    • Analysis of the legal liability for instigating mental disorders to do harmful behavior

      Abstract:The mental health law of the PRC,which have been implemented On May 1,2013,protects the rights of patients with mental disorders. But the legal responsibility for instigating mental disorders to do harmful behavior is involved in a few. Through the interpretation of the relevant laws and regulations,and based on the existing definition of the legal responsibility for abetting the commission of crimes,the authors tried to clear the legal responsibility for instigating mental disorders to do harmful behavior,which could protect the rights of patients with mental disorders better and improve the construction of our country legal system.

      Abstract:In “Tort Liability Act”, a chapter “medical malpractice liability” based on different types of medical malpractice liability takes a different responsibility principle, in order to build a fault liability based on the principle of presumption of fault under special circumstances as well as the principle of liability without fault to supplement liability for medical damages principle system, find the best balance point that balanced injured patients, medical institutions, and the relationship between the interests of all patients, and scientifically handle the different interests relationship. However, the liability adscription principle system of medical malpractice is not strict, some problems exist in the standard design, language representation, and lacking of logic, which may affect the accurate interpretation and correct application of “Tort Liability Act”, and need further standardization and improvement.

    • The protection of medical students’ right and interest during clinical internship

      Abstract:Clinical internship is an important transformational process for medical students growing up to qualified doctors. However, the protection of medical students’ right and interest during clinical internship is weak. Malignant wound events happened in hospital influence the business confidence and employment psychology of medical students. The article analyzed the necessity of assertion medical students’ right and interest, the content of medical students’ rights, and suggested how to realize the protection of medical students’ right from legal relation, legislation and protective measure.

    • Review on mental health for the “post-90s” college students of China

      Abstract:The “post-90s” refers to Chinese citizens who were born in the 1990s and have entered university. The research showed that the mental health states of “post-90s” students were good,at least no worse than the “post-80s”,while it hadn’t yet formed a consensus conclusion on gender,grade,and urban-rural differences of mental health of the “post-90s”. Furthermore,the individual factors,such as personality,coping style and ego identity,together with the environmental factors,such as factors from family,school and society,were the main influence factors to their mental health. Therefore,in future,the relevant study should pay more attention to the positive psychology of “post-90s” students,as well as different categories students’ mental health;research methods should be integrated using the interview,the test method and experimental methods along with structural equation modeling,multilevel linear model and meta-analysis technology,and longitudinal studies should be actively carried out as well;and the education proposals should organically combine macro-regulation and micro-interventions.

    • A survey on the knowledge status of AIDS and its influence factors among residents in parts of China

      Abstract:Objective:To learn about the knowledge status of AIDS and its influence factors among residents in parts of China,and to provide evidence to the prevention and control. Methods:Multi-stage sampling method was used to indentify the research objects,the information was obtained through questionnaires,then chi-squaretest and Logistic regression were used to get the differences on the knowledge status on AIDS and its influence factors. Results:In this survey,a total of 4 372 valid questionnaires(2 291 family households)were taken back,and the respondents aged mainly concentrated in 33 to 44 years. About 34.13% of the respondents knew the AIDS window period,57.72% could distinguish patients with HIV and AIDS. The awareness rates of sexual transmission route and the blood-borne transmission route were relatively high,which were 93.12% an 89.84% respectively,and the awareness rate of non-transmission route was low. The differences between the above were statistically significant. The factors influencing the knowledge on AIDS were:different provinces,migrant people,degree of education and watching TV frequently. Conclusion:The respondents lack of basic knowledge on AIDS and have little knowledge on the awareness and preventive measures. There are some differences in basic knowledge of AIDS between different provinces in China.

    • A study of professional identity level of nurse and the relationship with the work environment in Jiangsu province

      Abstract:Objective:To investigate the level of nurse’s professional identity in Jiangsu province and its relationship with the work environment. Methods: Professional identity scale of nurse and work environment scale of nurse were used, and 360 nurses in 6 hospitals of Jiangsu province were analyzed with stratified random sampling method. Results:The scores of professional identity and work environment were 102.11 ± 14.96 and 108.65 ± 10.44. There were significant differences in age, department, seniority, position, marital status and hospital level. In addition to the “team”, the other 7 factors of work environment were significantly related to the each factors of professional identity. Conclusion:The professional identity of nurse in Jiangsu province shows a medium level. Strengthening the factors of the work environment such as “leadership and initiative”, “doctor-nurse relationship”, “control of the practice” can further improve the level of professional identity.

