Abstract:Objective:To explore the fluctuant trend of sex ratio of newly-born population in Jiangsu Province. Methods:The population sex ratio was obtained by the government’s official web site and Jiangsu family planning commission. Results:The birth sex ratio of Jiangsu Province was high. There were differences among 13 cities, as well as between urban and rural areas. Birth control policy also affected birth sex ratio. Conclusion:High birth sex ratio does great damages to building a solid family and keeping family friendly and stable. It is also disadvantageous to the peace and harmony of society and harmful to maintaining the low fertility level.

Abstract:Objective:To construct luciferase reporter plasmids of full-length and truncated promotors of rat Caspase 8 gene and detect their activity in HEK293 cells in response to interferon regulatory factor-1(IRF-1) overexpression,screening the possible binding sites for IRF-1. Methods:Rat Caspase 8 promoter(-1136~+101 nt) was amplified by PCR and cloned into the luciferase reporter plasmid(pGL3-basic). The recombinant plasmid(pGL3-Caspase 8-FL) and rat interferon regulatory factor-1(IRF-1) expression plasmid(pcDNA3.1-IRF-1) were co-transfected into HEK-293 cells and then the luciferase activity was detected to determine the role of IRF-1 in Caspase 8 gene transcription. Meanwhile,the potential IRF-1 binding sites within Caspase 8 promoter were predicted by bioinformatics software. Based on the predicted results,different luciferase reporter plasmids of truncated Caspase 8 gene promotor that named pGL3-Caspase 8-1~4 were constructed. The promoter luciferase reporter plasmids of pGL3-Caspase 8-FL or pGL3-Caspase 8-1~4 and the plasmid of pcDNA3.1-IRF-1 were co-transfected into HEK293 cells. Then,the luciferase activity was detected to screen the IRF-1 binding sites. Results:It was verified that different kinds of plasmids were all constructed correctly by PCR analysis and nucleotide sequencing. The plasmids of pGL3-Caspase 8-FL and pcDNA3.1-IRF-1 were also co-transfected into HEK293 cells,and then the luciferase activity was detected. The results showed that the transcriptional activity of Caspase 8 gene was increased markedly in response to IRF-1 overexpression. In addition,the plasmids of pGL3-Caspase 8-FL or pGL3-Caspase 8-1~4 and pcDNA3.1-IRF-1 were co-transfected into HEK293 cells,and then the luciferase activity in different groups was determined. The result displayed that the activity of pGL3-Caspase 8-4 was much lower than that in pGL3-Caspase 8-2 and pGL3-Caspase 8-3,indicating that the region of rat Caspase 8 promoter (-336~ -136 nt) might contain IRF-1 binding element. Conclusion:The rat full-length and truncated rat Caspase 8 promotor luciferase reporter plasmids were constructed successfully,and the IRF-1 binding region was identified.

Abstract:Questionnaire surveys were performed in six hospitals of Jiangsu and Ningxia Provinces to understand doctors’ and patients’ cognition and demand on doctor-patient communication. The results showed that doctors’ and patients’ cognition on doctor-patient communication was not completely consisted with each other, as most patients thought that their relationship with doctors was good. Both doctors and patients were highly recognized the role of doctor-patient communication.

Abstract:The development of private hospitals plays an important role in solving the medical treatment issues. In the article, the combination of SWOT analysis and analytic hierarchy process (AHP) were used to quantify the problems and make the strategic plan of the private hospital in Nanjing which was Taiwan-funded. Based on the analyse of internal and external environment of the hospital, and consultation and evaluation of experts, the authors advanced that at present the hospital should be a fight role in the strategic areas through AHP-SWOT quantitative method, and adjust the development strategy combining with the survival environment.

Abstract:Objective:To explore the current situation of occupational identity for physicians in Nanjing. Methods:1 186 physicians from 27 medical institutions in Nanjing were investigated by questionnaires and interviews. Results:The recognition rate was relatively higher in the aspect of occupational cognition, behavior, value and emotion, but lower in occupational commitment. The occupational identity was statistically different in gender, education level, weekly working hours and hospital grade(P < 0.05). Conclusion: Specific management measures should be carried out according to the impact of different demographic variables on the occupational identity of physicians.

