    • Appearence of autophagy and changes of mitochondria number in the process of 3T3-L1 preadipocyte differentiation

      Abstract:Objective:To explore the relationship between the changes of autophagy and the number of mitochondria in the process of the differentiation of 3T3-L1 cells. Methods:(1)3T3-L1 preadipocytes were cultured and then induced by media mixed with insulin,DEX,IBMX. Lipid droplets were observed on the fifth day. The peroxisome proliferation activated receptor γ(PPARγ)and CAAT Enhancer Binding Proteins α(CEBPα)protein were detected by Western blotting. (2)Autophagosome pictures were taken by transmission electron microscope (TEM)and LC3II level was detected by Western blotting. Results:The TEM showed that during the differentiation of 3T3-L1 preadipocyte,autophagy vesicles appeared on the second day,extended to greater range of cytoplasm on the fourth day,then gradually disappeared from the sixth day. While the expression of LC3Ⅱ began to increase in the second day, reached the highest on the fourth day,and then gradually declined. Furthermore,with the change of autophagy vesicles during the differentiation,the number of mitochondria increased after a decrease. Conclusion:There are a lot of autophagy vesicles in the early and middle stage of the differentiation of 3T3-L1,including some mitophagy,which may be one of the reasons for the decrease of the number of mitochondria in early stage of differentiation.

    • Active vitamin D deficiency resulting in brain vasopressin increment and cerebrovascular dysfunction

      Abstract:Objective:The aim of this study was to evaluate the influence of 1,25-dihydroxy vitamin D3[1,25(OH)2D3]deficiency on brain vasopressin expression and cerebrovascular function in hypertension. Methods:Mean arterial pressure,brain vasopressin(VP)expression and cerebrovascular reactivity of two-month-old 1-alpha-hydroxylase[1α(OH)ase]gene knockout mice and their wild-type littermates were analyzed and compared using tail-cuff plethysmography,immunohistochemistry and online videomicroscopy. Results:Compared with their wild-type littermates,1α (OH)ase-/- mice showed high mean arterial pressure,increased VP positive neurons in hypothalamus and cerebral arteries featured markedly decreased dilatory responses to acetylcholine (ACh)and a reduced basal production of NO as exemplified by the attenuated diameter decrease during NOS inhibition with L-NNA. Conclusion:l,25(OH)2D3 plays an important role in neuroendocrine regulation and cerebrovascular function in hypertensive progression.

    • Three-dimensional culture of mouse pancreatic islets and liver-derived perfusion-decellularized bioscaffold for potential clinical practice

      Abstract:Objective:To observe the growth and function of primary mouse islets in the liver decellularized bioscaffold(LDB),and explore the new method of tissue engineering for treatment of diabetes. Methods:We perfused the whole mouse liver using detergent through hepatic portal vein in a continuous way and manufactured the whole LDB with the complete structure. The collagenase P was perfused into the mouse common bile duct,and the number of the isolated isles was counted and the integrity of the islets’ structure and function were analyzed. The isolated primary mouse islets were transplanted into the LDB and cultured for 5 days in the three-dimensional culture system. Then we appled HE staining,insulin immunohistochemical analysis and fluorescence quantitative polymerase chain reaction(PCR). Results:The LDB maintained their original round shape after decellularization and demonstrated a complete lack of nuclear staining and blood vessel. Collagenase 1 fluorescent staining showed that the collagen structure intact. The DNA quantification showed(38±11)ng/mg dsDNA in dry weight of ECM. In vivo biocompatibility test of the LDB showed that there didn’t exist any pathological presentation. The result of dithizone(DTZ)staining showed that the staining islets had the scarlet color Glucose stimulation test showed that high quality islets results in a greater insulin stimulation at high glucose concentration than that observed at low glucose concentration (P < 0.01). Insulin immunohistochemistry examination showed that the LDB parenchyma in transplanted area had brown stain. Insulin gene expression displayed the expression level of insulin in LDB was two times greater than the level of plate culture with statistical significance(P < 0.01). Conclusion:In LDB 3D culture system,islets showed better cell viability and more effective insulin secretion function than traditional plate culture.

