      Abstract:After the opium war,the western dentistry was introduced into China with other western modern medicine,changed people’s concept of dental diseases,and provided a treatment mode which was significant different with Chinese dental treatment. Due to the restriction of the traditional culture and the social environment,dental treatment is still ignored,and the special obvious difference is showed on idea and behavior of dental problems. The persistent pursuit of “healthy” promotes the integration of idea and behavior and gradually achieves social and ecological pattern in which variety ideas and behaviors exist side by side.

      Abstract:Objective: To understand the status quo of service capabilities of township health centers (THCs),and give some advices on promoting the implementation of new healthcare reform in China. Methods: By methods of documentation, questionnaire survey and experts investigation, the service level of township health centers in Lianyungang city was studied. Descriptive statistical analysis method was mainly used. Results: The distribution of health resources was not reasonable, and obviously concentrated on the higher-level health institutions. Mainly by extensive pattern of growth, service capabilities of THCs had been enhanced in the past few years, public health services had been strengthened and medical services rapidly increased, while the overall quality and the structure of rural health professionals were not less than satisfactory, the phenomenon on paying more attention to medical services than prevention services was still existed. Conclusion: Health resources for grass-root service should be enhanced, and the programme of health human resources should be actively formulated and implemented. THCs should improve its management level, take the road of intensive development, and strengthen performance evaluation to promote public health services and basic medical services.

      Abstract:Township health centers (THCs) play a key role in rural three-tier health network and undertake the responsibility of primary health care in rural areas. However, with the implementation of market-oriented economic reform, a series of problems have arisen to THCs and their development are not satisfactory. Based on the literaturereview and statistics data of Ministry of Health, the paper analyzed the important position, development situation and the problems existing in THCs,focused on how to improve the management of THCs,and proposed some relevant strategies.

    • Study on outpatient pharmaceutical care of third-level hospitals in Nanjing from the pharmacists’ view

      Abstract:To understand the pharmacists’ awareness of outpatient pharmaceutical care and the status quo of their work, the general situation was investigated by spot questionnaire survey among third-level hospitals selected by random sampling in Nanjing. The outpatient pharmacists already had a certain degree of awareness of the pharmaceutical care, but they still worked to guarantee and supply medicine due to the effect of various factors. In view of this, the authors suggested that the departments of government, medical institutions, doctors, pharmacists and patients should cooperate with each other, and especially, pharmacists need to continually improve their own quality to provide better professional services for different people.

    • A survey on health literacy of university students in Nanjing city

      Abstract:Objective: To realize the status of university students’ health literacy in Nanjing city and analyze the correlative influence factors. Methods: Based on the questionnaire survey of Chinese citizens’ health literacy in 2008, a questionnaire survey of university students’ health literacy was designed, and conducted among 615 university students drawn from freshman to senior by using the multistage cluster sampling method. Results: 1.8% university students had health literacy; the rates of heath knowledge and concept, health life style and behavior and health skills were 15.4%, 0.7% and 36.7% respectively. The influence factors of university students’ health literacy were correlative to gender, place of graduation, parents’ level of education, grades and academic record. Conclusion: The level of university students’ health literacy is lower. The university educators should focus on the survey and study of health literacy of university students, formulate a unified standards about policy of health education and promotion, directly develop health literacy education, and increase the level of students’ health literacy.

    • Analysis on needs of community-based rehabilitation among elderly people in Fuzhou city

      Abstract:With the aging of population, China’s public health undertaking is facing new challenges. Under the environment of new medical reform, the authors made an analysis on needs of community-based rehabilitation among elderly people in Fuzhou city. The results showed that the current medical service in community couldn’t meet with rehabilitation needs of elderly people, medical resources must be integrated, the technical guidance for the rehabilitation team in community should be strengthened and private sector should be encouraged to develop community-based rehabilitation among elderly people while at the same time the government should increase the input.

    • Discussion on surgical specimen management

      Abstract:Objective:To summarize and improve the surgical specimen management. Methods:A retrospective analysis of operation specimens from 2010 to the present in the First Affiliated Hospital of Nanjing Medical University was made. Results:A total of 10 547 cases of surgical specimens were performed pathological examination in 2010, and 78 cases of defective specimens were found which accounted for 0.74%. There were 6 cases of improper preserved specimens accounting for 7.69%(6/78),57 cases with incomplete pathological application forms,serious pollution pathological application forms,or the data fuzzy accounting for 73.08%(57/78),14 cases with pathological application forms losing accounting for 17.95%(14/78),1 specimen lost accounting for 1.28%(1/78). After implementation of the new regulations of surgical specimens in 2011,all specimens were kept completely. Conclusion:Strengthening the correct management of samples and monitoring the transfer process can prevent medical disputes related to surgical specimens.

