    • Construction of luciferase reporter plasmids of rat S100A8 promoter and initial identification of SOX7 binding element

      Abstract:Objective:This study aims to construct luciferase reporter plasmids of rat S100 calcium binding protein A8(S100A8) gene promoter and detect their activity in HEK293T cells in response to SRY-box transcription factor 7(SOX7)overexpression,meantime screen the possible binding elements for SOX7. Methods:Rat S100A8 gene full length promoter was amplified by PCR and cloned into the luciferase reporter plasmid(pGL3basic),and named pGL3S100A8FL. The plasmid of pGL3S100A8FL and previously constructed plasmid of pIRES2-SOX7 were co-transfected into HEK293T cells,and then the luciferase activity was detected. Meanwhile,the potential SOX7-binding elements within S100A8 promoter were predicted by JASPAR. Based on the predicted results,four plasmids of truncated S100A8 gene promoter(pGL3S100A8 1~4)were co nstructed. The plasmids of pGL3S100A8FL or pGL3-S100A8 1~4 and pIRES2SOX7 were cotransfected into HEK293T cells respectively. Then,the luciferase activity was detected. Next,S100A8 gene promoter of SOX7-binding element(-86~-57 nt)mutated plasmid was constructed(pGL3-S100A8-M). The HEK-293T cells were transfected with pGL3-S100A8-M and pIRES2-SOX7 plasmid,and the luciferase activity was detected. Results:The plasmids of pGL3S100A8FL and pIRES2SOX7 were co transfected into HEK293T cells,found that the luciferase activity of S100A8 gene promoter was markedly increased in response to SOX7 overexpression. The plasmids of pGL3S100A8FL or pGL3S100A8 1~4 and pIRES2SOX7 were co transfected into HEK293T cells,and the result displayed that the activity of pGL3S100A8-4 was much lower than that in pGL3S100A8-FL and pGL3-S100A8-1~3,indicating that the region of rat S100A8 promoter(-200~+51 nt)might contain a SOX7-binding element(-86~-57 nt). Then the -86~-57 nt mutated plasmid(pGL3S100A8M) or pGL3S100A8FL and pIRES2SOX7 were co transfected into HEK293T cells,and the result revealed that the activity of pGL3S100A8-M was much lower than that of pGL3S100A8FL,indicating that the SOX7 may bind to the element of rat S100A8 gene promoter -86~-57 nt. Conclusion:The rat full length and truncated rat S100A8 promoter luciferase reporter plasmids were constructed successfully,and the possible SOX7 binding element was preliminary determinated.

    • The effect of follicular cytotoxic T cells in the process of immune injury in non⁃obese diabetic mice

      Abstract:Objective:This study aims to explore the characteristics of follicular cytotoxic T cells(TFC)and the role they played in the natural course of autoimmune diabetes in non-obese diabetic(NOD) mice. Methods:The spleen,lymph nodes and thymus lymphocytes of NOD mice in 4 weeks,10 weeks,15 weeks,20 weeks,25 weeks,and hyperglycemia(Hi) groups were separated and stained by flow cytometry antibodies. Through flow cytometry,we observed and compared the distribution of TFC and CXCR5-CD8+T cells in different immune organs,the changes of cell proportion in the natural course of autoimmune diabetes,the expression levels of degranulation markers cluster of differentiation 107a(CD107a) and recombinant human killer cell lectin-like receptor subfamily G,member 1(KLRG1),inhibitory molecules programmed cell death protein 1(PD-1) and T cell immunoglobulin mucin 3(TIM-3),and the ability of releasing interferon γ(IFN-γ)and granzyme B(GzmB). Results:The results of this study illustrated that the expression and distribution of TFC was different in various immune organs of NOD mice. The expression levels in the spleen and lymph nodes were equivalent,but there was almost no expression in the thymus. Before the blood sugar of NOD mice increased,the proportion of TFC cells showed a gradual upward trend along with the increasing age,which decreased after the onset. Compared with CXCR5-CD8+T cells,the proportion of TFC was significantly lower,but the inhibitory molecules PD-1,TIM-3 and the degranulation marker CD107a expressed on TFC were significantly higher. At the same time,TFC has a stronger ability to release GzmB[TFC(3.56±2.1)%;CXCR5-CD8+T cell(0.78±0.39)%,P < 0.05]and IFN-γ[TFC(43.65±12.25)%;CXCR5-CD8+T cell(21.92±3.72)%,P < 0.05]than CXCR5-CD8+ T cells. Conclusion:TFC exists and participates in the natural course of autoimmune diabetes in NOD mice,and promotes immune damage in NOD mice through cytotoxic effect.