Abstract:The article investigated the mental health status of graduate students by using SCL-90 scale,discussed the psychological characteristics by using the self-designed questionnaire of graduate students mental health. The five year longitudinal research found that graduate students mental health abnormality rate of any factor above 2.5 points was 4.9﹪~2.5﹪,any factor above 3 points was 1.0﹪~3.5﹪.The mental health status of whole female graduate students was better than the whole male graduate students. The aggregation of ten factors was very common between two groups of male and female graduate students. The needs of subjective mental health of male and female graduate students had a little difference. In addition,the author put forward five specific educational suggestions.

Abstract:Objective: To explore the happiness status and its related factors of medical graduates, and provide an evidence to improve subjective well-being of medical students. Methods: 260 medical graduates and PhD students of Nanjing Medical University were randomly selected as the research sample,and analyzed by the general well-being scale. Results: Subjective well-being scores of medical graduates were in the mid to upper level, which were higher than the national norm. There were significant differences in terms of gender. The scores of life satisfaction and interest, energy,relaxation and tension had significant difference in different age groups, and these scores of PhD were higher than the graduates. Conclusion: Gender, education and age have some effect on subjective well-being of medical graduates.

Abstract:Objective: To investigate the psychological anxiety status of contemporary medical graduates, provide scientific evidence to strengthen the mental healthy and implement psychological intervention for them. Methods: By using cluster sampling method,645 medical graduates were investigated with self-rating anxiety scale (SAS). Results: The anxiety index of graduate students was 41.62 ± 7.46,the total score and standard score were higher than the national norm(P < 0.01). Students with anxiety tendency account for 15.65% of the total survey, and the extent of anxiety tendency related to gender, grade, majors and degrees, etc. Conclusion: The anxiety tendency of medical graduates is obvious. It is proposed to strengthen the psychologically healthy education during the professional knowledge training of graduate students.

Abstract:This study summarizes our experience on the application of endoscopic technology in improving urological teaching effectiveness. Abstract:The authors detected 391 papers of Stomatology received from August 1,2012 to May 1 2013 with China academic misconduct detection system for analysis. It was found that the manuscript copy of 14.58% paper was larger than 30%. The clinical research,especially summary and orthodontics papers were very notable. The papers copying from other journals and dissertations were overlapped among a major part of treatment or research method,the concept of the ideas of others and citation part,and the evaluation index and standard of curative effect evaluation. Abstract:Contemporary metaphor theory holds that metaphor,rather than a rhetoric method affiliated with languages,is a tool of human cognition and a process of reshaping human experience. This study,drawing upon new theories of metaphor,attempts to make an overall exploration into the conception of “disease” through collecting and analyzing a vast body of English for medical purpose(EMP) literatures. The related metaphorical expressions and conceptual metaphors from the live language were analyzed,compared and explained from a cognitive view,thereby strongly confirming the ubiquity of metaphor in EMP discourse.

Abstract:Objective:This study examined the influence of sigma-1 receptor (σ1R) knockout on affective behaviors. Methods:We employed 5-month-old σ1R knockout (σ1R-KO) mice. Depression-like behaviors were evaluated through open-field test (OFT),tail suspension test (TST),forced swimming test (FST) and Y-maze task. Results:In comparison with male wide-type (WT) mice,male σ1R-KO mice spent less time in the center field of OFT,and had longer immobility time in FST and TST. Furthermore,the male σ1R-KO mice spent less time in the novel arm than the known arm in Y-maze. By contrast,no difference in depression-like behaviors was found between female WT mice and σ1R-KO mice. In addition,the serum levels of estrogen in female KO mice and testosterone in male KO mice were not altered compared to WT mice. Conclusion:The σ1R-KO induced depression-like behaviors were male mice-specific,and maybe related with the antidepressant effect of estrogen.

Abstract:Objective:To investigate the effect of melanoma inhibitory activity (MIA) on mesoderm formation during the embryogenesis of Xenopus lavies. Methods:The MIA sequence of Xenopus lavies was verified in EST database,and then cloned into pCS2+ for sequencing. The expression of MIA was detected during embryo development by RT-PCR analysis. The micro-injection of morpholino(MO) was used to knockdown MIA. The expression of marker genes was detected during germ layer formation by whole mount in situ hybridization. Results:The MIA sequence of Xenopus lavies was precisely cloned. The expression of MIA was detected at all the stages during embryogenesis. The knockdown of MIA by MO injection caused the delayed blastopore closure,inhibited the expression of mesodermal marker gene xbra compared to normal embryos. Conclusion:MIA is expressed during embryogenesis of Xenopus lavies,and MIA could play an important role in the mesoderm formation.