      Abstract:Objective:To study the stress source and the coping styles of nurses in high-quality ward. Methods:A total of 238 nurses from high-quality nursing demonstration wards of three third-grade class-A hospitals in Nanjing were investigated by a questionnaire survey. Results:The stress of nurses in high-quality wards showed statistical differences in the term of department, educational background and position. The score of positive coping styles of nurses in high-quality wards were higher than that of native coping styles. Negative coping style was positively correlated with the stress of the nurse. Conclusion:Some reasonable and effective measures should be adopted to reduce stress source and enhance job enthuse and satisfaction of nurses,including increasing the allocation of human resource,making scheduling system reasonable and promoting welfare for nurses.

      Abstract:To Jiangsu Institute of Medical Administration was the earliest predecessor of Nanjing Medical University. Chen Guofu,the senior statesman of Kuomintang established Jiangsu Institute of Medical Administration in 1934,and served as the first president. He carried out the new medical education and created new medical fusion of traditional and western medicine. During his administration, Jiangsu Institute of Medical Administration had made great progress in the idea of running a school, training system, management system and people’s livelihood services and so on, which became a unique medical college in the period of the Republic of China.

      Abstract:Whether system construction of department of infection control is adaptable to the clinical needs,department of infection control really plays its function in clinical,the existing management system is smooth and the control measures are carried out,there are difficult problems for the government to formulate relevant policies. Through visiting some medical institutions and holding informal discussions,problems of current management of hospital infection are found and the deep-seated reasons are analyzed. All in all,the team and discipline construction of infection control should be strengthened,work mode of infection management committee should be changed,training and test mode should be optimal,and the input of infection control should be increased.

      Abstract:Objective:To evaluate the effect of group counseling intervention on career decision-making self-efficacy of nursing students. Methods:48 junior nursing students in vocational colleges were randomly divided into experimental group and control group. Experimental group were received 8 weeks group counseling. Two groups of students were investigated with the career decision making self-efficacy scale(CDMSES)and coping style questionnaire (CSQ)before and after group counseling. Results:After the group counseling,the CDMSES score and mature coping style score in the experimental group were significantly higher than those in the control group(P < 0.05); immature coping style score was significantly lower than that of control group(P < 0.01). Conclusion:Group counseling intervention can effectively improve nursing students’ career decision-making self-efficacy,and enhance their ability to cope with difficulties.

    • Effect on cell viability and gene expression in human umbilical endothelial cell EA.hy926 by adenovirus mediated DEPP over-expression

      Abstract:Objective:To investigate the subcellular localization of decidual protein induced by progesterone(DEPP) as well as its effect on cell viability and gene expression in human umbilical endothelial cell line EA.hy926. Methods:Recombinant adenovirus expressing human DEPP was generated. The subcellular localization of DEPP was investigated by using immunofluorescence and Western blot assay with cells infected by ADV-DEPP. Cell viability was determined by using MTT assay. Apoptotic or necrotic cell numbers were measured by flow cytometry. Angiogenesis related protein expression pattern in response to DEPP over-expression was examined by using antibody microarray. Results:Over-expressed DEPP was localized both in the nucleus and cytoplasm in EA.hy926 cell. Compared with control cells,DEPP expression led to more severe cell shrinkage and detachment after adenovirus infection. MTT assay showed that cell viability was significantly decreased with exogenous DEPP. Flow cytometry analysis revealed that the main cell death induced by DEPP was necrosis. Finally,angiogenesis antibody microarray indicated that DEPP induced the increase of 7 genes and the reduction of 70 genes expression in endothelial cells. Conclusion:With increased expression,DEPP led to endothelial cell damage and may be involved in the regulation of angiogenesis.

      Abstract:Objective:To investigate the role of ATP7B overexpression for the survival of bone marrow-derived mesenchymal stem cells (MSCs) of LEC rat in presence of various copper concentrations in vitro. Methods:Bone marrow cells were isolated from the femurs of LEC rat by flushing with rat MSCs growth medium;the lentivirus package system carrying ATP7B/green fluorescent protein (GFP) gene was constructed,and MSCs were infected by the virus. The expression of ATP7B was measured by immunofluorescence,Western blot and Real-time PCR. The SRB assay was used to analyze the viability of MSCs and HepG2 in the presence of various copper concentrations. Results:We successfully built up LEC rat MSCsATP7B which can stably express high level of ATP7B in vitro. In the SRB assay,the viability of MSCsATP7B was significantly higher than that of either MSCsGFP or HepG2 in the presence of high copper concentrations. Conclusion:MSCs of LEC rat rescued by ATP7B can effectively antagonize copper toxicity in vitro,and may represent a novel strategy for therapy of Wilson disease.

