections in HIV-1 seropositive samples,and the annual HIV-1 incidence of MSMs was estimated. Results:40 samples were HIV-1 seropositive and the HIV-1 infection incidence was 5.63%. 23 samples were confimrcl to be recently infected,and the recent infection rate was 57.5%. The corrected annual HIV-1 incidence was 7.54%(95% CI:4.46%~10.62%)among MSMs. Conclusion:The HIV-1 infectin in MSMs in Jiangsu Province has entered a period of rapid growth,powerful prevention and control efforts must be performed.

      Abstract:Objective:To analyze epidemiological characteristics on new reported HIV infected people in Jiangsu Province in 2008. Methods:According to the data of surveillance reports on HIV/ AIDS in 2008,Excel database was established and statistical analysis of data were performed with SAS 8.0 software. Results:Of 870 new reported HIV positive cases in 2008,65% were the resident of Jiangsu;76% were male;50% were married;27% were farmer;heterosexual transmission accounted for 40.57%;men who have sex with men(MSM) accounted for 26.55%;the average age was 36±12 years;19.54% of cases were at the stage of AIDS patients. There were significant differences in the distribution of transmitted routes,occupation,marriage and region of residence between HIV and AIDS stage;CD4+ counts of 51.61% of cases were less than 350 counts/μl. Conclusion:Unprotected sexual contact was the major HIV transmitted mode,while the proportion of MSM has the trend to increase;the proportion of old people increased rapidly. It is necessary to devote greater efforts to control transmission of HIV among old people,farmer and student;HIV/AIDS,especially AIDS patients,were mainly found in medical organizations,measures of earlier finding of HIV positive cases need to be strengthened.

      Abstract:Objective:To investigate the status and characteristics of internet addiction of college students,and related risk factors of internet addiction,to provide evidence for early intervention. Methods:The self-compiled questionnaires networks,social support scale,as well as internet addiction scale were used. 1189 college students from Jiangsu Province were investigated to analyze various factors of the internet addiction between internet addiction group and non internet addiction group. Results:Jiangsu College Students internet addiction rate was 10.6%,and the main influencing factors were grade,network availability,network age,time of day access,weekly access frequency,study stress. Social support scale showed that total score and the dimension scores had sighificantly differences between internet addiction disorder group and the control. Conclusion:The survey results show that college students with internet addiction disorder should be intervented by control the time of internet access,change the internat environment,and personality education of college students.

    • Preliminary study on Assessment of disease activity by grey scale and power Doppler ultrasonography in Rheumatoid Arthritis

      Abstract:Objective:To score the synovitis in patients with rheumatoid arthritis(RA) using grey scale(GS) and power Doppler (PD) ultrasonography(US),and explore the potential value of the two methods for assessment of disease activity. Methods:The DAS28,ESR and CRP of 40 RA patients were recorded. A total of 18 joints,including bilateral wrist,the second through fifth metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints,were examined using GSUS and PDUS measures to derive US scores for each patient. Results:US detected significantly more joints with synovitis than clinical examination did. The GS joint count (GSJC),GS joint score (GSJS) and the PD joint score (PDJS) were significantly correlated with DAS28. The PD joint count (PDJC),PDJS and the GSJS were significantly correlated with ESR and CRP. We did not find significant correlations between the PDJC and DAS28. Similarly,GSJC did not correlate with ESR and CRP. Conclusion:Sonography has the potential to evaluate disease activity in RA patients.

    • Detection on distribution and flowing dynamics of the cutaneous perforators of peroneal artery by color Doppler flow imaging

      Abstract:Objective:To study the distribution and flowing dynamics of perforator vessel of peroneal artery detected by color Doppler imaging in order to provide reasonable evidence for the design of location and protocol of skin flap. Methods:44 perforator vessels of peroneal artery from 25 healty volunteers were examed. The number of perforator vessel vascular initial diameter,distribution,and flowing dynamics were measured. Results:In 44 samples studied,161 cutaneous branches were found. Two to six branches were found on one specimen,and the average number of cutaneous perforators was 3.6.The perforating points of cutaneous branches mostly (81.3% case) appeared within 9~23 cm below the protruding point of head of fibula 36(22.3%),104(64.4%)and 21(13.1%)of the perforating points appeared in the upper,middle and lower part of the lower log respectively. The average initial internal diameter of cutaneous was 0.95±0.23 mm,and vascular flow peak systolic velocity was 0.17±0.08 m/s. There were no significant differences in the peak systolic velocity between left and right dominant cutaneous perforators. Male’s peak systolic velocity was higher than that of female (P < 0.05). Conclusion:The color Doppler ultrasonography with high resolution could show the distribution and location of perforators of peroneal artery clearly,and assess the quality of the blood flow and consequantly provide scientific basis for planning skin flap.