Abstract:Objective:To investigate the role of endothelial progenitor cells(EPCs) in modulation of ischemic injury in a nephron sparing surgery (NSS) rat model by ischemic preconditioning (IPC). Methods:Sprague-Dawley rats were randomly divided into three groups after right-side kidney nephrectomy:the Sham group underwent surgery without vascular clamping;the NSS group,the renal blood vessels were clamped for 40 min and partial nephrectomy(PN) was performed;the IPC group,the rats were pretreated with 15 min ischemia and 10 min reperfusion. At 24 h after reperfusion,the pool of circulating and kidneys were harvested. We assessed the extent of renal injury,observed the home of EPCs and detected angiogenesis as well as the expression of vascular growth factor. Results:Pretreated rats exhibited significant improvements in renal function and morphology. The number of EPCs in the kidneys was increased in the IPC groups as compared to the NSS or sham groups. Neovascularization in peritubular capillaries was markedly increased in the kidneys which treated with IPC. In addition,the expression of SDF-1 mRNA in kidneys of pretreated rats was increased compared to rats subjected to ischemic injury. Conclusion:IPC may attenuate renal ischemic injury induced by NSS;EPCs play an important role in renal protection,which involves the promotion of angiogenesis through the release of several angiogenic factors.

Abstract:Objective:To investigate the influences of human herpesvirus 6 variant A DR7 gene on the proliferation, migration and invasion of human glioma U87 cells. Methods:Lentiviral vectors pLenti6.3-DR7-IRES2-EGFP was constructed and then transfected to human glioma cells U87. A separate U87 cell line stably expressing DR7 was established after blasticidin screening. The cell proliferation was detected by cell counting Kit-8 and the cell cycle was mearsured by flow cytometry. The migration and invasion abilities were detected by wound healing assay and transwell assay, respectively. Results:The pLenti6.3-DR7-6 × His-IRES2-EGFP lentivitral expression vector was constructed and U87-DR7-EGFP cell line stably expressing DR7 gene was established successfully. Compared with U87-NC-EGFP and U87 cells, The cell proliferation of U87-DR7-EGFP cells was significantly increased. Flow cytometry showed that the overexpression of DR7 increased the proportion of U87 cells at S and G2/M phase compared with U87-NC-EGFP and U87 cells. The percentages of S phase were (34.73 ± 1.12)%, (24.89 ± 0.93)%, (25.39 ± 0.96)% (P < 0.001), and G2/M phase were (17.35 ± 1.61)%, (11.36 ± 1.50)%, (13.17 ± 1.95)% (P < 0.05), respectively. The cure rates were significantly increased in the U87-DR7-EGFP cells compared with U87-NC-EGFP and U87 cells: 6 h cure rates were(33.55 ± 2.83)%, (23.50 ± 3.18)%, (22.03 ± 1.47)% and 12 h cure rates were (70.50 ± 5.39)%, (53.60 ± 4.67)%, (55.09 ± 2.83)%, respectively. The average number of invasive cells in each visual field was significantly increased in the U87-DR7-EGFP cells compared with that of the U87-NC-EGFP and U87 cells (543.00 ± 22.94, 387.00 ± 15.63, 412.00 ± 20.30, P < 0.001). Conclusion:Human herpesvirus 6 variant A DR7 gene promote the proliferation, migration and invasion of human glioma cell U87 in vitro, suggesting that it may play a role in the development of glioma

Abstract:Objective:To investigate the therapeutic effect of genipin on renal cell carcinoma,and the relationship of UCP2. Methods:Renal carcinoma cells were treated with different concentrations(0,40,80,120 μmol/L)of genipin. Cell growth inhibition and apoptosis were detected by assay of MTT and flow cytometry. Expression of UCP2 was detected by PCR and ELISA. Contents of ROS and Ca2+ were tested by fluorescence probe. Results:Genipin significantly suppressed the growth and promoted apoptosis rate of renal carcinoma cells,statistical significance were found in 80 and 120 μmol/L groups. Expression of UCP2 was significantly increased in 80 and 120 μmol/L groups. Genipin improved contents of ROS and Ca2+,and statistical significance were found in 80 and 120 μmol/L groups also. Conclusion:Genipin can suppress renal carcinoma cells growth,and UCP2 may be involved in its mechanism.