    • Role of HIV-1 Nef protein carried by recombinant lentivirus in proliferation of primary effusion lymphoma cell line and vascular endothelial cells

      Abstract:Objective: To construct the recombinant lentivirus containing HIV-1 Nef gene and detect the protein expression in its target cell lines, such as 293T, BCBL-1 and EA.hy926, and explore the effect of Nef on cell proliferation. Methods:The fragment of Nef gene from expression vector pCI-neo-Nef was cloned into the lentivirus vector pHAGE-CMV-MCS-Izs-Green, then the recombinant vector pHAGE-Nef, packaging vector psPAX2 and envelope vector pMD2.G were cotransfected into the 293T cells. Culture media were harvested and filtered through a 0.45 μm filter to remove the cells. The viral titer was checked by observing the expression of green fluorescent protein (GFP). After infection of the recombinant lentivirus, the protein expression of Nef in 293T, BCBL-1 and EA.hy926 cells were detected by Western blot. The effect of Nef on cell proliferation was investigated by Cell Counting Kit-8 assay. Results:The recombinant lentivirus vector carrying HIV-1 Nef was constructed successfully. The viral titer was 1×107 TU/ml. 293T, BCBL-1 and EA.hy926 cells could be efficiently infected by it. The expression of Nef in these cell lines could be detected by Western blot. In addition, Nef protein could inhibit the proliferation of BCBL-1 and EA.hy926 cells. Conclusion:Recombinant lentivirus with high titer and efficient infection of 293T, BCBL-1 and EA.hy926 cells, could be obtained quickly and simply by using the lentivirus vectors system, and Nef expression could be detected in 293T, BCBL-1 and EA.hy926 cells infected by lentivirus-Nef. Furthermore, our study suggested that Nef protein could inhibit the proliferation of BCBL-1 and EA.hy926 cells.

      Abstract:Objective:To study correlated factors of oral mucositis caused by radiotherapy in nasopharyngeal carcinoma patients with intensity modulated radiotherapy,in order to standard particular nursing steps and reduce the incidence of serious radiogenic stomatitis. Methods:One hundred and fifty patients were investigated and 12 indexes were analyzed by -字2 text. Results:All the patients suffered oral mucositis,Ⅱ and above oral mucositis were about 81%. Single factor analysis found that nutrition,smoking,dental heath,oral pH,antibiotics using and chemotherapy were correlated with oral mucositis. Conclusion:Attention should be paid to the patients receiving intensity modulated radiotherapy especially with smoking, antibiotics using and chemotherapy. Preventive nursing during radiogenic or after radiogenic should be strengthened to reduce the incidence and severity of oral mucositis.

    • Preliminary study on infection of human gingival fibroblast cells with Kaposi's sarcoma-associated herpesvirus in vitro

      Abstract:Objective: To establish a model of infecting human gingival fibroblast(HGF)cells with KSHV in vitro for further studying the mechanism of oral KS pathogenesy. Methods:The supernatants containing KSHV virion harvested from primary effusion lymphoma(PEL) cells BC-3 were used to infect HGF cells. The cytopathic effect (CPE) of HGF cells was observed post-infection and the lytic replication of KSHV in HGF cells was detected by RT-PCR and Western blot. Results:CPE appeared in post-infective HGF cells. KSHV ORF26 and ORF73 mRNA transcription was detected in HGF cells by RT-PCR at diffenrent time points. And the specific band of vIL-6 protein was detected at 12 hours post-infection by Western blot. Conclusion:KSHV could infect HGF cells and establish latent infection,providing a model in vitro for further studying the mechanism in KSHV leading to oral KS.

    • Experiential meta-function analysis of English abstracts in medical research articles

      Abstract:In this article the authors mainly discussed the relationship between genre and experiential organization. The analysis was carried out from three perspectives: i) distribution of transitivity processes in different moves of medical research article abstracts; ii) distribution of voice in different moves of medical research article abstracts; iii) interrelations between distribution of transitivity processes and distribution of voice. Through analysis of 50 English structured abstracts selected from 10 SCI medical journals, it was found that there was a close relationship between the communicative purposes of genre and the choice of transitivity and voice. To some extent, the choice of transitivity conditioned the choice of voice. The forms of language represent language functions, meanwhile the functions of language condition the language forms.