      Abstract:Objective:This study aims to explore the effect of 11β-hydroxysteroid dehydrogenase(11β- HSD1) gene specific knockout on intestinal epithelial barrier function in mice. Methods:the 11β-HSD1 knockout model was established in C57BL/6J mice. The wild-type control group and 11β-HSD1 knockout group were fed with high-fat diet(from the 6th week,feed for 8 weeks). The number of villi,crypts,goblet cells,and expression changes of ZO-1 and F4/80 were detected by immunohistochemistry. Results:11β-HSD1 gene knockout could change the villus length and goblet cell number of obese mice fed with high-fat diet,significantly reduce the intestinal epithelial inflammation and change the expression of tight junction protein in obese mice fed with high-fat diet. Conclusion:The changes of intestinal epithelial barrier function induced by high-fat diet are closely related to the effect of 11β-HSD1.

      Abstract:Objective:This study aims to investigate if low intensity pulsed ultrasound(LIPUS)has the inhibition effect of on calcification of rat vascular smooth muscle cells(rVSMC)and rat vascular adventitial fibroblasts(rVAF)induced by hypercalcemia and hyperphosphatemia. Methods:RVSMCs and rVAF were extracted,calcium quantification,alkaline phosphatase (ALP) activation and Alizarin red staining to detect the different intensity of LIPUS on calcification;Western blot and real-time PCR were performd to explore the effect of LIPUS on expression of osteogenic markers. Results:LIPUS treatment significantly inhibited the calcium quantification,ALP activation of rVSMCs and rVAF induced by hypercalcemia and hyperphosphatemia. Meanwhile,the expression of osteogenic markers of rVSMCs and rVAF decreased after 14 mW/cm2 LIPUS treatment. Conclusion:LIPUS treatment could inhibit the expression of osteogenic markers in both rVSMCs and rVAF induced by hypercalcemia and hyperphosphatemia. Therefore,LIPUS has protection effect on rVSMCs and rVAF calcification caused by hypercalcemia and hyperphosphatemia.

      Abstract:Objective:This study aims to investigate the never in mitosis gene a-related kinase 7(NEK7)expression in liver fibrosis and its effect on nucleotide-binding oligomerization domain-like receptors(NLRP3)signal pathway to regulate liver fibrosis. Methods:HE staining,Masson staining and quantitative reverse transcription PCR(qRT-PCR)were used to evaluate those degree of hepatic fibrosis in normals and patients with cirrhosis. The expression of NEK7 in liver tissues of non-cirrhotic and cirrhotic patients was detected by Western blot and qRT-PCR. CD68-positive cells was determined by immunofluorescence tissue staining. To induce hepatic fibrosis,C57BL/6J mice were injected intraperitoneally with carbon tetrachloride(CCL4)twice a week for 8 weeks. C57BL/6J mice were injected through tail vein with macrophage-specific siRNA to knockdown the expression of NEK7 in macrophages. HE staining,Masson staining and sirius red staining were used to observe the changes of liver fibrosis in mice liver tissues. The level of the migration and aggregation of neutrophils and macrophages were monitored by immunohistochemistry tissue staining and immunofluorescence tissue staining. The impacts of NEK7 on NLRP3 signaling pathway were measured by Western blot. Results:In the fibrotic liver tissues,NEK7 expression was significantly higher than that in the normal tissues. Specific blockade of NEK7 in macrophages obviously attenuated liver fibrosis and hepatic inflammation in vivo. Specific blockade of NEK7 in macrophages also inhibited the activation of NLRP3 inflammasome signaling pathway. Conclusion:Inhibition of NEK7 may alleviate macrophage inflammation reaction and liver fibrosis through inhibiting NLRP3 inflammasome signaling pathway.