Abstract:Objective:To investigate whether miR-135a/b could modulate the drug resistance of the human lung cancer cell line A549/CDDP to cisplatin(CDDP),and explore the mechanism of miR-135a/b on the CDDP sensitivity of A549/CDDP cells. Methods:MiR-135a/b expression was measured by quantitative real-time PCR. Transient transfection was used in A549 and A549/CDDP cell lines. Cell viability was detected by MTT assay. MCL1 3’-untranslated region-based luciferase reporter plasmids was constructed to testify the target gene of miR-135a/b.Protein expressions were measured by western blot. Flow cytometry was used to detect CDDP-induced apoptosis. Results:We found that miR-135a/b were downregulated while MCL1 was upregulated in A549/CDDP cells,compared with the parental A549 cells. In vitro drug sensitivity assay demonstrated that the over-expression of miR-135a/b sensitized A549/CDDP cells to cisplatin. The luciferase activity of MCL1 3’-untranslated region-based reporter constructed in A549/CDDP cells suggested that MCL1 was the direct target gene of miR-135a/b. Enforced miR-135a/b expression reduced MCL1 protein level and sensitized A549/CDDP cells to CDDP-induced apoptosis. Conclusion:This study demonstrated that hsa-miR-135a/b could play a role in the development of CDDP resistance in lung cancer cell line at least in part by modulation of apoptosis via targeting MCL1.

Abstract:Objective:To investigate the effects of wogonin on human glioblastoma cell lines U87,and the role of miR-128 in the mechanism of wogonin effect on U87 cells. Methods:U87 cells were treated by wogonin within three different concentrations(25,50,100 μmol/L) for 24 h,48 h,respectively. The morphological change of U87 cells were observed by using fluorescence microscope. The MTT was used on cell proliferation test. Flow cytometry(FCM) was performed to analyze the cell cycle and cell apoptosis. The qRT-PCR of miR-128 was detected from U87 cells after treated with or without wogonin. Results:Obvious appearances of cell apoptosis were observed under fluorescence microscope,including chromatin condensation and nuclear fragmentation. The analysis of MTT assay showed that the cell proliferation of U87 was inhibited by wogonin and associated with dose- and time-dependent manner. The FCM showed that the U87 cells treated by wogonin were arrest in S phase and the apoptosis ratio of U87 treated by wogonin was significantly increased after 24 h. The expression of miR-128 increased after being treated with wogonin. Conclusion:Wogonin induces glioblastoma cell U87 apoptosis and may restrain cell proliferation through miR-128.

Abstract:Objective:To investigate the effect of Toll-Like receptor 4(TLR4) on the severity of acute graft-versus-host disease(GVHD) after haploidentical stem cell transplantation (HSCT) in mouse and explore its potential mechanism. Methods:Establish a mouse model of MHC haploidentical bone marrow transplantation. The mice were randomly divided into the experimental group 1(n=12):each mouse was received 20 μg/kg infusion of TLR4 agonist LPS; the experimental group 2(n=12):each mouse was received 20μg/kg infusion of TLR4 monoclonal antibody; the control group(n=12):six mice were received 20 μg/kg infusion of sterile saline; other six mice without any treatment after transplantation. We observed the clinical manifestation of GVHD and the survival rate by using Kaplan-Meier analysis. On the 14th day after transplantation,the histopathology of GVHD was evaluated in the liver,and the change of serum IL-1β,TNF-α was detected. Results:Compared with the control group,the mice in the experimental group 2 had reduced severity of acute GVHD symptoms,improved survival rate(P < 0.05) and less histopathological evidence of GVHD. Correspondingly,the above changes of the mice in the experimental group 1 were severe. Conclusion:The TLR4 inhibitors can prevent acute GVHD in murine haplo-HSCT model.