    • Optimized polymer microbubbles for ultrasound imaging in vitro/vivo

      Abstract:Objective:This study focuses on the optimization on stability and size of MBs with poly-L-lactic acid(poly L-lactic acid,PLLA)and polyvinyl alcohol (polyvinyl alcohol,PVA)as shell materials,which made MBs more suitable to work in vivo at the same time keeping the ability of enhancing the ultrasound imaging. Methods:First,PVA was modified to obtain telechelic PVA with carboxylic groups at the chain ends. The modified PVA was used to fabricate MBs present carboxyl on their surface,whose Zeta potential was -38.87 ± 2.19 mV,thus improving the stability of MBs. Next,The particle size and stability of MBs were regulated through adjustments the three factors,i.e. the concentration of PVA aqueous solution,the water-oil ratio of the first emulsification and the rate of second emulsification during the preparation process. Results:In vitro/vivo US imaging experiment was performed on the optimized MBs,which had a average particle size of 1.38 -滋m,uniform particle size distribution and good stability. In vitro experiments showed that the MBs exhibited significantly ultrasound(US)contrast enhancement. In vivo US imaging of tumors in nude mice showed that,by intratumoral injection,MBs can rapidly improve the ultrasound imaging of tumor;by intravenous injection,MBs can reach the tumor site through the blood circulation,accumulate in the tumor site after a period of time,and enhance ultrasound imaging. Conclusion:This study lays a good foundation for the application of MBs in vivo.

    • YM155 enhances apoptosis in triple negative breast cancer MDA-MB-231 cells via autophagy

      Abstract:Objective:To investigate the effect of YM155,a survivin inhibitor,on the apoptosis and autophagy of the triple negative breast cancer MDA-MB-231 cells. Methods:MDA-MB-231 cells was treated with different concentrations of YM155,the survival rate of the cells was determined by CCK-8 assay and the IC50 (half inhibitory concentration)value of YM155 was calculated. The apoptosis rate was examined by Annexin V-FITC/PI double staining. The mRNA expression of survivin,beclin 1 and bcl-2 in MDA-MB-231 cells was detected by Real-time PCR. The protein expression of survivin,bcl-2,caspase 3,PARP,beclin 1 and LC-3 were detected by Western blot. Results:It was revealed that YM155 significantly inhibited the growth of MDA-MB-231 cells in a dose-and time-dependent model. The apoptosis rates of cells treated with 0.5,1.0,1.5 ng/mL YM155 were (11.9 ± 2.4)%,(21.7 ± 2.6)%,and (30.8 ± 4.5)%,respectively,which all had significant difference compared to control cells[(6.4 ± 1.2)%]. When a combination of 3-MA effect after 24 h,cell proliferation rate was significantly enhanced compared to that of the single YM155 group (P < 0.05).With the increasing of YM155 concentration,the expression levels of mRNA and protein of survivin and BCL-2 were decreased,while the expression levels of caspase-3,PARP,beclin1 and LC-3 were increased. Compared with the YM155 group,the protein levels of LC-3 and caspase-3 were lower in YM155 + 3-MA group. Conclusion:YM155 could effectively inhibit MDA-MB-231 cells proliferation by inducing apoptosis and autophagy,while autophagy induction effect can enhance its apoptosis effect.

      Abstract:Objective:To investigate the effects of miRNA145 combined with metformin on the proliferation of human HCT116 cancer cells. Methods:miRNA145 overexpression mediated by recombinant lentivirus was exployed in HCT116 cells. Cell proliferation assay by CCK-8 method was conducted to determine the roles of miRNA145 and/or metformin on the cancer cells. Western blot analysis was used to determine the expression levels of AMPK and mTOR proteins. Results:Both miRNA 145 and metformin can inhibit the cell proliferation of HCT116. The combination of miRNA 145 and metformin exerted a much stronger inhibitory effect on cell proliferation than any of the two. The activation level of AMPK was also higher by the combination,and the p-mTOR expression level was lower by the combination. Conclusion:miRNA145 and metformin can exert an anticancer effect synergistically.