      Abstract:Objective:This study aims to investigate the effects of bovine serum albumin(BSA)coated Ag2S storage solution on the surface characteristics of pure titanium and the behaviors of osteoblasts. Methods:After polishing and cleaning,pure titanium disks were stored in air,saline,BSA solution and BSA-coated Ag2S storage solution(Ag2S@BSA) respectively for two weeks. Scanning electron microscope(SEM),energy dispersive spectrometer(EDS) and contact angle test were used to analyze surface characteristics of pure titanium preserved in four media. Assays of adhesion,proliferation,and differentiation of MC3T3-E1 cells were conducted to evaluate the biocompatibility of different samples. Results:SEM and EDS results demonstrated that sodium chloride crystals were scattered on titanium surface in the saline group,while carbon and oxygen distribution were denser in the BSA group. Nano-sized particles containing silver and sulfur were adsorbed on the titanium surface without changing the original morphology. Compared with the other three groups,titanium surfaces preserved in Ag2S@BSA solution showed superior hydrophilicity and significantly promoted the adhesion proliferation and differentiation abilities of osteoblasts with high expression of osteogenesis-related proteins. Conclusion:The Ag2S@ BSA-containing storage solution can enhance the osteogenic activity of osteoblasts by improving the surface characteristics of pure titanium.

    • The effects of abrupt fluctuation of blood glucose and insulin on serum PCSK9 level during IVGTT

      Abstract:Objective:This study aims to investigate the influence of abrupt fluctuation of blood glucose and insulin on serum PCSK9 level during intravenous glucose tolerance test(IVGTT),and its potential relationship with glucose and lipid metabolism. Methods:IVGTT were detected on 23 healthy lean and 52 patients with obesity or abnormal glucose metabolism. Personal medical history and anthropometry were recorded. Lipid profile,liver function and kidney function were measured in fasting serum samples. Serum PCSK9 levels were detected in serum samples collected during IVGTT at different time points. According to the ratio of serum PCSK9 content between 4 minutes post intravenous glucose challenge and baseline,participants were grouped into the down-regulation group,the stable group and the up-regulation group. Glucose and lipid metabolism parameters were compared across these three groups. Besides,free fatty acids,ceramides and their metabolites in 39 serum samples were determined by high throughput targeting lipidomics. Results:In general,baseline PCSK9 level was positively correlated with total cholesterol and fasting C-peptide level after adjustment,but basal PCSK9 level was not corrected with fasting insulin level. With the increase of blood glucose during IVGTT,insulin increased sharply,accompanied by the fluctuation of serum PCSK9 level. The results showed that the basal PCSK9 and fasting insulin levels in the down-regulation group were significantly higher than those in the stable group and the up-regulation group,and the proportion of prediabetes and diabetes in the down-regulation group was higher. In addition,the serum levels of total ceramide and total glucosylceramide in the down-regulation group were significantly higher than those in the other two groups. The positive correlation between baseline PCSK9 content and CER D18:1/24:0 and GM3 D18:0/24:0 in lipid subclass analysis remained even after adjustment for other factors. Conclusion:During IVGTT,serum insulin level increases sharply along with instant increase in blood glucose,and serum PCSK9 content fluctuates as well. The fluctuation patterns of serum PCSK9 level during IVGTT is related with basal glucose and metabolism status.