Abstract:Objective:To explore the impact and possible mechanism of praeruptorin C on cardiac hypertrophy in spontaneously hypertensive rats(SHR) and its possible mechanisms. Methods:Totally 20 SHR were randomly divided into the praeruptorin C group[praeruptorin C 20 mg/(kg·d)intragastric administrated,n=10]and the untreated control group(n = 10),with 10 age matched Sprague Dawley rats as normal group. The rats of each group were intervened for 8 weeks. We calculated the heart quality mass index (MHI) and left ventricular mass index(LVMI). We observed the morphological and structural changes of cardiomyocytes with optical microscope,and then the injuries of cell ultrastructure and mitochondria with transmission electron microscope. Then,we made homogenate and extracted RNA taken from heart tissue. The gene expression levels of phospholamban were determined by the method of semiquantitative RT-PCR. The phosphorylation of PLB was counted by liquid scintillation. Results:Compared with the SHR control group,the LVMI and HMI of SHR treated with praeruptorin C for 8 weeks were decreased(P < 0.05),the cardiac hypertrophy were reversed, the levels of PLB mRNA expression and the phosphorylation of PLB of the rats in the praeruptorin C group were increased (P < 0.05). Conclusion:Praeruptorin C had a certain effect on reversing the cardiac hypertrophy. The possible mechanisms might be upregulating the gene expression of sarcoplasmic reticulum phospholamban and increasing the phosphorylation level.

Abstract:Objective:To investigate the effect of sildenafil citrate on left ventricular function and the levels of serum TGF-β1 and hydroxyproline(Hyp) content of myocardial tissue in rats with chronic heart failure (CHF). Methods:Female SD rats were randomly divided into the control group,the CHF group and the CHF + sildenafil treated group(n=7). The control rats derived from the normal rats; the CHF model rats were induced by intraperitoneal injection of doxorubicin; the CHF + sildenafil group were received the sildenafil 21 mg/(kg·d) gavage for 4 weeks after the success of CHF model rats. The CHF group and the control group rats were received placebo(1 ml distilled water) gavage for 4 weeks. After 10 weeks,the LVDd,LVDs,EF and FS of rats were measured by transthoracic echocardiography; the levels of serum TGF-β1 were tested by ELISA reagent kit and collagen content in myocardial tissue was determined by hydroxyproline assay. Results:Compared with the control group and the treatment group,the LVDs,LVDd,and the levels of serum TGF-β1 of the CHF group were significantly increased (P < 0.01) and the EF,FS were significantly decreased (P < 0.01); Hyp content in cardiac tissue of the CHF group was significant increased compared with the control group (P < 0.01) and the treatment group (P < 0.05). Conclusion:Sildenafil could improve the left ventricular function and reduce the deposition of collagen in the myocardial tissue and the levels of serum TGF-β1 in the rat models of CHF.

Abstract:Objective:To study the influence of osteoclast generation and function in different methods of osteoclast culture in vitro. Methods:We used 3 methods of osteoclast culture in vitro:the group A,10 ng/ml M-CSF was added in mice marrow cell for 24 h. Cells which not adhere to the surface were pre-induction with 30 ng/ml M-CSF for 3 d,and then 50 ng/ml M-CSF + 100 ng/ml RANKL were added;the group B,mix culture mice marrow cell with mice osteoblast on 10∶1,and 10-6 mol/L PGE2 and 10-8 mol/L VitD3 were added;the group C,RAW264.7 was added with 100 ng/ml RANKL to inducement. Osteoclastogenesis and their resorption function were examined by TRAP staining and scanning electron microscope (SEM) observation of dentin resorption lacunaes. The gene expressions of NFATc1 and c-Fos were also detected by Real-time PCR. Results:TRAP positive multinuclear cells were observed and resorption lacunaes were formed in each group. However,the group B showed more TRAP positive multinuclear cells and larger amount of resorption lacunaes than the group A and C. The group C showed more TRAP positive multinuclear cells but less amount of resorption lacunaes than the group A. Real-time PCR detection also showed that the gene expressions of NFATc1 and c-Fos were higher in the group B than the group A and C. Conclusion:As the comparison of three methods of osteoclast culture,the group B is better than the group A and C in osteoclastogenesis and resorption function. The group A is better than the group C in the resorption function of osteoclast,but the group C is better than the group A in osteoclastogenesis.