      Abstract:Objective:To investigate the expression of FGFR4 mRNA in lung adenocarcinoma tissues and its association with prognosis. Methods:The quantitative expression of FGFR4 mRNA in 126 lung adenocarcinoma tissues and normal lung tissues in south China area were detected by real-time fluorescence quantitative-PCR,and the relationship between the quantitative expression of FGFR4 mRNA in lung adenocarcinoma and the prognosis were analyzed. Results:The expression of FGF4 mRNA in lung adenocarcinoma was significantly higher than that in normal lung tissues(P < 0.0001). The survival time of patients with lung adenocarcinoma with higher expression of FGFR4 mRNA was significantly longer than those with lower expression level of FGFR4 mRNA (P < 0.05). Conclusion:FGFR4 mRNA expression in lung adenocarcinoma of south China area was significantly down-regulated than that in tumor-adjacent normal lung tissue. Postoperative survival time of patients with lung adenocarcinoma in high FGFR4 mRNA expression was significantly longer than that with low expression.

      Abstract:Objective:To evaluate left ventricular myocardial systolic dyssynchrony in the patients with hypertrophic cardiomyopathy(HCM)by ultrasound speckle tracking imaging. Methods:Myocardial imaging of 35 HCM patients (HCM group)and 30 healthy subjects(control group)was collected,and their regional time to peak systolic strain in longitudinal,radial,circumferential motion were measured respectively. The data was compared and analyzed between the two groups. Results: In control group,no significant difference was found in time to peak longitudinal,radial and circumferential strain. Compared to control group,time to peak longitudinal strain(Tl)was significantly delayed,not only in the hypertrophic segments,but also in the segments with normal thickness. Time to peak radial strain (Tr)and time to peak circumferential strain (Tc)was also delayed,but more severe delay was found in longitudinal direction. Conclusion:Ultrasound speckle tracking imaging can quantitatively evaluate the left ventricular myocardial systolic dyssynchrony.

      Abstract:Objective:To investigate the correlation between preoperative plasma N-terminal pro-brain natriuretic peptide(NT-proBNP) and prognosis of cardiac resynchronization therapy as well as left ventricular function in patients with chronic heart failure.Methods:From March. 2012 to October.2014,60 patients who received CRT implantation in the first affiliated hospital,Nanjing Medical University were included. All the patients had the routine test contained NT-proBNP. Left ventricular ejection fraction(LVEF)were measured by echocardiography. The patients were divided into responders and non-responders according to whether the left ventricular ejection fraction (LVEF) increased ≥5% by 6 months. Major adverse cardiac events (MACE) was observed during follow-up. Results:Levels of NT-proBNP and MACE rates were much higher in non-responders than that in responders (P < 0.01). At the optimum cutoff point of 2 354.5 pg/mL,the sensitivity and specificity of NT-proBNP predicting CRT non-response were 95%and 92.5%,respectively.At the optimum cutoff point of 2 254.5 pg/mL,the sensitivity and specificity of NT-proBNP predicting MACE were 95.2% and 92.3%,respectively. Kaplan Meier survival curve showed the survival time in patients with BNP 2 254.5 pg/mL or less were higher than that in patients with BNP 2 254.5 pg/mL or more(P < 0.01). Conclusion:Preoperative NT-proBNP is associated with response to CRT and postoperative MACE in CHF patients.

    • Application of Bethesda system for reporting cytology in thyroid fine needle aspiration

      Abstract:Objective:Application of the Bethesda system (TBS)for reporting thyroid cytology in fine needle aspiration(FNA)to improve the capability of cytopathological diagnosis. Methods:Totally 1 705 FNA cytopathological samples were analyzed from May 2011 to Jun 2012. Sixty-six histological pathological samples from these patients were analyzed in the meantime. Results:According to TBS,1 705 FNA cytopathological samples were classified into six categories with 295(17.30%)as nondiagnostic(N),1 291(75.72%)as benign(B),30(1.76%)as atypia of undetermined significance(AUS),19(1.11%)as follicular neoplasm(FN),36(2.11%)as suspicious(S)and 34(1.99%)as malignant(M). Total positive cell rate was 82.70%. It was concluded from the comparison of histopathological and cytopathological slides of 66 patients that the overall occurrence rate of malignancy was 11.11% in former two groups,which was significantly lower than that (63.33%) in latter four groups (P < 0.01). The sensitivity,specificity,positive predictive value,negative predictive value,false positive rate and false negative rate for FNA in differentiation of benign and malignant lesions were calculated as 82.61%,74.42%,63.33%,88.89%,25.58% and 17.39%, respectively. Conclusion:TBS showed great importance in differentiation of benign and malignant thyroid lesions,with high probability of malignancy in AUS,FN,S and M groups.