      Abstract:Objective:This study aims to observe the expression and possible mechanism of a disintegrin and metalloproteinase 15(ADAM15)in asthma. Methods:The clinical data and serum samples of 32 asthma patients and 16 healthy controls were collected,enzyme-linked immunosorbent assay was used to detect the expression of serum ADAM15,and the correlation of peripheral blood eosinophils count and percentage,serum total IgE with ADAM15 was analyzed. A mouse model of OVA-induced allergic asthma was constructed,using immunohistochemistry to observe the expression of ADAM15 expression in lung tissue. Real-time quantitative PCR and Western blotting were used to detect the effect of different cytokines stimulation on the expression of ADAM15 in human bronchial epithelial cell line 16HBE. Results:The level of serum ADAM15 in the asthma group was higher than in the healthy control group[(749.5±132.3)pg/mL vs.(398.90±22.13)pg/mL,P=0.023]. ADAM15 is mainly expressed in bronchial epithelial cells in mouse lung tissue. After 24 hours of stimulation of exogenous IL-13(10 ng/mL),the mRNA and protein levels of ADAM15 were up-regulated. Conclusion:The expression of ADAM15 is elevated in asthma,and may be related with the IL-13-mediated airway inflammation.

    • The correlation between serum vitamin D binding protein and chronic kidney disease

      Abstract:Objective:This study aims to explore the relationship between serum vitamin D binding protein(DBP) level and the occurrence of chronic kidney disease (CKD),further establish a clinical risk prediction model for CKD and evaluate its predictive power for CKD. Methods:The patients who were hospitalized in the department of nephrology between March 2020 and September 2020 were selected. The serum DBP levels of the patients were detected by the ELISA method. The relationship between serum DBP and CKD related indicators was anacyzed. And the clinical data of the high DBP group and low DBP group were compared. A clinical risk prediction model including DBP was construct and its effectiveness was evaluated in predicting the occurrence of CKD. Results:Compared with the normal control group and CKD stage 3 patients,the serum DBP levels of patients with CKD stage 4 and CKD stage 5 were significantly lower. Serum DBP and CKD related indicators and interventions have obvious correlation. Compared with the comprehensive clinical risk prediction model A(AUC=0.917,95% CI:0.849~0.985,P<0.001),the comprehensive clinical risk prediction model B(AUC=0.942,95% CI:0.894~0.991,P<0.001) increased the serum DBP as a predictor variable,and significantly improved the AUC and,specificity for predicting CKD. Conclusion:Serum DBP decreases significantly in CKD 4-5 stages,which are correlated with the occurrence of CKD. The addition of the predictive variable of serum DBP significantly improves the predictive performance of the model in predicting CKD and helps to screen high-risk patients with CKD.

    • Analysis of postoperative progression pattern of pancreas intraductal papillary mucinous neoplasm

      Abstract:Objective:This study aims to analyze the postoperative progression pattern of intraductal papillary mucinous neoplasm(IPMN)and risk factors related. Methods:The clinical and follow-up data of 153 patients who received surgical resection and pathological confirmed of IPMN from 2010,March to 2019,December in the First Affiliated Hospital of Nanjing Medical University was retrospectively analyzed. According to the location of the progressive lesion,the progression patterns included:lesion growth at the pancreatic resection site,lesion growth in other parts of the pancreas,and distant metastasis. The proportion of each progress pattern,progress time and risk factors which affecting progress were analyzed. Results:Total 153 patients were included in the study group,107 patients were male(70%),median age was 66 years old(29-87 years old),59(39%)was branch duct IPMN(BD-IPMN),94(61%)was main duct IPMN(MD-IPMN)or mixed type IPMN(MT-IPMN). There were 88 cases(58%)with low grade dysplasia(LGD),28 cases(18%)with high grade dysplasia(HGD),and 37 cases(24%)with intraductal papillary mucinous carcinoma(IPMC). A total of 14 patients(9%)developed postoperative progression,including 5 patients(36%) with LGD and 9 patients with IPMC (64%). Progressive patterns included growth at the resected site in 7 cases(50%),growth at other sites in the pancreas in 2 cases(14%),distant metastasis in 4 cases(29%),and growth at the resected site combined with distant metastasis in 1 case(7%). Progression occurred within 5 years of surgery in 12 patients(86%)and 5 to 10 years after surgery in two patients(14%). Univariate analysis suggested that postoperative pathology was a risk factor for progression(P < 0.001). Conclusion:Although the prognosis of IPMN is good,there is still a high postoperative progression rate. It is necessary to conduct regular and long-term follow-up for patients with IPMN after surgery. Univariate analysis suggested that postoperative pathology was a risk factor affecting the progression. The progression pattern was mainly lesion growth at the resection site of the pancreas,suggesting that incomplete resection of the lesion during the first operation might be an important reason for the progression.