Abstract:Objective:To conjugate anti-EMMPRIN monoclonal antibody on the surface of carboxyl-SPIO nanoparticles. The efficiency of superparamagnetic iron oxide (SPIO) conjugated with anti-EMMPRIN monoclonal antibody and the size of anti-EMMPRIN-SPIO were investigated in different pH values. Methods:Anti-EMMPRIN monoclonal antibody conjugated SPIO nanoparticles were performed through a coupling strategy of EDC and sulfo-NHS. Bradford assay and SDS-PAGE analysis were used for measuring the conjugation efficiency. The size of anti-EMMPRIN-SPIO was evaluated by dynamic light scattering method. Results:The smallest diameter of 63.15 nm was obtained when pH 7.8 was used. The lower concentration of antibody in supernatant with pH6.8 and pH7.3 was 1.943 2 × 10-4 g/L and 3.511 1 × 10-4 g/L, respectively. Anti-EMMPRIN mAb was detected in lanes of pH6.8, pH7.3 and pH7.8 which revealed two bands with weight of 55 000 and 25 000. Conclusion:EDC/sulfo-NHS method is an efficient method for conjugating anti-EMMPRIN monoclonal antibody on SPIO nanoparticles. Under the amidation condition with pH value of 6.8, 7.3 and 7.8, higher conjugation efficiency and smaller size of anti-EMMPRIN-SPIO were obtained. The results showed that pH value is a very important factor which influences conjugation efficiency and the size of anti-EMMPRIN-SPIO.

Abstract:Objective:To investigate the expression of IL-17 in human thyroid papillary carcinoma tissues, and explore the role of IL-17 in the development of papillary thyroid cancer. Methods:Real-time RT-PCR,Western blot,immunohistochemistry assay were used to detect the expression of IL-17 in 60 cases of papillary thyroid carcinoma,10 cases of nodular goiter,and 10 cases of Hashimoto’s disease tissues. Statistics methods were applied to analyze the results. Results:①The level of IL-17 mRNA in papillary thyroid carcinoma tissues was higher than that in nodular goiter tissues,and lower than that in Hashimoto’s disease tissues. While While the level in paraneoplastic tissues was higher than that in the cancerous tissue,P < 0.05;②IL-17 protein in nodular goiter tissues was rarely seen,but expressed in papillary thyroid cancer,higher in adjacent tissues and highest in Hashimoto’s disease tissues by comparing the Western blot’s gray value;③Immunohistochemistry assay showed the expression of IL-17 in papillary thyroid cancer and Hashimoto’s disease tissues,and no expression in nodular goiter tissues. The dyed brown areas in thyroid cancer were mainly located on the edge of the tumor;④Clinical data statistics showed that the expression of IL-17 had no correlation with the patient’s age,sex,tumor numbers,but related with tumor size,lymph node metastasis and TNM staging. Conclusion:The expression of IL-17 increases in papillary thyroid carcinoma tissues. It may be involved in the occurrence and development of thyroid cancer by interacting a variety of cytokines and inflammatory cells.

Abstract:Objective:To investigate the killer immunoglobulin-like receptor (KIR) genotype in patients with chronic myelogenous leukemia(CML). Methods:The sequence specific primer polymerase chain reaction (SSP-PCR) was employed to identify the KIR genes and pseudogenes in 84 CML patients. Results:The most frequent genotypes were KIR frame genes,including 2DL4,3DL2,3DL3 and 3DP1 (100%),followed by 2DL1,2DL3,2DP1 and 3DL1 (all above 80%). The frequencies of 2DL2 and 2DS2 in CML patients were lower than those in the control group,but there were no statistically difference between the two groups except 2DL5B (OR = 0.208,P < 0.05). Conclusion: KIR 2DL5B may be associatied with CML.

Abstract:Objective:To investigate the expression and significance of MT1-MMP in the synovium of knee joint of patients with osteoarthritis. Methods:We detected the expression of MT1-MMP protein in 80 cases of osteoarthritis by using.immunohistochemically and Western blot. Results:The expression of MT1-MMP protein in the synovium of knee joint of patients with osteoarthritis was markedly higher that in the normal synovium (χ2=29.46,P < 0.01), and the MT1-MMP protein was mainly located in the cytoplasm; The results of Western blot showed that the expression of MT1-MMP protein in the synovium of knee joint of patients with osteoarthritis(1.042 ± 0.219) was significantly higher than that in the non-osteoarthritis (0.256 ± 0.050,F = 12.838,P = 0.001). Conclusion:There is a close relation between the expression of MT1-MMP protein and OA, it may be the marker of diagnosis and the judgment of osteoarthritis.