    • The initial research of endoscopic diagnosis and treatment strategy for esophageal mucosal lesions

      Abstract:Objective:To explore the diagnostic and therapeutic value of combination of endoscopic Lugol staining,miniprobe ultrasonography(MPS),argon plasma coagulation(APC),endoscopic mucosal resection (EMR) and endoscopic submucosal dissection(ESD) for esophageal mucosal lesions. Methods:①The measurement of range and depth of lesions,endoscopic Lugol staining and MPS were performed on 232 patients with esophageal lesions detected by routine endoscopy. ②Multi-point biopsies were collected to confirm the histopathologic diagnosis. ③APC,high-frequency knife,EMR and ESD were chosen to treat esophageal lesions according to their chromoendoscopy features,ultrasonography definitions and histopathologic findings.④Resected mucosal samples were histopathologically re-examinated and all patients were followed up for endoscopic examination periodically after the therapy. Results:232 cases were included:35 cases with squamocellular papillary hyperplasia;34 cases with chronic mucosal inflammation;58 cases with high-grade intraepithelial neoplasia;54 cases with low-grade intraepithelial neoplasia;31 cases with mucosal carcinoma and 20 cases with polyps. All intraepithelial neoplasia,mucosal carcinoma and polyp cases were endoscopic treated successfully and no recrudescence was found in short-term follow-up (3~42 months,average 21.6 months). Conclusion:The strategy of combination of endoscopic Lugol staining,MPS,APC,EMR and ESD has a promising value on dignosis and treatment for esophageal mucosal lesions.

    • Rediscussion of diagnositic value of PSA,F/T ratio and PSAD in prostate biopsy

      Abstract:Objective:To evaluate diagnositic value of prostate specific antigen (PSA),free/total PSA (F/T PSA),and PSA density (PSAD) in prostate biopsy. Methods:Total serum PSA and free PSA were detected while F/T PSA value and PSAD were calculated in 285 patients who underwent transrectal ultrasonography-guided prosate biopsy. Optimum PSA,F/T PSA value and PSAD for detecting Pca were discussed. Results:When the cut off value of PSA was set as>15 ng/ml,the diagnostic sensitivity,specificity and the positive predictive value were 70.0%,68.5% and 58.6% respectively. When PSA <15 ng/ml,the difference of PSA between BPA and Pca groups was not significant(t=0.018,P > 0.05),while F/T value and PSAD showed significant differences(t=4.137,3.158,P < 0.01). When the cue off value of F/T was set as 0.16 and the cue off value of PSAD as 0.35 ng/(ml·cm3),the sensitivity,specificity and the positive predictive values were higher. Conclusion:PCa should be highly suspected when PSA is more than 15 ng/ml. The detection rate of prostate cancer could be significantly improved in combination of F/T and/or PSAD.

    • Comparison between video-assisted thoracic surgery(VATS)and traditional open surgery on lobectomy for non-small cell lung carcinoma

      Abstract:Objective:To retrospectively compare the differences between Video-assisted thoracic surgery (VATS) and traditional open surgery(TOS) on lobectomy for non-small cell lung carcinoma. Methods:The patients with non-small cell lung carcinoma undergone lobectomy from June,2008 to March,2009 were devided to two groups:VATS group and TOS. The operation time,perioperative blood loss,time for chest drainage,postoperative pain,hospital stay and cost,and complications of two groups were analyzed. Results:The operation time,time for chest drainage and hospital stay in VATS group are similar to TOS group. Perioprative blood loss and postoperative pain in VATS group are less than those in TOS group significantly. The hospital cost in VATS group is higher significantly than that in TOS group. No death occurred in two groups. Conclusion:Lobectomy with VATS caused less wound pain,blood loss and postoperative complications. The patients with VATS recovered more rapidly. But the cost was much higher.

      Abstract:Objective:To explore the risk factors of childhood acute lymphoblastic leukemia(ALL) in Chinese. Methods:A case-control study including 233 cases and 380 controls and univariate and multivariate logistic regression were employed to evaluate the association of the factors in childhood ALL. Analysis was carried out with SPSS13.0 software. Results:In the univariate logistic analysis,subjects living without parents,with immature labor,un-spontaneous delivery,biocide exposure,paternal alcohol exposure,actino-exposure,houses painting and chemical exposure were the statistically significant risk factors of childhood ALL. Multivariate logistic analysis also revealed that living without parents,with immature labor,biocide exposure,paternal alcohol exposure,and houses painting were the risk factors. Conclusion:Living without parents,with immature labor,biocide exposure,paternal alcohol exposure,and houses painting are associated with the childhood ALL.