      Abstract:Objective: To explore the mechanism of prostaglandin E2(PGE2)affects the cell proliferation ability of human bile duct carcinoma cells CCLP1. Methods:CCLP1 cells were treated with PGE2,EP1-4 receptor agonist,AC agonist(Forskolin),PKA antagonist(H89),cAMP analogues(db-cAMP). The expression levels of SnoN mRNA and SnoN protein,and the cell proliferation ability were examined by RT-PCR,Western blot and WST methods in CCLP1 cells. Results:The expression of SnoN mRNA in CCLP1 cells increased by 22.5%(P < 0.01),while the level of snoN protein increased by 35.6%(P < 0.05)after treated with 10 μmol/L PGE2 for 24 h;The expression of SnoN protein in CCLP1 cells increased by 64.9%(P < 0.05)after treated with EP2 receptor agonist(10 μmol/L)for 24 h,and cell proliferation ability of CCLP1 increased by 26.5%;The levels of SnoN protein and CREB phosphorylation in CCLP1 cells increased by 25.1% and 71.3%(P < 0.05)respectively compared with the control group after treated with AC agonist Forskolin(10 μmol/L)for 24 h,and cell proliferation ability of CCLP1 increased by 4.4%(P < 0.05)after treated with 10 μmol/L Forskolin;The expression of SnoN in CCLP1 cells increased by 90.1%(P < 0.05)when treated with cAMP analogues db-cAMP(500 μmol/L)for 24 h,and decreased when treated with PKA antagonist H89(10 μmol/L)for 24 h,the levels of SnoN expression and CREB phosphorylation in CCLP1 cells treated with H89 decreased by 9.1% and 14.1%(p<0.05)compared with those treated with Forskolin.Cell prolifelation of CCLP1 decreased by 45.7%(p<0.05)after treated with 20 μmol/L H89.Conclusion:PGE2 might up-regulate the expression level of SnoN through EP2 receptor of CCLP1 cells which could be partly related to the cAMP-PKA-CREB signaling pathway,and promote the cell proliferation.

    • Effect of STAT3 gene silencing on growth of chronic myelogenous leukemia K562 cells

      Abstract:Objective: To construct the STAT3 shRNA (short hairpin RNA) lentivirus and investigate the effects of STAT3 SiRNA(small interference RNA)on the biological property of human chronic myelogenous leukemia (CML) K562 cells. Methods:The STAT3 shRNA lentiviral vector was constructed according to the effective target sequences for RNAi of STAT3 gene. The STAT3 SiRNA lentivirus were produced by transfecting into packaging cell line 293T. After transfected into K562 cells, the knockdown of STAT3 gene were determined by RT-PCR and Western-blot assay. The survival and proliferation of K562 cells were detected by both cell growth curve and MTT assay. The FCM was used to detected cell cycles alternation and the early apoptosis by Annexin V-PI assay. Results:PCR and sequencing analysis verified that the construction of the lentiviral vector of STAT3 shRNA was successful and the expression of STAT3 gene was knockdowned around 70 percent by two target sequences for STAT3 RNAi in K562 cells. Proliferation of K562 cells became slower and the proliferation activity of K562 cell was decreased to 40% post-transfection 5 days by lentiviral-mediated STAT3 SiRNA(P < 0.05). The proportion of cells in G0/G1 phase were increased while the cells in S phase decreased, which indicated a delay at G1 to S checkpoint in cell cycles. More than half the K562 cells underwent apoptosis after transfected with STAT3 SiRNA lentivirus compared to the control cells[(51.80±5.60)% vs (1.13±0.12)%],respectively(P < 0.01). Conclusion:Lentivirus-based STAT3 SiRNA can effectively mediate STAT3 gene silencing in K562 cells. SiRNA targeting STAT3 gene can inhibit K562 cell growth and proliferation, block cell cycle at G1 to S phase and induce cell apoptosis.

      Abstract:Objective: To compare the teaching effects of two different performance rating systems. Methods: The test system reform and comprehensive evaluation were carried out including emphasizing the practice operation, taking care of the whole process of study, making the purpose of study clear, and increasing comprehensive analysis test to cultivate practice innovation, clinical thinking and ability on analyzing and solving problem. The scores before and after the test reform of two grades students were compared and a questionnaire survey for the students and teachers was used. Results: The students’ scores after the test reform had been improved. The number of low scored students had been reduced. Most of the students would like to select the comprehensive evaluation system. Conclusion: The comprehensive evaluation system makes the students focus on the whole process of studying, makes the evaluation be more objective, and contributes to the improvement of the teaching quality.

      Abstract:Objective:To improve teaching effect by applying multimedia teaching as the intuitional instruction mode. Methods:Application of performance appraisal and the form of questionnaires were used to assess the effect of two teaching methods. Results:Test score of the students used of multimedia teaching and results of survey were significantly better than traditional teaching students. Conclusion:Application of multimedia technology in neurosurgery can achieve good results.