    • The expression of miR-195 in esophageal cancer and its effect on proliferation of esophageal cancer cell lines

      Abstract:Objective:To observe the expression of microRNA-195(miR-195) in ESCC tissues and the effects of miR-195 on the proliferation of Eca109 cells. Methods:The expression of miR-195 in ten pairs of ESCC tumor tissues and adjacent normal esophageal tissues after surgical resection was detected by Taqman Real-time PCR. The miR-195 mimics was transfected into Eca109 cell by using LipofectamineTM 2000. The ability of cell proliferation and cell cycle were detected by Cell Counting Kit-8(CCK-8)and FACS,respectively. Protein was detected by Western Blot. Results:The miR-195 was down-regulated in ESCC tissues compared with normal esophageal tissues (P < 0.05). After transfected miR-195 mimics,cell proliferation was inhibited by CCK-8 assay and cell cycle was arrested in G0/G1 phase by FACS assay (P < 0.01). Western blot result indicated that Cdc42 protein was reduced after miR-195 mimics transfected (P < 0.01). Conclusion:The miR-195 could induce G1 arrest and inhibit proliferation in Eca109.

    • Construction and phenotype analysis of miR-19b transgenic mouse

      Abstract:Objective:To construct heart-special miR-19b overexpression transgenic mouse and analyze their phenotype. Methods:Firstly,we found out the suitable restriction enzyme cutting site on the plasmid which carrying alpha-MHC promoter,and design the primers according recombination principle provided by CloneEZR PCR Cloning Kit. We inserted the pre-miR-19b sequence cloned from mice chromosome into the plasmid using recombinase. Secondly,after linearization of recombinant plasmid,they were injected into the embryonic stem(ES) cells. We transplanted the recombinant ES cells into the uterus of the pseudocyesis female C57BL/6J mice. At last,we selected out the lines of transgenic mouse and analyzed the phenotype of TG mouse. Results:We successfully constructed miR-19b transgenic mouse. The scores of interventricular septal thickness,left ventricle posterior wall and ejection fraction of miR-19b transgenic mouse were increased. Conclusion:The overexpression of heart-special miR-19b may lead to cardiac hypertrophy.

    • Expression of KSHV K1 protein in vascular endothelial cells and preliminary study on its function

      Abstract:Objective:To obtain human umbilical vein endothelial cells (HUVECs) that stably expressing KSHV K1 protein and explore the effect of K1 protein on the proliferation and migration ability of HUVECs. Methods:The constructed fragment of K1 gene from expression plasmid pCI-neo-K1 was cloned into the lentiviral vector pHAGE-CMV-MCS-IZs-Green. We co-transfected the recombinant plasmid pHAGE-K1,the packaging plasmid psPAX2 and the envelope plasmid pMD2.G into 293T cells. Culture media were harvested and filtered through a 0.45 μm filter to remove the cells. The expression of K1 protein in recombinant lentivirus-infected HUVECs was detected by Western blot assay. Next,HUVECs stably expressing KSHV K1 protein were obtained by flow cytometry assay (FCM) screening and verified the expression of K1 protein by Western blot assay again. Finally,the effect of K1 protein on the proliferation and migration ability of HUVECs was detected by CCK-8 assay and wound-healing assay. Results:Nucleic acid sequencing confirmed that cloned K1 gene was 906 bp,wihch was 100% homologous with K1 gene registered in GenBank. The exact band of K1 protein in recombinant lentivirus-infected HUVECs was detectable by Western blot assay. The results of CCK-8 and wound-healing assay showed that the proliferation and migration ability of HUVEC stably expressing KSHV K1 protein was significantly increased than the corresponding control(P < 0.01 and P < 0.05,respectively). Conclusion:The recombinant lentivirus carrying KSHV K1 gene was packaged successfully,and K1 protein could promote the proliferation and migration of HUVECs.

