    • Efficient construction of Bama minipig F9 gene knockout cell line using CRISPR/Cas9 technology

      Abstract:Objective:This study aims to investigate the changes of glycosphingolipid,especially the ganglioside GM3,in the liver of NASH model mice induced by high-fat and high-sugar diet . Methods:A total of 16 C57BL/6J mice aged 6-weeks old were randomly allocated into control group and NASH group(control n=8;NASH n=8). Control mice were fed with a standard chow diet and water as control group,while the other mice were fed high fat diet accompanied by ad libitum consumption of water with high fructose and glucose content (23.1 g/L d-fructose +18.9 g/L d-glucose)to establish NASH models as model group. Serum levels of ALT and AST were measured to evaluate the pathological changes of liver tissues. Liver histology was assessed using HE stains in paraffin-embedded sections using standard commercially used methods. Liver lipidomics analysis was perfonned in an electrospray ionization mode using Exion UPLC-QTRAP 6500 PLUS(Sciex)LC/MS. The student’s t test and Mann-Whitney U test were used for statistical analysis. Results:①The levels of alanine aminotransferase(ALT) and aspartate aminotransferase(AST) in NASH mice were significantly higher than that in control group(P < 0.05). ②Compared with the control group,total GM3 and subtype species,including GM3 d18:1/20:0(P=0.005),GM3 d18:1/22:0(P=0.004),GM3 d18:1/24:0(P=0.004)and GM3 d18:1/26:0(P=0.042)were significantly lower in the model group,which were dignosed as NASH group based on the NAFLD activity score. Conversely,total ceramide and total lactosylceramide(LacCer) were remarkably elevated in NASH group than control group(P < 0.05). Conclusion:The synthetic pathway from LacCer to GM3 is dampened in NASH model mice,which are established by high-fat and high-fructose diet.

      Abstract:Objective:This study aims to investigate the effects of adenovirus E1A associated 300 kDa protein(p300) on the migration,invasion and matrix metalloproteinase-2(MMP2) expression in non-small cell lung cancer(NSCLC) cells stimulated by IL-17. Methods:After NSCLC cells(H1299) incubated with IL-17,scratch and transwell experiments were performed to observe the cell migration and invasion. Then the p300 and MMP2 protein expression was detected using Western blot. Meantime,p300 overexpression plasmids(pcDNA3.1/p300) or short hairpin RNA(shp300) plasmids were transfected into H1299 cells(the latter was stimulated with IL-17),respectively,and the cell migration,invasion and MMP2 synthesis were again examined by the same method described above. Results:The migration and invasion as well as the expression of p300 and MMP2 were obviously elevated in H1299 cells treated by IL-17. And p300 overexpression significantly promoted the cell migration,invasion,and MMP2 expression. While p300 gene knockdown by shp300 led to the decrease of migration,invasion and MMP2 production in H1299 cells induced by IL-17. Conclusion:IL-17-upregulates p300 in H1299 cells which enhances the cell migration,invasion and MMP2 expression.

      Abstract:Objective:This study aims to investigate the effect of PI3K/Akt pathway on the regulation of large conductance Ca2+-activated K+ channel(BK channel)in diabetic coronary smooth muscle cells. Methods: Human coronary artery smooth muscle cells(HCASMC) were incubated with different concentrations of glucose(5 mmol/Land 15 mmol/L),reactive oxygen species(ROS) inhibitor GKT137831 and PI3K inhibitor wortmannin. Streptozotocin-induced diabetic rats were established successfully by intraperitoneal injection. Expression of DNA-Pkcs,Akt,p-Akt(S473), BK-α subunit and BK-β1 subunits were determined by Western blot. ROS generation in HCASMC was measured in terms of fluorescence using the cell-permeable fluorogenic probe DCFH-DA. Results: Compared with that of HCASMC incubated in 5 mmon/L glucose concentration,the expression of DNA-Pkcs,Akt,p-Akt(S473) increased significantly and BK-β1 subunit decreased significantly(P < 0.05). Compared with the control group,the expression of p-Akt(s473)decreased significantly(P < 0.05) and BK-β1 increased significantly (P < 0.05) after pretreated with Wortmannin. Conclusion:PI3K/Akt channel is involved in the regulation of BK channel expression in diabetic coronary smooth muscle cells.