    • Anticancer effects of combination of anti-EBV-LMP1 humanized antibody Fab and mitomycin C on nasopharyngeal carcinoma xenografts in nude mice

      Abstract:Objective: To investigate the inhibitory activity of anti-latent membrane protein 1(LMP1)antibody fragment (Fab) combined with mitomycin C(MMC) on nasopharyngeal carcinoma NPC-LMP1 xenografts in nude mice. Methods:Nasopharyngeal carcinoma LMP1 cells (5×106) were subcutaneously transplanted in to 20 nude mice, which were randomly divided into Fab group, MMC group, Fab+MMC group and control group. The mice were injected once every 3 days. After corresponding treatments for 5 times, the tumor inhibition rate was evaluated, vascular endothelial growth factor (VEGF) was measured with immunohistochemistry, and the apoptotic rates were analyzed by flow cytometry. Results:The tumor growth inhibition rates in Fab group, MMC group, Fab+MMC group were 18.90%,28.95% and 49.12%,respectively. VEGF in Fab group and Fab+MMC group significantly reduced and showed lower expression compared with control group (P < 0.05). The apoptotic rates in MMC group and Fab+MMC group were (11.84±1.50)% and (17.55±3.21)%, dramatically decreased compared with control group(P < 0.01). Conclusion:Fab and MMC can both inhibit the subcutaneous xenograft tumor growth of NPC LMP1 cells in nude mice with distinct mechanisms. Fab may inhibit the angiogenesis of tumors, while MMC presumably induces apoptosis of tumor cells.

    • Construction of the recombinant lentiviral vector containing human ANXA2 gene and its expression in hepatocellular carcinoma cells

      Abstract:Objective: To construct the recombinant lentiviral vector containing human annexin A2 (ANXA2) gene and measure the expression level of ANXA2 in hepatocellular cell line of Hepa 1-6. Methods:The ANXA2 fragment was amplified by PCR and subcloned into the lentiviral vector pWPT. The resultant lentivirus were confirmed by PCR, restriction enzyme digestion and DNA sequencing. Recombinant lentivirus (LV-ANXA2-Flag) were produced from 293T by transient calcium-phosphate co-transfection with pWPT-ANXA2-Flag, CMVR8.91and pCMV-VSV-G. Hepa 1-6 cells were infected by LV-ANXA2-Flag lentivirus, and the expression of ANXA2 was confirmed by RT-PCR and Western blotting. Cell cycles were measured via flow cytometry. Results:The recombinant lentiviral vector carried the ANXA2 gene was successfully constructed. RT-PCR and Western blot analysis revealed that the ANXA2 gene can be correct transcripted and translated in Hepa 1-6 cell which infected by LV-ANXA2-Flag. After infected,the Hepa 1-6 cells were higer than control cells in percentage of S phase cells. Conclusion: The recombinant lentiviral vector pWPT-ANXA2-Flag was constructed successfully, it could be used to transfect hepatocellular carcinoma cells. ANXA2 could be stablely expressed and promote the proliferation of Hepa 1-6 cell.

    • The effect of triptolide on the proliferation, apoptosis and the expression of BCL-2/BAX of human choriocarcinoma cell line JAR in vitro

      Abstract:Objective: To investigate the effect of triptolide(TP) on the proliferation,apoptosis and the expression of BCL-2/BAX of human choriocarcinoma cell line JAR in vitro. Methods:Human choriocarcinoma cell line JAR was cultured. After the treatment with triptolide, MTT assay was used to detect the inhibitive effect of triptolide on cell proliferation; TUNEL assay was exploited to determine the level of cell apoptosis; Western blot was used to observe BCL-2/BAX expression. Results: TP could inhibit the proliferation of cell line JAR. The inhibition effects were concentration and time dependent. TP could affect the apoptosis of cells, and the difference was statistically significant with control group(P < 0. 05). PCR and Western blot showed the mRNA and protein levels of BCL-2 were significantly decreased, and the mRNA and the protein levels of BAX were significantly increased after exposure to TP, compared with control group(P < 0.05). Conclusion:Triptolide could inhibit the proliferation and induce apoptosis of JAR cells. The apoptosis mechanism involves the down-regulation of BCL-2 protein and up-regulation of BAX protein.