    • Preparation and immunogenicity characteristics analysis of a epitope fusion protein of Epstein-Bar virus encoded latent membrane protein 2A

      Abstract:Objective:To prepare a immunogenic epitope tandem of latent membrane protein 2A (LMP2A) of Epstein-Barr virus and analyze its characteristics of immunology. Methods:LMP2A epitopes were analyzed by using DNAstar software,and two immunogenic epitopes were selected due to stronger immunogenicity. The genes of these two epitopes were integrated through gene synthesis,and then cloned into prokaryotic expression vector pET28a. The recombinant epitopes were expressed and purified by E.coli BL21,and then the protein was utilized to immunize mouse to prepare anti-epitope fusion protein polyclonal antibody after the analysis of SDS-PAGE and Western blot assay. The titer of the antibody was assessed by ELISA and the immunohistochemical experiment was performed to test the specificity of the polyclonal antibody for natural LAMP2A. Results:Through prokaryotic expression and purification,high purity fusion protein was obtained. By mouse immunization,anti-LMP2A polyclonal antibody of high titer and specificity was prepared,which can be applied to ELISA and immunohistochemical analysis. Conclusion:An epitope fusion protein,which shares the natural antigen immunogenicity,can be used to prepare the polyclonal antibody specific for LMP2A. This study has laid a solid foundation for screening whole humanized genetic engineering antibody by epitope fusion protein.

    • Cloning,expression and purification of a recombinant Tat-PFKMVV peptide in Escherichia coli

      Abstract:Objective:We sought to construct the recombinant expression vector pED-Tat-PFKMVV to express and purify the recombinant peptide Tat-PFKMVV. Methods: The gene fragment encoding a recombinant peptide Tat-PFKMVV was constructed by PCR and ligated into expression vector for recombinant expression in a T7 RNA polymerase-based expression system. The recombinant plasmid vector was transduced into E.coli. The expressed recombinant peptide was partially purified by the wash solution of Tris-Cl/TritonX-100,alcohol precipitation,acid hydrolysis and isoelectric point precipitation. Results:The correct recombinant DNA coding fragment was validated by DNA sequencing. The 17 000 AsnB-C-Tat-PFKMVV fusion protein was highly expressed in E.coli BL21 transduced by pED-Tat-PFKMVV. The analysis of the purified Tat-PFKMVV by mass spectrometry indicated that its molecular weight was 2 659. GST-pull down experiments showed that Tat-PFKMVV in vitro disturbed the coupling of nNOS and PFK-M. Conclusion:The experiment constructed the recombinant expression vector of AnsB-C-Tat-PFKMVV,and expressed and purified the peptide Tat-PFKMVV. The novel preparation method is a potentially useful strategy for the large-scale preparations of bioactive peptides due to its high yield and simple steps.

      Abstract:Objective:To investigate the effects and mechanism of myosin light polypeptide 4(MYL4) on injury caused by viral myocarditis in mice. Methods:The experimental animals were randomly divided into the viral myocarditis group (n = 30) and the control group (n = 10). The viral myocarditis models were achieved by infection with coxsackievirus B3(CVB3). The proteomics assay was performed to identify the different proteins involve in the pathogenesis of viral myocarditis. The expression and the role of one identified protein MYL4 were evaluated by Western blot and ELISA. Results:Six proteins were successfully identified by MS:MYL4, heat shock protein β1, isocitrate dehydrogenase subunit α mitochondrial precursor, voltage dependent anion-selective channel protein 2, proteasome subunit α type 1 and macrophage capping protein. The expression of MYL4 was higher in the heart tissue and serum of VMC mice (P < 0.01), and ELISA results showed that the expression of MYL4 was related to the severity of disease(r = 0.97,P < 0.00). Conclusion:The MYL4 expression of VMC was higher than that of the control group, and also could indicate the injury degree, which suggesting that MYL4 might be involved in the pathogenesis of VMC.