    • Comparative study of endoscopic submucosal dissection in the treatment of early gastroesophageal junction cancer and gastric cancer

      Abstract:Objective:The purpose of this study was to compare the difference of clinical and pathological features between early gastroesophageal junction cancer and gastric cancer and analyze the endoscopic submucosal dissection (ESD) efficacy,to evaluate the ESD safety and effectiveness of early gastroesophageal junction cancer. Methods:We retrospectively analyzed 338 patients who underwent ESD in the First Affiliated Hospital of Nanjing Medical University from January 2013 to February 2019,who were confirmed as early cancer by postoperative pathology. According to the location of lesions,157 cases of early gastroesophageal junction cancer were studied as the study group and 181 cases of gastric cancer as the control group. Results:Multivariate analysis showed that age ≥62.5 years,male,endoscopic flat type,differentiated adenocarcinoma and submucosal infiltration were independent risk factors for early gastroesophageal junction cancer(OR=2.182,2.299,2.497,2.425,2.939,P < 0.05). The operation time of the early gastroesophageal junction cancer group[(104.14 ± 58.49)min]was longer than that of the gastric cancer group[(73.39 ± 45.06)min,P<0.05],and the curative resection rate was lower than that of the gastric cancer group(70.7% vs. 80.7%,P=0.032). According to Kaplan-Meier analysis,there was no statistically significant difference in the overall survival rate(Log-rank P=0.889)and the survival rate of patients with curative resection(log-rank P=0.712) between the two groups. Univariate analysis showed that lesion diameter ≥2 cm(91.3% vs. 67.6%,P=0.002),poor pathological type(21.7% vs. 0.9%,P < 0.001),submucosal infiltration(60.9% vs. 13.5%,P < 0.001),lymphatic vascular infiltration(4.3% vs. 0,P=0.027),ulceration(23.9% vs. 5.4%,P < 0.001) were risk factors for noncurative resection of early gastroesophageal junction cancer. Conclusion:ESD is a safe and effective treatment for early gastroesophageal junction cancer,but the curative rate of ESD treatment is reduced for early gastroesophageal junction cancer with lesion diameter ≥2 cm,poor pathological type differentiation,submucosal infiltration,lymphatic vessel infiltration and ulcer.

    • Diagnostic value of inflammatory markers in malignant transformation of intraductal papillary mucinous neoplasm of pancreas

      Abstract:Objective:This study aims to investigate the diagnostic value of inflammatory markers including neutrophil/lymphocyte(NLR)and platelet/lymphocyte(PLR)in malignant transformation of pancreatic intraductal papillary mucinous neoplasm(IPMN). Methods:The IPMN patients in the First Affiliated Hospital of Nanjing Medical University from January 2015 to December 2020 were retrospectively analyzed. The general information,clinical indexes,imaging indexes and inflammatory indexes were collected,and the nomogram was used to construct the prognostic model of malignant IPMN. Results:NLR,PLR,CA19-9,(carcino embryonic antigen,CEA),cyst size,enhanced mural nodule and abrupt changes in the pancreatic duct caliber were associated with the malignant progression of IPMN. NLR,CA19-9 and cyst size were independent risk factors for malignant IPMN. PLR and cyst size were independent risk factors for IPMN with high-grade dysplasia(HGD). CA19-9 was an independent risk factor for IPMN with invasive carcinoma(IPMC). The area under the curve for predicting malignant IPMN based on NLR,CA19-9 and cyst size was 0.806. The prognostic model had good prediction performance. Conclusion:NLR,CA19-9 and cyst size were independent risk factors for predicting malignant IPMN,while PLR and cyst size were independent risk factors for predicting HGD. Inflammatory markers played important role in the malignant progression of IPMN,which can be used as a useful supplement to Fukuoka guidelines.