    • The antitumor effect of triptolide on HepG2 cells in vitro and in vivo

      Abstract:Objective: To study the effect of triptolide on human hepatocelluar carcinoma HepG2 cells proliferation and apoptosis in vitro and in vivo, as well as its possible mechanisms. Methods:HepG2 cells were cultured in triptolide media with different concentrations. Cell viability was assessed by MTT assay. Cell apoptosis was observed by flow cytometry(FCM). The heat shock protein 70(HSP70) was assayed by Western blotting and RT-PCR methods. Caspase-3 activity was examined by caspase-3 colorimetric assay. In a xenograft tumor model, the control group and experimental group were treated with 0.9% NaCl solution and triptolide(0.4mg/kg,q.d.), respectively .Three weeks after treatment, the influence of triptolide on the growth of hepatocellular carcinoma was observed. Results:Triptolide treatment resulted in dose- and time-dependent proliferation and apoptosis of HepG2 cells(P < 0.05). Triptolide decreased HSP70 mRNA and protein levels in a dose-dependent pattern, whereas caspase-3 activity was significantly increased in a dose-dependent manner. Moreover, Mice receiving triptolide therapy had significantly smaller tumors than controls(P < 0.05). Conclusion:Triptolide causes human hepatocelluar carcinoma HepG2 cells death in vitro and in vivo by inducing apoptosis and its mechanism of action might be mediated via the inhibition of HSP70 and up-regulation of caspase-3 activity.

      Abstract:Objective:To investigate the effect of matrix metalloproteinase-9 on the expression of caspase-3 and its possible mechanisms. Methods:The model of 70% hepatic ischemia-reperfusion was established in SD male rat. Total 48 SD rats were randomly divided into three groups(n = 16): sham operation group(S group),ischemia-reperfusion group(I/R group) and doxycycline treated group(DC group). After 3 hours of reperfusion,the enzyme levels in serum were examined, including ALT,AST,TNF-α and MDA. The expressions of MMP-9 and Caspase-3 were determined in immunohistochemistry. Results:After ischemia-reperfusion,the levels of ALT,AST,TNF-α and MDA expression in I/R group was significantly higher than those in S group. After treatment of oral doxycycline,the all above abnormal parameters were retrieved remarkably(P < 0.05). In the I/R group,MMP-9 and caspase-3 expressions increased,comparing with that in S group,while its expression decreased in the DC group,comparing with that in I/R group. Conclusion:The low expression of MMP-9 reduces the release and infiltration of inflammatory cells,inhibits caspase-3-mediated apoptosis of liver cells.

    • The involvement of apoptosis in vasculogenic mimicry formation of human breast cancer cells

      Abstract:Objective: To investigate the effect of cell apoptosis on the vasculogenic mimicry in human breast cancer cells(MDA-MB-231). Methods:The tube formation ability was assessed using a matrigel basement membrane model, and the structures of the tubular networks were observed under the inverted microscope. Apoptosis cells were stained with Annexin-FITC and PI,and then quantified by fluorescent microscope. Using the broad range caspase inhibitor Z-VAD-FMK and the caspase-3 inhibitor Ac-DEVD-CHO,we further explored the influence of cell apoptosis on forming capillary like structures. Results:MDA-MB-231 cells linked to each other to form wreaths and network structures after 24 h based on three dimensional culture. Capillary-like structures of MDA-MB-231 cells were stained positively with PAS. Apoptosis occurred before capillary-like structure formation but not after capillary-like structures had assembled. Both Z-VAD-FMK and Ac-DEVD-CHO blocked capillary-like structure formation. Conclusion:Breast cancer cells can adopt a capillary-like structure and engage in vasculogenic mimicry. Capillary-like structure formation requires apoptotic cell death through activation of a caspase-dependent mechanism.

      Abstract:Objective: To investigate the effect of excelluar signal-regulated kinase(ERK) on rat hippocampal neurons with oxygen-glucose deprivation(OGD). Methods:Cultured hippocampal neurons were designed into four groups:normal group,OGD group, PD98059 10 μmol/L and 30 μmol/L groups. The apoptosis of neurons was determined by flow cytometry, and Western blot was used to evaluate protein expression of ERK1/2 and pERK1/2. Results:Compared with normal control group, neuron apoptosis increased while pERK1/2 protein expression decreased obviously in OGD group(both P < 0.01). Neuron apoptosis increased and pERK1/2 protein expression decreased more significantly in PD98059 groups compared with OGD group(both P < 0.01). Changes in 30 μmol/L PD98059 group were more significant than 10 μmol/L PD98059 group (P < 0.01). Each group showed no significant change in ERK expression(P > 0.05). Conclusion:ERK may be involved in neuronal apoptosis after OGD, and inhibition of ERK pathway can promote neuronal apoptosis.