Abstract:Objective:To determine the effect of obesity on bone mineral density of lumbar spine and hip in male patients with type 2 diabetes. Methods:The study was carried out in 199 male with type 2 diabetes. Bone mineral density (BMD) at the sites of lumbar spine and hip (lumbar spine L1~L4,femoral neck,Ward’s triangle and total hip) were obtained by dual X-ray absorptiometry. The influence of obesity on BMD was evaluated. Results:According to body mass index,BMDs at lumber spines,total hip,femoral neck and ward’s triangle were significantly higher in obesity subjects compared with non-obese subjects. BMDs at lumber spines and total hip were significantly higher in visceral obesity objects define as waist circumference not less than 90 cm. The significant difference was disappeared in femoral neck and Ward’s triangle. Compared with subjects with normal BMD and osteopenia,those with osteoporosis had lower weight,BMI,waist circumference and hip circumference and were older. BMDs were positively correlated with height,weight,BMI,waist circumference and hip circumference. Only BMDs at hip were negatively correlated with age. After correction of height and weight,BMDs at femoral neck negatively correlated with waist-to-height ratio (r = -0.179,P = 0.019) and hip circumference (r = -0.179,P = 0.019). BMDs at ward’s triangle negatively correlated with waist (r = -0.177,P = 0.019), hip circumference (r = -0.227,P = 0.002) and waist-to-height ratio (r = -0.229,P = 0.002). Conclusion:Visceral obesity in male type 2 diabetes patients is related to lower BMDs at hip and the risk of osteoporosis.

Abstract:Objective:To investigate the drug resistance of Klebsiella pneumonia in ventilator associated pneumonia in pediatric intensive care unit (PICU) in Suzhou,and provide the basis of prevention and clinical treatment. Methods:Children recruited in PICU from Aug. 2011 to Jan. 2012 who met the diagnostic criteria of ventilator associated pneumonia(VAP) and sputum culture showed Klebsiella pneumonia positive were studied. According to extended-spectrum β-lactamases(ESBLs),they were divided into two groups:ESBLs positive and ESBLs negative. The difference of drug resistance was analyzed. Results:Totally 39 sputum specimens from 31 children were collected and under susceptibility test. All strains showed ampicillin resistance,32 strains showed eftazidime resistance,the drug resistance rate was increased to 82.05% (32/39). Other antibiotics resistances were cefuroxime axetil (89.74%,35/39),cefepime (76.92%,30/39) and cefotaxim (76.92%,30/39). Cefoxitin had a lower resistance (17.95,7/39). All strains were sensitive to imipenem. The drug resistance(of the 2nd,3rd and 4th generation of cephalosporin) rate in the ESBLs positive group was higher than that in the ESBLs negative group(P < 0.05). Conclusion:ESBLs positive strains are more common in VAP Klebsiella pneumonia infection in PICU in Suzhou,and have a higher cephalosporin resistance compared to the ESBLS negative strains. Carbapenem is the first choice drug to treat VAP Klebsiella pneumonia infection. Doctors should choose suitable antibiotics according to the resistance.

Abstract:Objective:To analyze the prevalence and integrons distribution of carbapenem-resistant Klebsiella pneumonia (CRKP) which isolated at the affiliated hospital of Nanjing Medical University from 2009 to 2010. Methods:All the isolates were identified by VITEK2 compact or API identification system. The antibiotic susceptibility of 81 CRKP was detected by K-B method. ClassⅠ,Ⅱ and Ⅲ integrons and their variable regions were detected by PCR. Results:The isolating rate of CRKP increased rapidly from 1.6% in 2009 to 11.2% in 2011. Most of the isolates were resistant to the β-lactams. Among the 81 isolates,92.6% of them are detected of intI1 and 78.7% of the isolates are detected of variable regions. The most common gene cassette is aadA2,followed by the aacA4-catB8-aadA1 and aadA2+dfrA25. Conclusion:The proportion of CRKP increased rapidly in our hospital and efficient measures should be carried out to inhibit the dissemination of CRKP;ClassⅠ integrons are common in the CRKP and the isolates have high capability to capture antibiotic resistance gene cassette.