      Abstract:Objective:To investigate the effect of IKKα on inflammatory cell infiltration and the expression of inflammatory mediators in the repair phase of ischemia-reperfusion(IR)renal injury in mice. Methods:C57BL/6 mice were divided into 4 groups,including the Sham group,the IR group,the interference group and the control of interference group. The interference group and interference control group were injected with lentivirus in renal two weeks ago. The bilateral renal pedicle was occluded for 20 min and then with reperfusion of 3 days to establish IR renal injury model. The sham group only separated bilateral renal pedicle without clipping. Serum creatinine,blood urea nitrogen and HE staining were preformed to evaluate renal function and morphological changes. The macrophages(F4/80+)infiltration and proliferation factor Ki67 expression were detected by immunohistochemical detection, and the distribution and the expressions of IKKα,p-IKKα and inflammation mediators tumor necrosis factor-α(TNF-α),interleukin-18(IL-18) and IL-1β were detected by Western blot. Results:Compared to the control of interference group and the IR group,the serum creatinine and blood urea nitrogen of the interference group was significantly higher(P < 0.05)and renal pathological damage was severer. The cell proliferation factor Ki67 expression was significantly decreased in the interference group(P < 0.05); the infiltration of macrophage and the expression of inflammatory mediators of TNF-α,IL-18 and IL-1β were increased(P < 0.05). Conclusion:The expression of IKKα has effect and promote the repair of renal ischemia-reperfusion injury.

      Abstract:Objective:To investigate the role of mineralocorticoid receptor(MR)in regulating the function of mouse primary brown adipocyte. Methods:The primary brown adipocytes were separated from C57BL/5J mice and induced for differentiation. The expression of genes related to brown fat function, including uncoupling protein-1(UCP1), the cell death-inducing DNA fragmentation factor-α-like effector, domain- containing protein(CIDEA), peroxisome proliferator-activated receptor alpha(PGC1α) and fatty acid binding protein 4(FABP4) were observed in MR ligands(aldosterone), MR antagonist(spironolactone)and glucocorticoids(cortisone)treated cells, respectively. Results:Mineralocorticoid could significantly increase the expressions of UCP1,PGC1α and FABP4. Conversely, the expression of UCP1 was suppressed by MR antagonists. Moreover, MR regulated the brown adipocyte function with the interaction of glucocorticoid. Conclusion:MR plays an important role in the function of primary brown adipocyte.

    • The effects of diabetes-like environment on P19 cell for myocardial cell differentiation

      Abstract:Objective:To investigate myocardial transcription factor GATA4 and myocardial specific marker cardiac tropenin T (cTnT) expression of P19 cells differentiated into cardiomyocytes in diabetes-like environment. Methods:P19 cells were induced to myocardial cells in supplemented diabetic teratogenic medium. Real-time PCR and Western blot were performed to detect GATA4 and cTnT expressions in the two groups on differentiated process. Results:Compared with the control group,the elementary bodies(EB) that cultured in diabetes-like environment propagated obviously slower and the aggregation outgrowths were scattering and sparse. The volumes of GATA4 in diabetes-like environment medium on day 6,8 and 10 (0.05 ± 0.06,0.11 ± 0.04 and 0.45 ± 0.12) of culture were significantly lower than the same time of the control group(1.00 ± 0.04,0.44 ± 0.06 and 0.78 ± 0.20) of culture respectively. The volumes of cTnT in diabetes-like environment medium on day 6,8 and 10 (0.12 ± 0.03,0.07 ± 0.04 and 0.46 ± 0.11) of culture were significantly lower than the same time of the control group (1.02 ± 0.25,0.25 ± 0.02 and 0.71 ± 0.21) of culture respectively. Western blot assay resulted with the integral optical density (IOD) values of quantitative data. The protein expressions of GATA4 in diabetes-like environment medium on day 6,8 and 10 (0.40 ± 0.06,0.25 ± 0.03 and 0.92 ± 0.13) of culture were significantly lower than the same time of the control group (0.69 ± 0.09,0.75 ± 0.08 and 1.05 ± 0.07). The protein expressions of cTnT in diabetes-like environment medium on day 6 and 8(0.39 ± 0.08,0.37 ± 0.05) of culture were significantly lower than the same time of the control group (0.79 ± 0.05,0.54 ± 0.07). There were significant differences between the day 6 and 8 groups(P < 0.05). Conclusion:P19 cell induced decreasingly to myocardial cell differentiation in diabetes-like environment culture.