      Abstract:Objective:This study aims to explore the inhibitory effect and mechanism of sulforaphane (SFN) on proliferation and migration of mouse lewis lung cancer cells (LLC). Methods:CCK-8 method was used to identify the effect of SFN on LLC proliferation. Flow cytometry technology was used to detect the effect of SFN on cell cycle and apoptosis of LLC cells. DCFH-DA probe was used to detect the ROS expression in LLC cells. The effect of SFN on ERK signaling pathway in LLC cells was detected by Western blot. Finally,C56BL/6 mouse lung cancer tumor model was established by subcutaneous injection. After modeling,the experimental group and the control group were given SFN and PBS gavage treatment respectively,to detect the effect of SFN treatment on mouse tumors. Results:Cell experiments confirmed that SFN can significantly inhibit the proliferation and promote the apoptosis of LLC cells(P < 0.05). With the increase of SFN concentration,the proportion of late apoptosis of LLC cells gradually increased(P < 0.05). Cell cycle analysis showed that 12.5 μmol/L and 25.0 μmol/L SFN treatment reduced the proportion in G1 phase(P < 0.05)and increased the proportion of LLC cells in G2/M phase(P < 0.01). The expression of cycle-related genes CyclinD1,CDK4,CDK6,CyclinB1 and CDK1 decreased significantly(P < 0.01). Western blot suggested that SFN promoted ERK phosphorylation and increased Nrf2 expression in LLC cells(P < 0.05). The flow test results showed that SFN could significantly increase ROS level in LLC cells. The results of animal experiments showed that the tumor volume of SFN treated mice was significantly smaller than that of control group(P < 0.05). Conclusion:In vivo and in vitro experiments confirmed that SFN triggered ERK phosphorylation,which increased the expression of Nrf2,improved the expression level of ROS and blocked cell cycle,thus accelerating apoptosis and inhibiting the growth of tumor cells. SFN may be a safe and effective anti-cancer drug.

      Abstract:Objective:This study aims to investigate the mechanism of lansoprazole on skin inflammation through in vivo and in vitro experiments. Methods:In vivo experiment,mice were given continuous gavage for 6 months. The skin tissue damage of mice was evaluated and analyzed by hematoxylin-eosin staining(HE staining)and immunohistochemistry(IHC). In vitro experiment,CCK-8 was used to determine the changes of HaCaT cell viability after incubation with 10-200 μmol/L lansoprazole for 24 h and 48 h. After incubation with 10 μmol/L,20 μmol/L and 40 μmol/L lansoprazole for 24 h and 48 h,respectively,the phosphorylation levels of NF-κB in HaCaT cells and the protein expressions of interleukin(IL)-6 and IL-1β in HaCaT cells were detected by Western blot. In addition,the protein expressions of P-p65,IL-6 and IL-1β in HaCaT cells were detected after 24 h and 48 h incubation of HaCaT cells with pyrrolidine dithiocarbamate,an inhibitor of NF-κB signaling pathway,alone or in combination with lansoprazole. Results:In vivo experiment showed that lansoprazole can cause skin damage in mice. In vitro studies found that lansoprazole can decrease cell viability,increase the expression of P-p65 proteins,mediate the activation of the pathway,and further stimulate the expression of inflammatory cytokines IL-6 and IL-1β. PDTC can inhibit the lansoprazole-mediated activation of NF-κB pathway and reduce the expression of inflammatory factors. Conclusion:Lansoprazole can promote the expression of inflammatory factors and induce skin injury,and the mechanism may be related to the activation of NF-κB signaling pathway.

    • The role of MCT1 in predicting the prognosis of patients with hepatocellular carcinoma