    • Analysis of fasting hyperglycemia and its related factors in hospitalized patients with diabetes——a multicenter clinical study

      Abstract:Objective:To investigate the status and related factors of fasting hyperglycemia in hospitalized patients with diabetes. Methods:Total 1055 diabetic patients who underwent continuous glucose monitoring from Jan.2013 to Dec. 2014 in five hospitals were enrolled in this study. Sex,age,type of diabetes,duration,body mass index(BMI),blood pressure,glycosylated hemoglobin (HbA1c),blood lipid,urinary albumin,macroangiopathy and so on were collected to analyze the risk factors. Results:Totally 83.9% of diabetic patients had fasting hyperglycemia,of which 75.2% was poor glucose control throughout the day,15.4% was dawn phenomenon and 9.4% was Somogyi effect. The incidence of Somogyi effect in type 1 diabetes patients was significantly higher than that in type 2,and the incidence of poor glucose control throughout the day in type 2 diabetes patients was significantly higher than that of type 1. The one-way ANOVA analysis results showed that there were significantly differences of age,type,duration,blood pressure,blood lipid between dawn phenomenon group,Somogyi effect group,and poor glucose control throughout the day group (P < 0.05). The binary logistic regression analysis results showed that older,longer duration,higher TG,and higher LDL-C were more likely to occur in poor glucose control throughout the day (P < 0.05). Patients with younger age,shorter duration,higher systolic blood pressure or higher TC were more likely to have dawn phenomenon (P < 0.05). Lower TC and TG were more prone to Somogyi effect (P < 0.05). Conclusion:Fasting hyperglycemia was a common phenomenon among hospitalized patients with diabetes. The causes in sequence were poor glucose control,dawn phenomenon and Somogyi effect. Type 1 diabetes patients were more likely to have Somogyi effect. Age,duration,TC,and LDL-C were risk factors for poor glucose control throughout the day; Age,duration,systolic blood pressure and TC were risk factors for dawn phenomenon; TC,TG were risk factors for Somogyi effect.

    • A clinical analysis of vitamin D status in patients before spinal surgery

      Abstract:Objective:To study preoperative concentrations of vitamin D in patients undergoing spinal surgery and analyze relevant risk factors associated with vitamin D deficiency. Methods:A retrospective analysis was conducted on 503 patients undergoing spinal surgery. Preoperative concentrations of serum 25-hydroxyvitamin D[25(OH)D]were measured. Normal vitamin D was defined as a serum 25(OH)D level ≥ 32 ng/mL,vitamin D insufficiency as 25(OH)D ≥ 20 and <32 ng/mL,and vitamin D deficiency as 25(OH)D < 20 ng/mL. Results:The rate of vitamin D deficiency and insufficiency was 71.4% and 23.9%,respectively. Rate of vitamin D deficiency in female patients was significantly higher than that in male patients (P < 0.001). Rate of vitamin D deficiency in patients with high body mass index (P = 0.009)or patients with smoking habit (P < 0.001)was much higher compared with that in the other patients. An analysis of monthly vitamin D levels revealed that rate of vitamin D deficiency was the lowest in September(44.8%),and the highest in January(90.6%). The analysis of quarterly vitamin D levels showed that rate of vitamin D deficiency was the lowest in winter and the highest in fall (P = 0.001). No correlation between vitamin D status with age or bone mineral density was identified. Multivariate analysis demonstrated that female(P < 0.001),smoking(P = 0.005),and winter(P = 0.001)were potential risk factors for vitamin D deficiency. Conclusion:Severe preoperative vitamin D insufficiency and deficiency existed in spinal surgery patients,and vitamin D insufficiency or deficiency may largely affect surgical outcomes of patients. Therefore,screening of preoperative vitamin D status and relevant treatment should be reinforced in spine surgery patients.

    • Plasma BRCA1 mRNA expression and clinicopathology in patients with gastric cancer

      Abstract:Objective:To investigate the correlation of the plasma mRNA levels of the breast cancer susceptibility gene1(BRCA1)with clinicopathological parameters in patients with gastric cancer. Methods:The plasma BRCA1 mRNA was measured using quantitative reverse transcription polymerase chain reaction,and the relationship between BRCA1 level and clinicopathological parameters as well as survival was studied. Results:BRCA1 expression was not related to age,gender,TNM stage,tumor location,Lauren’s classification,vessel invasion,seros invasion,nerve invasion,differentation,surgical peocedures and sample sourse. However,BRCA1 level was associated with tumor size (P = 0.033)and overall survival of patients(P = 0.034). Conclusion:The plasma BRCA1 mRNA level could reflect certain information of gastric cancer including the tumor size and predict the overall survival.