Abstract:Objective:To explore the clinical application of the CAT scale in the evaluation of the health status of chronic obstructive pulmonary disease (COPD) patients in stable stage. Methods:A simple sampling survey of 104 cases of COPD patients in stable stage was enrolled. All participants were examined with CAT score,dyspnea index(mMRC),6-minute-walk test and pulmonary function test. Analysis on the correlation between CAT score and these test results was executed by Pearson correlation, and stepwise regression. Results:The associations between CAT score and FEV1%pred,6MWD,mMRC score were significant(P < 0.01). Using stepwise regression,CAT score = 30.11 + 3.00 × mMRC dyspnoea score + 0.002 × exacerbation -0.03 × 6MWD (r2 = 0.67,P < 0.001). Conclusion: The CAT scale is reliable for the assessment of the quality of life and the frequency of exacerbations can seriously impact the patients’ health status.

Abstract:Objective:To investigate the prevalence and influential factors of anxiety and depression in patients with primary open-angle glaucoma (POAG) before and after surgical interventions. Methods:Totally 83 patients with POAG were investigated with Hamilton anxiety scale (HAMA),Hamilton depression scale(HAMD),family support scale and low vision quality-of-life questionnaire (LVQOL) preoperatively,and with LVQOL,HAMA and HAMD in one-month-post operative time. Totally 83 controls were assessed by using HAMA and HAMD. We investigated the prevalence and influential factors of anxiety and depression in patients with POAG before and after surgical interventions. Results:Of all the POAG patients,49(59.0%) of them were identified as anxiety and 25 (30.1%) of them were identified as depression preoperatively. A total of 17(20.5%) patients were identified as anxiety,and 13 (15.7%) patients were identified as depression in one-month-post operative time,whereas the incidence of anxiety and depression in the control group was 8.4% and 4.8%,respectively. Multiple linear regression analysis showed that preoperative HAMA scores were correlated with sex and family support whereas preoperative HAMD scores were correlated with sex,QOL,family support and age (P < 0.05). In multivariate logistic regression,the impact factors of anxiety in one-month-postoperative time were age and QOL (P = 0.027,P = 0.009);QOL was the factor affecting depression in one-month-postoperative time (P = 0.047). Conclusion:There existed high prevalence of anxiety and depression disorders with POAG patients before and after surgical interventions and more attentions should be paid on this phenomenon.

Abstract:Objective:To discuss dosimetric characteristics and acute clinical radiotherapy effects of integrated volume for chest wall target and supraclavicular target with intensity modulated radiation therapy (IMRT) for breast cancer after mastectomy. Methods:Among 146 cases of patients with breast cancer after mastectomy,there were 43 cases of integrated volume IMRT plan(IMRT group) and 103 cases of general 3D-CRT plan(general group). Prescription dose was 50 Gy /25 times/5 weeks,and 90% of target area reached prescription dose. Dosimetry characteristics on the chest wall target,supraclavicular target area and normal organ were compared,while observing and following the partial skin reaction and radiation pneumonitis. Results:Compared with general group the plan of integrated volume with IMRT for the chest wall target area resulted in a better target conformal and uniformity, and in the plan of integrated volume with IMRT for supraclavicular target area,95% prescription dose containing the target volume,conformal and uniformity for the target region were increased obviously, and in the plan of integrated volume with IMRT for breast cancer for the normal organ,the V30 of the ipsilateral lung and V30 of the heart were decreased,while the V5 of the ipsilateral lung and heart were increased. There was no statistical difference of V20 and Dmean of the ipsilateral lung. The occurrence rat of the radiation dermatitis was 37.7%(55/146) and no statistical difference was observed between two groups. None radiation pneumonitis occurred. Conclusion:The plan for integrated volume with IMRT for breast cancer can improve the dose of supraclavicular target significantly by the quantity under the premise for organ at risk,and improve conformal and uniformity of the dose for chest wall target area and supraclavicular target volume.