      Abstract:Objective:The aim of this study was to explore whether pretreatment Epstein-Barr virus DNA(EBV DNA)status may influence prognosis in newly diagnosed Hodgkin lymphoma(HL)patients. Methods:We retrospectively analyzed 82 HL patients diagnosed between January 2013 and November 2019 in the First Affiliated Hospital of Nanjing Medical University. Results:Among 82 enrolled HL patients,the median age was 32 years(range,12-79). With a median follow-up of 25 months(4-84 months),the 2-year progression-free survival(PFS) and overall survival(OS) was 68.4% and 93.0%,respectively,and 12.2%(10/82)patients had positive EBV DNA status. The median viral load of them was 8.66×103 copies/mL(range,5.21×103-5.35×104 copies/mL). Those with positive EBV DNA status had inferior PFS(P=0.003)as well as OS(P < 0.001). Baseline EBV DNA status was observed as an independent prognostic factor in OS(P=0.027)on multivariate Cox analysis while had a trend to predict PFS(P=0.056). Conclusion:Our data demonstrated that pertreatment EBV DNA status can be an optimal biomaker to reflect HL patients prognosis.

      Abstract:Objective:This study aims to compare the differences between CT measured and MRI measured femoral posterior condylar angle(PCA)in varus osteoarthritis knees. Methods:The CT data of lower-extremity and MRI data of involved knees in 71 patients(73 varus osteoarthritis knees)who underwent primary TKA at department of orthopedic surgery,Jiangsu Province Hospital from November 2018 to February 2020 were retrospected in our study. The CT and MRI images were used to measure PCA respectively. MRI images were also used to measure the cartilage thickness of femoral posterior condyles and distal femoral condyles. At the same time,the thickness of femoral posterior condylar cartilage of specimens was also measured. The measurement results were compared and analyzed for correlation. Results:The PCA-CT(4.09 ± 1.39)°was significant larger than PCA-MRI(2.25 ± 1.01)°(P < 0.01). The MRI measured cartilage thickness of medial posterior condyle(1.9 ± 0.6)mm and cartilage thickness of lateral posterior condyle(2.8 ± 0.5)mm,MRI measured cartilage thickness of medial distal femoral condyle(1.3 ± 0.4)mm and cartilage thickness of lateral distal femoral condyle(2.5 ± 0.4)mm,cartilage thickness of medial posterior condyle of specimens(1.9 ± 0.6)mm and cartilage thickness of lateral posterior condyle of specimens(2.8 ± 0.6)mm all presented significant difference between each other(P<0.01). But there was no statistical difference between the cartilage thickness of medial and lateral posterior condyles measured by MRI and that measured on the specimens(P>0.05).Also,Pearson correlation analysis results showed that:if 2mm cartilage thickness difference exists between medial and lateral posterior condyle,almost 3° external malrotation was found between PCA-CT and PCA-MRI. Conclusion:Cartilage wear of medial posterior condyle commonly occurs in varus knee osteoarthritis and this has an impact on the rotational alignment of femoral component. Significant differences exist in preoperative measurements of MRI and CT,and orthrpedic surgeons must take these differences into consideration when making preoperative planning.

      Abstract:Objective:This study aims to explore the effect of quantitative susceptibility mapping(QSM)in evaluating brain iron deposition in the patients with end-stage renal disease(ESRD)and the impact of iron deposition on cognitive function by comparing the magnetic susceptibility value(MSV)of the deep gray matter nuclei between the patients with ESRD and healthy controls,and analyzed the correlations between the MSVs of the patients with ESRD and the scores of the Montreal Cognitive Assessment(MoCA). Methods:Twenty-five patients with ESRD and 25 healthy controls who are age and sex-matched were recruited in this study. The MSVs of the deep gray matter nuclei of all the patients with ESRD and healthy controls were measured with QSM. The differences of the MSVs of the nuclei between the patients and controls were analyzed by the Mann-Whitney U test. The diagnostic efficacy of the MSVs was analyzed by the receiver operating characteristic(ROC)curve. The correlations between the MSVs of the patients and the scores of the MoCA were analyzed by Spearman correlation analysis. Results:In the patients with ESRD,the MSVs of bilateral red nucleus(RN),substantia nigra(SN),caudate head(CA)and putamen(PU) were higher than those in the controls(P<0.05 respectively). The MSVs of bilateral RN,SN,PU and CA can distinguish the patients from healthy controls. ROC analysis revealed that the cut-off values of PU MSV(0.085) provided the best combination of sensitivity(67%) and specificity(91%) to distinguish the ESRD patients from controls. In the patients with ESRD,the MSV of the right RN was negatively correlated with language ability and abstract ability,the left CA was negatively correlated with attention and language ability,and the left globus pallidus(GP) was negatively correlated with visuospatial and executive ability(P<0.05 respectively). Conclusion:In patients with ESRD,QSM not only can evaluate brain iron deposition quantitatively,but contribute to analyze the impact of iron deposition on cognitive function.