      Abstract:Objective:To evaluate the differential expression of Akt signaling pathway in the genital tubercle(GT) of hypospadiac Spraque-Dawley rats induced by maternal exposure to dibutyl phthalate (DBP) and the control rats in order to further explore the mechanism of hypospadias. Methods: Twenty pregnant rats were randomly divided into two groups and administered with DBP at a dosage of 800 mg/kg or soybean oil intragastrically from gestation day (GD)14 to GD18. On GD19,the expression of Akt signaling pathway was analyzed by Western blot and immunohistochemistry. Results:The expression of phosphorylated Akt (p-Akt),Akt and NF-κB had statistical difference between hypospadiac fetal rats and control (P < 0.05). The p-Akt was mainly located in the cytoplasm of epithelium; the staining intensity in the hypospadiac fetal rats was stronger than that in the control rats. Conclusion:Maternal exposure to DBP could increase the expression of Akt signaling pathway in GT epithelium,affect cellular proliferation,apoptosis and urethral fold fusion,and then result in the occurrence of hypospadias.

      Abstract:Objective:To evaluate the neuroprotective effect of borneol and its analogues on glutamic acid induced neuron damage. Methods:The lactate dehydrogenase (LDH) was leaked to extracellular space after the neurons were treated with glutamic acid. The neuroprotective effect of the test drugs was evaluated by the enzyme activity of the extracellular LDH. Results:Borneol(1),bornylfromic acid (3),2-bornylethanol (4),2-bornylacetic acid (5),2-carboxymethylisoborneol (6),2,6-diisopropylbenzonic acid(8),2-(2’,6’-diiso-propylphenyl)ethanol (9) and 2-(2’,6’-diisopropylphenyl)acetic acid (10) showed neuroprotective effect,while bornylamine (2) and menthylformic acid (7) exhibited neuron injury. Conclusion:Borneol and most of its analogues showed neuron protective effect in glutamic acid induced LDH releasing model,among which,2,6-diisopropylbenzonic acid (8) showed good neuroprotective effect in the concen-tration of 1 × 10-5~1 × 10-7 mol/L. This deserves further research.

      Abstract:Objective:To investigate the expression and significance of PTEN and P53 in non-muscle-invasive bladder cancer. Methods:The expression of PTEN and P53 in 121 cases of non-muscle-invasive bladder cancer tissues were determined by the immunochemical SP methods. We combined the clinical follow-up data to analye the relationship between these two markers with tumor stage, grade and recurrence. Results:The expressions of PTEN and P53 protein were detected in 89.2% and 39.6% of bladder cancer specimens, respectively. The positive expression rate of PTEN was negatively correlated with the pathological grades and clinical stages of bladder cancer. However, P53 was positively correlated with the pathological grades and clinical stages of bladder cancer. P53 was significantly correlation with the postoperative recurrence. No significant correlation between PTEN in non-muscle-invasive bladder cancer. Conclusion:The detection of the expression of PTEN and P53 in non-muscle-invasive bladder cancer is helpful to estimate the biological behavior and postoperative recurrence.

      Abstract:Objective:To investigate the sensitivity and specificity of a new insulin resistance index derived from triglycerides and blood glucose through the Botnia clamp and other simple index in clinical application. Methods:A total of 43 subjects were included in the present study. All the patients underwent Botnia clamp. Lipid profiles and other biochemical markers were measured from the fasting blood sample. Correlation coefficient between insulin resistance index and glucose metabolic rate(M value) were studied. Results:The Pearsons’ linear correlation coefficient between TyG and M value (r = -0.536,P < 0.001),HOMA1-IR(r = 0.464,P < 0.01),HOMA2-IR (r = 0.402,P < 0.01),QUICK index(r = -0.448,P < 0.01) and ISI index(r = -0.464,P < 0.01) were statistically significant. The area under ROC curve are TyG(0.879)> HOMA1-IR(0.762)> HOMA2-IR(0.738). The best value of TyG index for the diagnosis of insulin resistance was 4.76,which showed the highest sensitivity(96.4%) and specificity(66.7%). Conclusion:The TyG index is a better index in evaluating the insulin sensitivity in type 2 diabetes. It is accessible and has higher sensitivity and specificity, suggesting that it could be widely used in epidemiological studies or some large-scale clinical trials.