      Abstract:Objective: To observe the effects of methylprednisolone(MP) on expressions of FOS and NOS in the paraventricular nucleus (PVN) of the hypothalamusfollowing the acute spinal cord injury(SCI). Methods:The total of 36 adult SD rats were randomly divided into 3 groups: control group, SCI group and MP treatment group. The rats in SCI and MP treatment groups were received the spinal transection at the fourth thoracic segment(T4). MP treated rats were given intravenous injection MP(30 mg/kg) for once, or once daily for one week or four weeks, respectively. FOS and/or NOS positive neurons were mapped in the PVN using NADPH-d histochemistry combined with Fos immunohistochemistry. Results:Compared with control group, the numbers of FOS positive neurons and NOS positive neurons were increased in the PVN, which were decreased following MP treatment for one week or four weeks. Conclusion:MP may have the protective effects on neurons in the PVN following the SCI.

      Abstract:Objective: To investigate biological activities after CD147 was down-regulated by microRNA in cervical cancer cell line SiHa. Methods:The plasmids pSilencer 4.1-CMV neo/CD147 siRNA was reconstructed, and then they were stablely transfected into SiHa. The expression of CD147,Bcl-2 and caspase-3 were detected by RT-PCR and Western blot assay. Afterwards, the method of MTT was used to analyze the tumor proliferation. The experiment of nude mice was performed to observe the tumor growth. Results:The expression of CD147 mRNA and protein could be inhibited by pSilencer 4.1-CMV neo/CD147 siRNA Sequence 1 and 2. Compared with the control group, cancer cells have more apoptosis and lower proliferation, After cancer cells were transplanted into nude mice,the tumor volume of siRNA groups were smaller than that of the control. Conclusion:The expression of CD147 could be down-regulated effectively by CD147 SiRNA plasmids, the proliferation and the ability of forming tumor could be inhibited evidently after this treatment in cervical carcinoma cells SiHa.

      Abstract:Objective:To construct a recombinant vector containing Tip-α of H.polyri, and transfect it in E.coli to purify recombinant protein. Methods:By PCR technique, Tip-α cDNA was amplified, and then cloned into the pET28a vector. The recombinant plasmid was transformed into E.coli. The correct clone was identified by sequence analysis. The expression product was purified by nick-nitrilotriacetic acid(Ni-NTA) metal-affinity chromatography and analyzed by SDS-PAGE and Western blot methods. Results: Enzyme digestion analysis and sequencing assay showed that Tip-α gene had been successfully inserted into the vector. SDS-PAGE showed a 23 ku protein identified by Western blot analysis. Conclusion:A recombinant plasmid containing Tip-α has been constructed and expressed successfully.

      Abstract:Objective: To investigate the distribution and variety of beta-lactamase genes and ompC gene for outer membrane porin protein C in Escherichia coli (E.coli) strains isolated from urine. Methods:From January 2006 to October 2008, 60 strains of E.coli isolated from urine were collected from The First Affiliated Hospital of NJMU, China. Then, 21 kinds of beta-lactamase genes and ompC gene for outer membrane porin protein C were analyzed by PCR and verified by DNA sequencing. Results: In 60 strains of E.coli, 51 strains (85.0%) , 6 strains(10.0%)and 38 strains(63.3%) were detected to harbor TEM-1-like,OXA-30-like and CTX-M-55-like by DNA sequencing, respectively. Moreover, 54 strains were detected mutaions in ompC gene for outer membrane porin protein C (90.0%), and 6 strains among them were confirmed as new mutation types by BLASTn. But other 6 strains(10.0%) were detected no mutaion in ompC gene. Conclusion:Both CTX-M-55-like beta-lactamase gene and ompC gene for outer membrane porin protein C play a key role in resistance to beta-lactam antimicrobial agents. Among 6 strains of E.coli containing new mutation types in ompC gene for outer membrane porin protein C, 3 strains combined with co-existing TEM-1-like and CTX-M-55-like beta-lactamase genes, 2 strains combined with co-existing TEM-1-like beta-lactamase gene, 1 strain combined with co-existing TEM-1-like and OXA-30-like beta-lactamase genes.

    • Root form and canal morphology of maxillary first premolars

      Abstract:Objective: To investigate the root form and canal morphology of maxillary first premolars. Methods:The cleared teeth were used. The number of the roots was recorded, and the root canal morphology of the cleared teeth was examined under a dental operating microscope. Results:Total 268 cleared teeth were examined. There were 201 teeth(75.00%) with one root, 62 teeth(23.13%) with two roots, and 5 teeth(1.87%) of the samples with three roots. The percentage of one canal, two canals and three canals were 3.06%, 83.58% and 1.87%, respectively. Only 1.49% of the total samples demonstrated other types. The percentage of lateral canals was 25.00%, and the apical foramina were mainly localized laterally. Conclusion:The root canal morphology of maxillary first premolars from high to low incidence was one root with two canals, two roots with two canals, one root with one canal, and others.