    • Analysis of bone marrow biopsy pathology in chronic myelomonocytic leukemia

      Abstract:Objective:To analysis the cell morphological characteristics in chronic myelomonocytic leukemia (CMML),and investigate the expression of immune marker in the diagnose and clinical application value of CMML. Methods:Using the bone marrow biospy pathology to analysis the pathology characteristics in 39 cases of CMML. By using the immunohistochemistry to detect the expression of antibodies which included CD15,MPO,CD34,CD14,CD56,CD68 (PG-M1)and CD163 in CMML,comparison with chronic myelocytic leukemia (CML) and the acute monocytic leukemia (AMoL) patients. Results:Hypercellularity was the markedly feature of the bone marrow histology in 33 cases (84.6%)of CMML. Increased granulocytic proliferation and obviously morphological abnormalities were observed in these cases. Monocytic cells were increased in 29 cases (74.4%)and 20 cases(51.3%)presented megakaryocytic morphological abnormalities. The marker in the granulocytic lineage expresses CD15 and MPO,which had respectively positive percentage of 21.69 ± 7.06% and 33.96 ± 5.94%,there were significantly statistical differences compared with CML and AMoL. The marker in the monocytes expresses CD14-CD56-CD68(PG-M1)and CD163,which were positive (10.10 ± 2.51)%-(8.69 ± 2.66)%-(13.35 ± 4.32)%-(11.43 ± 4.15)%,respectively. Except from CD56 positive CMML and AMoL,the rest had significantly statistical differences. The original marker which was positive expressed in CMML such as CD34 had the percentage of (4.31 ± 1.98)%,which had significantly statistical differences with CML. Conclusion:Morphological and immunohistochemical features of bone marrow biopsy are helpful in the diagnosis and differentiation.

    • The clinical observation of transcatheter arterial chemo-embolization (TACE) combined with sorafenib compare with TACE alone in the treatment of primary hepatocellular carcinoma

      Abstract:Objective:To evaluate the clinical efficacy of transcatheter arterial chemo-embolization (TACE)combined with sorafenib on the treatment of primary advanced hepatocellular carcinoma. Methods:Totally 115 patients with primary hepatocellular carcinoma (TNM stage)were randomly divided into two groups:58 cases in treatment group received TACE + sorafenib treatment ;57 cases in control group received TACE therapy. The short-term effect,long-term survival,and adverse reactions were analyzed between two groups. Results:Two months after treatment,the disease control rate in the treatment group and the control group was 74.1 % vs 64.9%,there was no statistically significant difference betweer two groups(P = 0.282). The median survival time in the treatment group was 13.2 months,and that of the control group was 9.6 months,and there was a significant difference between two groups(P < 0.001). One-year survival rate of the treatment group and the control group was 62.5 % and 32.1 %,respectively,and there was a significant difference between two groups(P = 0.001). Adverse events in two groups were mainly gastrointestinal reactions,bone marrow suppression,mucocutaneous reactions and liver abnormalities. The incidence of oral mucositis and hand-foot skin reaction in the treatment group was significantly higher than that of control group,and there were significant difference between two groups(P < 0.001,P < 0.001). Conclusion:TACE combined with sorafenib in patients with advanced hepatocellular carcinoma can improve patients’ 1-year survival rate with the mild side effects.

    • Clinical application of extracorporeal membrane oxygenation support for low cardiac output syndrom after heart surgery

      Abstract:Objective:To retrospectively summarize the clinic experiene of extracorporeal membrane oxygenation support for low cardiac output syndrom after heart surgery. Methods: From April 2010 to April 2014,23 patients received ECMO support,for the low cardiac output syndrom after heart surgery,were enrolled. The data were retrospectively obtained from our hospital database. Results:Fifteen patients(65.2% )weaned off successfully from ECMO, 10 patients (survival rate 43.5% )were discharged and 5 patients died of post-complications. Eight patients could not wean off from ECMO. Over all mean support time was 191 ± 121 hours. Bleeding,renal failure,lung infection and neurological complication were the most reported complications. The lactate levels before ECM0 support and the amount of blood loss in survivors and nonsurvivors had a significant difference(P < 0.05). Conclusion: ECMO is an effective mechanical assistant therapy method for severe cardiac and pulmonary failure after heart surgery.Earlier usage of ECMO and avoiding massive bleeding are the key point of success of ECMO.