      Abstract:Objective:This study aims to explore the application value and possible mechanism of monocarboxy transporter 1(MCT1)in the prediction of prognosis for the patients with hepatocellular carcinoma(HCC). Methods:We selected the tissues of HCC patients after liver resection,and used RT-PCR to estimate the expression of MCT1 and divided the patients into MCT1 high and MCT1 low groups;the phenotype and activation of the immune cells from the peripheral blood as well as the immune statues of the resected liver tissues from both groups were analyzed. A retrospective analysis of the relationship between the overall survival(OS) and disease-free survival(DFS) rate to the expression of lactate or MCT1 between 43 HCC patients who underwent liver surgery was also compared. Results:Although there was no difference in the phenotype of immune cells from peripheral blood,CD4+ T cells were easier to transform into Treg cells under the stimulation of lactic acid in the high MCT1 expression group;under the environment of lactic acid,the Treg cells from the MCT1 high expression group presented a significant increase of metabolism level;MCT1 high expression group had lower immune cell infiltration and up-regulated expression of anti-inflammatory cytokines(Foxp3,CTLA3,IL-10). The OS and DFS rate were much worse in MCT1 high patients comparing with MCT1 low group. Conclusion:MCT1-Treg related targets may serve as a new target for the diagnosis,therapeutic and prognosis prediction of HCC.

    • Expression and significance of tumor associated macrophages subtypes in endometrial cancer

      Abstract:Objective:This study aims to investigate the expression of CD163,CD68 in endometrial cancer(EC)and its relation with the clinicopathological features and overall survival rate. Methods:A total of 46 EC tumors were collected from January 2010 to December 2015 in Changzhou Maternal and Child Health Care hospital affiliated to Nanjing Medical University as the experimental group,while contemporaneous 25 cases with benign endometrial diagnosed were selected as the control group. Immunhistochemistry was applied to detect the expressions of CD68 and CD163 in the two groups. Taking the median expression level of tumor-associated macrophage(TAM)as the cut-off value,experimental group was divided into high expression group and low expression group of CD68 or CD163. The median ratio of CD163/CD68 was also taken as the cut-off value for CD163/CD68 high ratio group and low ratio group. The relationship of their expressions and clinicopathological features and prognosis of endometrial cancer were analyzed. Results:The expression of CD163 and CD68 in EC was significantly higher than that in control group. The expression of CD163 was associated with high pathological grade,high FIGO stage,lymph node metastasis. The expression of CD68 was associated with myometrial invasion,high FIGO stage. The ratio of CD163/CD68 was associated with high pathological grade,high FIGO stage,lymph node metastasis. Survival analysis found that there was no sigfnificant statistical difference in 5 year overall survival rate between the patients with the expression of CD163, CD68 and the ratio of CD163/CD68. Only lymph node metastasis was related to survival. Conclusion:TAM may be involved in the development of uterine endometrial cancer. The expression of CD163 and CD68 may play an important role in the progression of EC.

    • Prognosis after mechanical thrombectomy in elderly atrial fibrillation patients with anterior⁃circulation acute ischemic stroke

      Abstract:Objective:To evaluate the clinical outcomes after mechanical thrombectomy(MT)in elderly atrial fibrillation(AF)patients with anterior-circulation large vessel occlusion(LVO)-related acute ischemic stroke(AIS). Methods:Between January 2019 and July 2021,consecutive elderly(≥80 year)AF patients who received MT due to anterior-circulation stroke were enrolled. The primary outcome was modified Rankin scale(mRS)score at 90 days. Secondary outcomes included all-cause mortality,the recanalization status after MT(assessed using modified thrombolysis in cerebral infarction scale,mTICI)and any intracranial hemorrhage(ICH). A multivariable logistic regression model was performed to identify predictors of the functional outcome. Results:A total of 74 eligible patients were finally enrolled. The proportion of patients with mRS score 0-2 at 90 days after stroke was 21.6%. The rate of satisfied recanalization with mTICI2b-3 was 77%. The rate of all-cause mortality and intracranial hemorrhage during 90 days after stroke was 36.4% and 35.1%,respectively. A multivariable logistic regression analysis showed that non-paroxysmal atrial fibrillation(OR=18.35,95%CI:1.98~169.77,P=0.010),baseline National Institute of Health stroke scale(NIHSS)score(OR=1.42,95%CI:1.15~1.75,P < 0.001),total procedure time(OR=1.01,95%CI:1.00~1.02,P=0.030) were independent risk factors for poor prognosis. Conclusions:MT treatment in elderly AF patients with AIS should be performed with caution,especially in those with high NIHSS scoreand/or non-paroxysmal AF. In addition,efforts should be made to improve the anticoagulant rate of elderly patients with AF to prevent stroke.