    • Assessment of atrial volume and function with full volume three-dimensional echocardiogra-phy in patients with atrial septal defect

      Abstract:Objective: To quantify the volume and function of atrium in the patients with atrial septal defect(ASD) with full volume three-dimensional echocardiography(FV3DE) and evaluate the influence of occluder closure on the volume and function of atrium. Methods:FV3DE data of 43 patients with ASD and 40 normal controls were acquired and analyzed. Minimum volume (Vmin), maximum volume (Vmax) and pre-contraction volume (Vp) of atrium were measured, and the reservoir function, conduit function and pump function were calculated. All the results were compared respectively. Results:Vmin,Vp, Vmax, active emptying volume (Vae),passive emptying volume(Vpe)and total emptying volume(Vte)of right atrium in the patients with ASD were larger than those in the controls(P < 0.05); While passive emptying fraction (Vpef),active emptying fraction (Vaef) and total emptying fraction (Vtef)were smaller than those in the controls significantly(P < 0.05). After the occluder closure of ASD, the Vmin,Vp,Vmax,Vae,Vaef,Vpe,Vte and Vtef of right atrium became smaller, while Vp, Vmin, Vmax of left atrium became larger that before. And the Vaef, Vpef, Vte, Vtef of left atrium became smaller than those before operation. Conclusion:The atrial volume and function in the patients with ASD can be evaluated with FV3DE. As for the patients with ASD, reservoir function and conduit function of the right artrium increases and pump function decreases. After occluder closure, the volume of right atrium was retrieved, volume of left atrium enlarges for compensation, while the conduit function and pump function of both atria reduce.

    • The prognostic analysis of tumor size in gastric cancer patients

      Abstract:Objective: To investigate the prognostic significance of tumor size in patients with gastric cancer. Methods:To evaluate the influence of tumor size on the outcome, we reviewed 489 gastric cancer patients who underwent surgery and chemotherapy. Results:An appropriate cutoff value of tumor size affecting patient survival was 3 cm. Patients were divided into two groups as follows: small size group (SSG)with tumor size <3 cm, and large size group (LSG) with tumor size ≥3 cm. Tumor size was significantly related to histological grade, status of lymphovascular and vessel invasion, depth of primary tumor invasion, lymph node metastasis and TNM stage. Univariate analysis revealed that the prognosis of patients in SSG was significantly better than that in LSG. Multivariate analysis showed that tumor size was an independent prognostic factor along with macroscopic appearance, depth of primary tumor invasion, lymph node metastasis and distant metastasis. In SSG,patients with T2 and T3 had significant higher 5-year survival rate than those with T2 and T3 in LSG. Conclusion:Tumor size is one of highly significant parameters, served as independent predictor of survival in patients with gastric cancer.

      Abstract:Objective: To study the amyotrophic lateral sclerosis functional rating scale revised(ALSFRS-R) and its progression rate of ALSFRS-R correlation with survival time. Methods:Application of 2000 word Union of Neurology El Escorial diagnostic criteria retrospectively analyzed 64 case of amyotrophic lateral sclerosis clinical date, be ALSFRS-R score and the progress rate(⊿FS)and Kaplan-Meier analysis was used from onset to tracheostomy or death ,the median group and Log-rank analysis was used to estimate the difference between two groups. Application of Cox proportional hazards model,adjust the parameters of multi-factor analysis: sex, age, onset duration , forced vital capacity(FVC). Results:Total 64 ALS patient were followed, 8 lost,56 with incision of tracheal or death, M∶F=2.7∶1, the average survival time of 3.5 years. Log-rank analysis of two groups was statistically significant. Application of Cox proportional hazards model ALSFRS-R(HR:0.87,95%CI:0.82-0.93,P < 0.01),⊿FS(HR:5.52,95%CI:3.13-9.71,P < 0.01 ). Conclusion:ALSFRS-R and ⊿FS predicting the prognosis of patients.

    • A comparison study of clinical application between dilator-kyphoplasty,balloon-kyphoplasty and Sky-bone expander kyphoplasty

      Abstract:Objective: To compare the clinical results of dilator-kyphoplasty(DKP),balloon-kyphoplasty(BKP) and Sky-bone expander kyphoplasty(EKP). Methods:A retrospective study was done from October 2006 to February 2010, and 71 cases(71 vertebra) with DKP, 55 cases (55 vertebra) with BKP and 22 cases (22 vertebra) with EKP procedures were performed. The operating time, bleeding volume, cement injecting volume of cement were recorded during operation. The patients′ VAS score and ODI score were evaluated after operation. The distribution of the cement and the restoration of vertebral height and Cobb angle were observed post-operation. Results:There are no significant differences in operative time and bleeding volume of every vertebrae in these three groups. The EKP group had smaller cement injected volume than other groups. The VAS and ODI scores were decreased significantly, and the vertebral height and Cobb angle were restored in three groups after operation. The post-operative changes were significantly more pronounced in the DKP group. Conclusion:DKP, BKP and EKP were all effective and safe in the treatment of osteoporosis vertebral compression fractures. The vertebral height and Cobb angle were restored in three groups. The post-operative changes were more pronounced in the DKP group. There is lower cement leakage rate in DKP than the other two methods, because the bone cement was injected in dough state and the vertebral body was parallelly strutted.