    • Incidence and risk factors of cardiac surgery-associated acute kidney injury

      Abstract:Objective:To investigate the incidence and related risk factors of cardiac surgery-associated acute kidney injury(CAS-AKI). Methods:A retrospective analysis of 1 575 patients undergoing cardiac surgery with cardiopulmonary bypass(CPB)between January 2009 and December 2011 in Nanjing First Hospital,was recruited. Univariate and multiple logistic regression models were employed for determining the association between the development of CSA-AKI and risk factors. Multiple Cox-proportional hazards modeling was used to evaluate the impact of CSA-AKI on the mortality of ICU and hospital length of stay. Results:Among this cohort,of 1 575 patients,534 (34%)occurred AKI. A total of 22 (1.4%)patients required renal replacement therapy. The overall in-hospital mortality rate was 1.3%(20 of 1575). Logistic regression analysis showed that mechanical ventilation duration (P < 0.001)CPB duration of ≥100 min(P < 0.01)erythrocytes transfusion (P < 0.001)and postoperative body temperature greater than 38℃ within 3 days (P < 0.05)were found to be independent risk factors of CSA-AKI,while ulinastatin use was associated with lower incidence for CSA-AKI (P < 0.05). CSA-AKI was significantly related to high in-hospital mortality (P < 0.05)especially in patients requiring RRT (P < 0.001). Conclusion:The incidence of CSA-AKI in patients form this cohort was 34%. Mechanical ventilation duration,erythrocytes transfusion and postoperative body temperature greater than 38°C within 3 days were independent risk factors of CSA-AKI. Ulinastatin was associated with lower incidence of CSA-AKI.

      Abstract:Objective:Use color doppler ultrasonography to detect the blood flow alterations of renal artery in hypertensive disorders complicating pregnancy women for the purpose of early diagnosis of renal impairment. Methods:The research included 156 pregnant women chosen in Maternal-Child Health Centers affiliated to Nanjing Medical University. These women were classified into five groups:preeclampsia(PE,n=40),chronic hypertension complicating preeclampsia(CHCPE,n=25),chronic hypertension complicating pregnancy(CHCP,n=21),gestational hypertension(GH,n=22),normal pregnancy(NP,n=48). In 32 to 36 gestational weeks,each woman’s blood serum creatinine(Scr),urea nitrogen(BUN),uric acid(UA),glomerular filtration rate(GFR)were tested and compared. At the same time,we use color doppler ultrasonography to detect the blood flow alterations of renal artery in these women. Results:①In the PE and CHCPE group,the BUN and UA of the participants were significantly higher than that of the NP and CHCP groups. Compared with CHCP,GH and NP groups,the Scr of the participants in PE groups were significantly increased,while the GFR was markedly decreased(P < 0.05). ②The acceleration times of main renal artery(MRA)and segmental artery(SRA)in PE,CHCPE and CHCP groups was remarkably higher than that in NP group(P < 0.05). The acceleration times of SRA in GH group was significantly higher than that in NP group,while there was no difference of AT in MRA between the two groups. Also no difference of peak systolic velocity(Vs),end diastolic velocity (Vd)and resistance index(RI)in MRA and SRA was found among the five groups(P > 0.05). Conclusion:Renal impairment caused by preeclampsia was more quick and severe than that caused by chronic hypertension. AT of renal artery,checked by color doppler ultrasonography,was a sensitive indicator to detect renal impairment early. Using AT,combined with UA and GFR can provide a reliable basis for evaluation of renal function and formulation of treatment plan.