    • The correlation between retinol binding protein 4 and nonalcoholic fatty liver disease in newly-diagnosed type 2 diabetes mellitus

      Abstract:Objective:To investigate the metabolic characteristics of newly-diagnosed type 2 diabetes with nonalcoholic fatty liver disease(NAFLD) and the correlation between retinol binding protein 4 in newly-diagnosed type 2 diabetes with nonalcoholic fatty liver disease. Methods:A total of 216 patients with newly-diagnosed type 2 diabetes were enrolled in this study. They were divided into two groups according to with or without NAFLD. We compared the blood pressure,body mass index (BMI),blood lipids [TC,TG,HDL, LDL, Lp (a)], fasting glucose, fasting insulin, HbA1c,uric acid, liver function and retinol binding protein 4(RBP4). Logistic regression and Pearson correlation were performed to analyze the related factors. Results:The levels of BMI, systolic blood pressure, diastolic blood pressure were much higher in T2DM with NAFLD group than T2DM without NAFLD group. After adjustment of age,sex and BMI, the levels of triglyceride, total cholesterol, fasting insulin, insulin resistance index (HOMA-IR),ALT,γ-GGT, uric acid,RBP4 were significant higher in newly-diagnosed type 2 diabetes with NAFLD disease (P < 0.05). In logistic regression analysis,BMI,ALT and RBP4 were significant and independent predictors of newly-diagnosed type 2 diabetes with NAFLD(P < 0.05). RBP4 was significantly correlated with HOMA-IR,HbA1c,FINS,γ-GGT,UA (P < 0.05). Conclusion: The results suggest that newly-diagnosed type 2 diabetes with NAFLD displayed with the components of metabolic syndrome RBP4 may be an important indicator of nonalcoholic fatty liver disease in newly-diagnosed type 2 diabetes.

      Abstract:Objective:To compare clinical and pathologic manifestation between patients with idiopathic IgA nephropathy (IgAN) accompanied by metabolic syndrome (IgAN-MS) and IgAN-non-MS and to investigate the impact of MS on IgAN. Methods:A total of 453 patients with renal biopsy-proven idiopathic IgAN from Jan. 2006 to Jun. 2012 in Wuxi People’s Hospital were enrolled in this study. Then 87 patients were assigned to the IgAN-MS group and other 366 patients were assigned to the IgAN-non-MS group. A comparative analysis of clinical and pathological data between these two groups was performed. Results:Urine protein, serum creatinine in the IgAN-MS group were significantly higher than those in the IgAN-non-MS group(P < 0.05). Pathological changes in the IgAN-MS group were significantly more severe than those in the IgAN-non-MS group(P < 0.05). eGFR was negativly correlated with urine protein of 24 hours, mean arterial pressure,age, body mass index, and serum triglyceride, while urine protein of 24 hours was positive correlated with fasting blood-glucose, body mass index, serum triglyceride and albumin,LDL-C and urine acid by Pearson correlation analysis. There were positive relationship between MS and Lee’s classification(or damage index in Oxford classification). Conclusion:MS may be an important risk factor for IgAN progression. The factors of MS could aggravate renal damage.

      Abstract:Objective:To investigate the changes of serum nitric oxide (NO) and endothelin-1 (ET-1) concentrations and their relationship with umbilical arteries blood flow velocities in gestational diabetes mellitus(GDM) women. Methods:A total of 45 pregnant women with GDM were collected as the GDM group,and 43 normal pregnant(NP) women were selected as the control group. They were awaiting delivery during January to October in 2012. All of them underwent antepartum glucose tolerance test in their prenatal examination. Ultrasonic testing of cord blood flow parameters(S/D) were measured for the GDM group and control group at one week before delivery. Venous blood samples were collected from all the subjects when they were awaiting delivery in hospital. Venous blood NO and ET-1 levels were detected by nitrate reductase and ELISA,respectively. Results:Compared with the control group,venous blood NO levels decreased and ET-1 levels and S/D increased significantly (P < 0.05) in the GDM group (P < 0.05). NO level was negative (r = -0.596,P < 0.05) and ET-1 level was positive (r = 0.610,P < 0.05) related with S/D value in the GDM group. The incidence of adverse pregnancy in the GDM group was higher than the control group. Conclusion:The decrease of NO and the increase of ET-1 content lead to the increase of blood flow resistance of the fetal-placental,which may be an important factor leading to adverse pregnancy in patients with GDM.

      Abstract:Objective:For guiding the clinical therapy,we sought to analyze the changing of department distribution and the resistance of Acinetobacter baumannii from the First Affiliated Hospital of NJMU during 2008 to 2011 Methods:The bacteria were identified by VITEK2 identification system or API bacterial identification strip. Antimicrobial susceptibility testing were carried out by the disc diffusion method (Kirby-Bauer method). Data analyses were performed by Whonet 5.6 software. Results:Acinetobacter baumannii ranked the highest proportion (80.0%) in sputum samples. The bacteria was detected in some clinical departments,including geriatrics,ICU,emergency department,respiratory and neurosurgery. The resistance rates to cefoperazone/sulbactam,imipenem and meropenem among Acinetobacter baumannii in 2008 were 12.0%,14.8% and 23.3%,respectively;while the resistance rates in 2011 increased to 67.4%,90.8% and 91.1%,respectively. Conclusion:We found that the resistance rates of many clinical used antimicrobial agents significantly increased in Acinetobacter baumannii isolates. We should pay attention to the antimicrobial resistance monitoring and control its spread.