      Abstract:Objective:The aim of this study was to analyze the brain 18F-FDG PET-CT scan images of patients with Diffuse large B-cell lymphoma(DLBCL)without brain invasion by statistical parametric mapping(SPM)analysis. Methods:This retrospective study enrolled 52 DLBCL patients who underwent 18F-FDG PET-CT for the pre-treatment staging. The control subjects were 52 persons who underwent health checks with 18F-FDG PET-CT. All the patients and control subjects had no organic brain lesions and no lesions suggesting malignant tumors. The degree of regional cerebral glucose metabolism was evaluated on a voxel-by-voxel basis using SPM. The whole-body metabolic tumor volume(MTVwb)and the whole-body total lesion glycolysis(TLGwb)were measured,and the correlation between the level of cerebral glucose metabolism and MTVwb,TLGwb was analyzed. Results:Compared with the control subjects,visual analysis revealed extensive reduction in cortical glucose metabolim in some DLBCL patients. SPM showed that in DLBCL patients group,the FDG uptake in bilateral parietal,frontal lobe and occipital lobe were significantly lower than those in control group(P < 0.001). White matter,right hippocampus,left basal ganglia,bilateral globus pallidus and cerebellar tonsil showed higher FDG uptake either. TLGwb and MTVwb of the body tumor load were negative correlated with some small locations in cortex. Conclusion:18F-FDG PET-CT scan can sensitively detect the abnormal regional cerebral glucose metabolism in DLBCL patients. Cerebral glucose metabolism changes should be considered when evaluating DLBCL.

    • The diagnostic value of CT⁃guided percutaneous lung biopsy for the nodules of lung hilum

      Abstract:Objective:This study aims to investigate the diagnostic value,safety and feasibility of CT- guided percutaneous lung biopsy for hilar nodules less than 3.0 cm. Methods:The data of patients with the nodules of lung hilum without positive results from fibrobronchoscopy undergoing CT guided lung biopsy in our hospital from 2016,July to 2020,February were collected. The size and shape of the lesion,the length of the puncture path,the needle angle,cutting times and complications were recorded. Results:Total 62 patients were enrolled in the study. All cases were successfully conducted in the first biopsy including adenocarcinoma(n=29),squamous cell carcinoma(n=14), small cell lung cancer(n=4), specific infection(n=2), mixed cancer(n=6),metastatic cancer(n=1),and undetermined lesions(n=7). The accuracy rate,sensitivity and omission diagnostic rate was 98.44%(63/ 64),98.21 %(55/56)and 1.79%(1/56),respectively. The main complications were pneumothorax and pneumorrhagia,the incidence rates of which were 15.63%(10/64)and 43.75%(28/64),respectively. The length of the puncture path is important factor for two complications. The age and gender of patients are correlated with the incidence of pneumothorax. The cutting times and the shape of lesion are correlated with the incidence of pneumorrhagia. Conclusion:CT-guided percutaneous lung biopsy has higher diagnostic value and better safety for lung hilar nodules,playing a good guiding role in clinical diagnosis and treatment.