      Abstract:Objective:To observe the rapid plasma reagin change in pregnant syphilis patients treated with combination of ceftriaxone sodium and bicillin. Methods:Retrospective analysis was performed to 65 cases ivas performed the petients to diagnosis and treatment of pregnant syphilis patients in our department,according to the different treatment methods, the patints were divided into the treatment group of 33 cases,32 cases in the control group. The treatment group was treated with ceftriaxone sodium combined with benzathine penicillin treatment,and the control group received benzathine penicillin therapy,clinical effect and the pregnancy outcome after treatment were compared between two groups. Results:The cases of RPR negative conversion in the treatment group were 26,and the rate of RPR negative conversion was 78.79%,while that of the control group were 17 (53.13%),showing significant differences between two groups (P < 0.05). Patients in the two groups after treatment didn’t give birth to baby with congenital syphilis,stillbirth,low birth weight fetal and neonatal death,and in the control group after treatment 1 cases of premature delivery was reported. No difference showed in pregnancy outcome. Conclusion:Combined usages of cefiriaxone sodium and bicillin for treatment of pregnant syphilis patients with rapid plasma reagin high titer can apparently shorten the time of RPR negative conversion.

      Abstract:Objective:We found a special case in clinic:4 generations in a family were detected neuromuscular disorders. The symptoms included the upper elbows had different degrees of limited straight,lower limbs showed equines deformity,and feet were talipes cavus and were not detected obvious varus and adduction. This research aimed to study genetic mutation and molecular mechanism in crucial gene using whole-genome sequencing. Methods:Blood samples from sick child,father with the similar symptom and healthy mother were used to perform whole-genome sequencing by high-throughput sequencing technique. Bioinformatic analysis was used to filter gene mutations associated with disease. Further PCR validations were performed using blood samples from patients and healthy relatives in the family. Results:A series of gene mutations were obtained based on bioinformatics,including ANXA3 gene mutation(chr4,c.C820T(p.R274*)),HIST1H3A gene mutation (chr 6,c.C205T(p.Q69*)) and MTHFR gene mutation (c.T784C(p.Y262H)). PCR validation using blood samples from the family indicated that ANXA3 may be potential causing gene,and the mutation led to change of three-dimensional structure of ANXA3 protein. Conclusion:These results implicated that the ANXA3 may be related with the occurrence of the disease. This study provides certain reference for further understanding the genetic disease from the molecular levels.

      Abstract:Objective:To study the clinical practice value of oxcarbazepine for the tinnitus treatment. Methods:We have collected 20 patients with varying degrees of tinnitus and given different doses of oxcarbazepine depending on the alleviative degrees of tinnitus,and tinnitus handicap inventory (THI)was used to assess the effect after the treatment of 4 weeks and 8 weeks,respectively.Results:Eighteen patients with tinnitus all got improvement with different levels,the change scores of THI showed statistically difference between before and after the treatment of tinnitus. There were correlations between the change of functional scores and THI scores,as well as the severity scores and THI,however,there was no relations between changes of emotional scores and THI scores. Conclusion:Oxcarbazepine is worthy of clinical practice in the tinnitus treatment.

    • Relationships between dietary patterns and metabolic syndrome and its components among adult residents in Jiangsu Province

      Abstract:Objective:To learn the relationship between metabolic syndrome (MS)and dietary patterns among adult residents in Jiangsu Province,so as to provide basis for the nutritional intervention. Methods:Totally 10 568 adults aged over 18 years old were randomly selected from 10 investigation sites in clusters,their general information,food intake were investigated and medical examination were made. The dietary patterns were sorted with factor analysis. The prevalences under different quartiles of factor scores were compared with Chi-Squared test. The effect of dietary patterns on MS was analyzed with logistic regression. Results:Four dietary patterns were categorized,traditional,animal food dominated,fast food dominated and legume dominated. The traditional pattern was related to the prevalence for overweight/obesity (χ2=15.2,P < 0.05). The legume dominated pattern was related to the prevalence for overweight/obesity,hypertension and MS (overweight/obesity:χ2=30.1,P < 0.01;Hypertension:χ2=18.9,P < 0.01;MS prevalence:χ2=8.3,P < 0.05). After multiple confounding factors were adjusted,it was indicated that traditional dietary pattern was the risk factor for overweight/obesity (OR:1.41),while the legume dominated pattern was the protective factor against overweight/obesity,hypertension and MS (the OR was 0.56,0.79,and 0.91,respectively). Conclusion:Dietary patterns are related to metabolic syndrome and its components. Increase the intake of legumes and control the intake of energy may play important roles in the prevention of metabolic syndrome.