      Abstract:Objective:To analyze the prevalence and predictors of syphilis and herpes simplex type 2 virus (HSV-2) infections among HIV-infected men who have sex with men(MSM) in Jiangsu province. Methods:MSM were convened with Cluster sampling and investigated with follow-up questionnaire survey. We reviewed the case reports of their questionnaire information in Nanjing and Zhenjiang city from March to June,2012. Blood samples were collected to detect syphilis and HSV-2 infections. Results:Total of 264 HIV-infected MSM enrolled in the interview and blood sampling. The prevalence of syphilis and HSV-2 infections was 13.64% (36/264) and 28.03% (74/264),respectively. Multivariate logistic regression analysis showed that active syphilis infection was associated with having more than 3 sex partners in the latest 6 months and HSV-2 infection. The HSV-2 infection was associated with age > 35 years old,locality census register,recent syphilis infection and accepting condom issue service. Conclusion:The HIV-infected MSM in Jiangsu province had high infection rates of syphilis and HSV-2. It is necessary to further strengthen and implement and explore new intervention and testing methods.

    • Use of cone-beam computed tomography to evaluate root and canal morphology of mandibular first molars in Chinese individuals

      Abstract:Objective:To evaluate the root and canal morphology of mandibular first molars in Chinese individuals using cone-beam computed tomography(CBCT). Methods:A total of 766 patients of Chinese descent with healthy,well-developed mandibular first molars were included. These patients went to the Stomatological Hospital Affiliated of Nanjing Medical University and underwent radiographic examination by CBCT as part of their routine examination,diagnosis and treatment planning. The root canal configurations were classified according to the method of Vertucci. Results:CBCT image data of 1,395 mandibular first molars from 766 patients (137 patients without unilateral mandibular first molars) were obtained. The prevalence of mandibular first molars with two separate roots was 68.5%,however,three roots were identified in 31.4% of mandibular first molars. Three canals were found in 57.9% of mandibular first molars and 38.5% had four canals. 93.1% mesial roots contained two canals,most distal roots had a simple type I configuration,whereas mesial roots had more complex canal systems. The incidence of permanent three-rooted mandibular first molars was 35.3%,the incidence had no significantly difference between male and female (P > 0.05). Of all the patients with 3-rooted mandibular first permanent molars,60% occurred bilaterally. There was a significantly higher incidence of three-rooted mandibular first permanent molars on the right side than on the left (P > 0.05). Conclusion:Three-rooted mandibular first molars occurred frequently in this Chinese population. CBCT is an effective tool for the detection of additional distolingual roots and it is a valuable aid for dentists providing root canal treatment.

    • Establishing a magnetic resonance imaging method to evaluate rat muscle tissue necrosis model

      Abstract:Objective:To investigate an experimental method of using magnetic resonance imaging (MRI) techniques to evaluate rat muscle tissue necrosis model. Methods:Firstly,0.2 ml anhydrous ethanol was injected into the left hind limbs of 6 rats to make a skeletal muscle necrosis model. MRI scans were performed after 0,2,4,8,12,14 h and 16 h respectively to analyze imagines and to find optimal scanning sequence and scanning time. Results:MRI scanning resulted from T2WI fs and 12 h showed clear pathological changing area and excluded the signal interference from peripheral fat,which proved T2WI fs is the optimized scanning sequence and 12 h is the optimized scanning time. Another 20 model rats were used for MRI scans with the optimal scanning sequence and time,then executed after MRI scans for pathological examination. There was no apparent size difference between Evans blue dye pathology images and MRI images of rat skeletal muscle necrosis area. The skeletal muscle necrosis area was (7.95 ± 0.18)% of the viewing area of the left hind in MRI images,and was (7.91 ± 0.15)% in the Evans blue dye pathology images. Conclusion:MRI scans can be used for evaluating skeletal muscle necrosis.