      Abstract:Objective:This study aims to analyze the predictive value of fractional flow reserve derived from coronary CTA(FFRCT)and plaque characteristics for major adverse cardiac events(MACE) in patients with coronary artery disease(CAD),and compare with myocardial perfusion imaging(MPI). Methods:Eighty patients with suspected CAD patients who underwent coronary CTA(CCTA) and MPI within 3 months were retrospectively enrolled and followed up. Measurement parameters included FFRCT,atherosclerosis plaque characteristics,summed stress score,summed difference score,phase standard deviation(SD),phase histogram bandwidth(BW). The differences of indicators were compared between positive MACE group and negative MACE groups. ROC curve analysis was performed to evaluate the predictive value of FFRCT combined with plaque characteristics and MPI for MACE. Results:MACE occurred in 11 patients. FFRCT,rest diastole SD(RDSD) and rest diastole BW(RDBW) in positive MACE group were significantly lower than that in negative MACE group(all P<0.05). Plaque length and non-calcified volume in patients with MACE were significantly higher(all P<0.05). The AUC of FFRCT and plaque length for prediction of MACE was 0.80 and 0.81,while the predictive value of RDSD and RDBW was 0.62 and 0.56,respectively. Conclusion:FFRCT and plaque characteristics demonstrated a higher predictive value for MACE than phase analysis of MPI in CAD patients.

      Abstract:Objective:This study aims to explore maternal factors and pregnancy related factors affecting intrauterine growth in mid and late pregnancy. Methods:We collected ultrasound data and maternal and health-related information during pregnancy of women who received antenatal examination and delivered in the Women’s Hospital of Nanjing Medical University. A total of 1 457 singleton live birth whose mother had pregnancies of at least 20 gestational weeks between August 2016 and May 2019 and received ultrasound examinations at 22-24,30-32 and 34-36 gestational weeks. Estimated fetal weight(EFW) was calculated based on head circumference(HC),abdomen circumference(AC) and femur length(FL) measured by ultrasound,and and the related factors of intrauterine growth in the middle and late pregnancy were analyzed by mixed linear model. Results:In multivariate analysis,maternal age was positively correlated with EFW at 22-24,30-32 and 34-36 gestational weeks. Compared with mothers with normal pre-pregnancy BMI(18.5 kg/m2≤BMI<24 kg/m2),mothers with underweight(BMI<18.5 kg/m2)had smaller fetuses,and mothers with pregestational overweight/obesity(BMI>24 kg/m2)have larger fetuses at 30-32 and 34-36 gestational weeks. EFW of girls was significantly smaller than that of boys at 22-24,30-32 and 34-36 weeks of gestation. Moreover,We found that pre-pregnancy dysmenorrhea was significantly negatively correlated with EFW during pregnancy. Conclusion:Maternal age,pre-pregnancy BMI,fetal sex and dysmenorrhea may affect estimated fetal weight.

      Abstract:Thyroid autoimmunity is the most common autoimmune disturbance in women of reproductive age. Numerous studies have shown that thyroid autoimmunity has been associated with infertility. In women treated with assisted reproductive technology,thyroid autoimmunity may influence embryo quality,pregnancy rate,and miscarriage rate. But its pathogenic mechanism is not yet clear. Some investigators believe that thyroid autoimmunity is not the main cause of adverse pregnancy outcomes. This paper focused on the effect of thyroid autoimmunity on assisted reproductive technology. And it also suggested investigators to optimize the experimental design and obtain more authoritative results for guiding clinical practice.

      Abstract:In mammals,the levels of gonadal hormones present circadian rhythm,which is controlled by the central circadian clocks and the suprachiasmatic nucleus. The disorder of circadian clock can change the internal environment of the body,while the circadian rhythm of gonadal hormones is destroyed,leading to the occurrence of some reproductive system diseases. In addition,gonadal hormones,such as estrogen and androgens,have effects on the central and peripheral circadian clock,play a role in the expression of clock genes,and affect the metabolism and reproduction. Therefore,to investigate the relationship between gonadal hormones and circadian clock system is crucial to understanding the development of reproductive system diseases. This article explored the interaction between gonadal hormones and circadian clocks to further understand the occurrence and development of reproductive system diseases.

It is closely related to the survival and development of cells,and has been confirmed to be associated with the development of many diseases including asthma,which is a hotspot of current research. This review will summarize and evaluate the progress of study of the relationship between autophagy and asthma.