    • Bladder inflammatory myofibroblastic tumor imaging findings and clinical analysis

      Abstract:Objective:To study the urinary bladder inflammatory myofibroblastic tumor (IMT) of the clinical manifestations,imaging features,pathological features and treatment methods to improve the understanding of this disease. Methods:From 5 pathologically confirmed cases of bladder IMT,CT,MR manifestations and clinical data of patients were systematically analyzed,and the summary of the literatures about IMT were done. Results:Clinical manifestations were hematuria,dysuria and other symptoms;CT scan showed homogeneous or heterogeneous tumors. T1WI of MRI showed low signal,and T2WI image showed slightly higher signal. Substantial part of the mass enhanced constantly under enhanced CT scan. Under microscope observation,spindle cells proliferated accompanied by mixed inflammatory cells infiltration with myxoid background.The expressions of Vim,SMA and MSA were mostly positive by immunohistochemistry method,while expressions of Actin,HHF-35 and Des were partial positive,and expressions of Kertin,EmA and S-100 were negative. Total 5 patients underwent surgical resection eventually. Conclusion:The clinical symptoms of IMT were similar to bladder cancer, and the image of IMT has characteristic manifestations. The diagnosis mainly depends on the pathological and immunohistochemical methods,and the treatment based on partial cystectomy or transurethral resection.

    • Epidemiological and etiological characteristics of bacillary dysentery in Changzhou city from 2004 to 2009

      Abstract:Objective: To analyze the epidemiological feature of bacillary dysentery and the drug resistance of this pathogenic bacterium in Changzhou city from 2004 to 2009. Methods:Reported data of bacillary dysentery from 2004 to 2009 and pathogens detected through laboratory tests were analyzed by descriptive epidemiological method. Results:A total of 8096 cases of bacillary dysentery was reported during this period, and the average incidence was 35.26/100000. The disease can occur throughout all the year with an obvious seasonal trend, and the peak period for bacillary dysentery was July to September. The incidence of bacillary dysentery in urban areas was higher than that of two county-level cities. The incidence in males was higher than that in females, and the highest incidence rate was found in people of 0- to 4- year-old and 20-to 39-year-old. The reported data also showed that bacillary dysentery might occur in all different kinds of occupation, but most were scattered inhabiting children, workers and farmers. Total of 102 strains of Shigella flexneri and 11 strains of Shigella sonnei were detected in the laboratory. Both strains had high resistance to ampicillin, sinomin compositea and nalidixic acid. Conclusion:The incidence of bacillary dysentery in Changzhou city was gradually declining from 2004 to 2009, however, the prevention and control can not be ignored because the incidence of bacillary dysentery still maintains a certain level. In order to control the incidence of bacillary dysentery in Changzhou city, the health education in focus groups and the epidemic dynamic monitoring need to be enhanced in our future work.

    • A case control study on the risk factors of viral diarrhea in children below 5 years old

      Abstract:Objective: To assess the risk factors of viral diarrhea among children below 5 years old. Methods:A case-control study was conducted on two groups of children matched by sex, age with or without viral diarrhea from hospitals in Xuzhou and Zhenjiang area. The pathogen of diarrhea was confirmed by ELISA and RT-PCR methods. Interview was carried out with uniform designed questionnaires. Cox’s proportional hazards regression analysis was then performed. Results:Factors that were independently associated with the development of viral diarrhea, would include children’s address, weight, parents’ Schooling and profession, washing-up sink in kitchen, cutting cooked and uncooked foods by different kitchen knife and target, raising pets, cooker washing hands before cook, the children contacting animals and taking antibiotics one week ago before diarrhea. Anamnesis and rotavirus vaccine inoculation were not found associating with viral diarrhea. Multivariate logistic regression model was then fitted with three variables, including washing-up sink in kitchen, cutting cooked and uncooked foods by different kitchen knife and target, taking antibiotics one week ago before diarrhea. The 95% CI of OR was 0.340~0.852, 0.295~0.705 and 2.153~6.227 separately. Conclusion:The risk of viral diarrhea increased with worse hygienic habits and antibiotics taken.