    • Reanalysis of risk factors of post⁃procedure heart blocks after transcatheter closure of perimembranous ventricular septal defect

      Abstract:Objective:This study aims to observe the condition of post-procedure heart blocks after transcatheter closure of perimembranous ventricular septal defect(pmVSD) under the guidance of the new treatment strategy of transcatheter closure of pmVSD based on previous experience. To investigate the risk factors and prevent post-procedure heart blocks. Methods:The clinical data of the 196 patients who successfully underwent transcatheter closure of pmVSD from January 2012 to July 2020 were analyzed. Univariate and multivariate logistic regression models were used to analyze and explore the risk factors. Results:Total 18 patients(18/196,9.2%)developed post-procedure heart blocks,including complete and incomplete right bundle branch block,left anterior fascicular block,intraventricular block and first degree atrioventricular block. No complete left bundle branch block and high-degree atrioventricular block occurred. All post-procedure heart blocks recovered within 2 months. The incidence of heart blocks in the basal occlusion group(12.7%,17/134)was higher than the intratumoral occlusion group(1.6%,1/62)(P=0.014). The analysis of basal occlusion group showed that distance from the aortic valve to the defect(DAVD)(P=0.015)and the diameter of the defect(P=0.022)were positively related to post-procedure heart blocks. Conclusion:The incidence of heart blocks is higher in the basal occlusion group. In the basal occlusion group,DAVD and the diameter of the defect are independent risk factors of post-procedure heart blocks.

    • Comparison of clinical efficacy of two long⁃acting gonadotropin⁃releasing hormone agonists in down⁃regulation combined with hormone replacement cycle of frozen⁃thawed embryo transfer

      Abstract:Objective:This study aims to compare the clinical efficacy of two long-acting gonadotropin-releasing hormone agonists(GnRH-a)in down-regulation combined with hormone replacement cycle frozen-thawed embryo transfer(FET). Methods:A retrospective cohort study of 462 frozen-thawed blastocyst transfer cycles using two different long-acting GnRH-a for endometrial preparation was performed at the Department of Reproductive Medicine,the Affiliated Obstetrics and Gynaecology Hospital of Nanjing Medical University from November 1,2019 to January 31,2021,including 324 cycles in the triptorelin group and 138 cycles in the leuprorelin group. Logistic regression model was used to analyze the relationship between two long-acting GnRH-a and the clinical efficacy of frozen-thawed blastocyst transfer. Results:The level of LH[(2.70±1.28)mU/mL]on estrogen initiation day was higher than that in triptorelin group[(2.41±1.19)mU/ mL],P=0.017];The implantation rate of leuprorelin group(62.6%)was higher than that of triptorelin group(54.6%),and the difference was statistically significant(P=0.044);the clinical pregnancy rate was increased compared with the triptorelin group,but the difference was not statistically significant(P=0.334),and the early abortion rate was decreased compared with the triptorelin group,but the difference was not statistically significant(P=0.490). In the logistic regression model,after adjusting for confounding factors including age,number of blastocyst,number of good-quality blastocyst,the level of LH on estrogen initiation day and endometrium thickness,the adjusted OR value of clinical pregnancy rate in leuprorelin group versus triptorelin group was 1.493(95% CI= 0.932~2.392,P=0.095). Conclusion:In the down-regulation combined with hormone replacement cycle of FET,both leuprorelin and triptorelin can achieve satisfactory down-regulation effect. Leuprorelin can improve the blastocyst implantation rate,and obtain similar clinical outcomes with triptorelin.

    • A clinical study on the correlation between 24⁃hour urinary protein and its adverse pregnancy outcome in pre⁃eclampsia

      Abstract:Objective:The correlation between 24-hour urinary protein quantity and pregnancy outcome in patients with preeclampsia was analyzed retrospectively,which provided clinical basis for diagnosis and treatment of preeclampsia. Methods:Total 222 patients with preeclampsia treated in the First Affiliated Hospital of Nanjing Medical University were selected(diagnosis of gestational age 22~40+6weeks). According to the results of 24-hour urinary protein quantification,they were divided into mild albuminuria group,moderate albuminuria group,and severe albuminuria group. We compared the clinical manifestations,laboratory indexes and adverse pregnancy outcomes of pregnant women,fetuses and newborns among the three groups. Results:①Clinical manifestations:the onset of preeclampsia was significantly earlier in the moderate and severe group than that in the mild group,and the clinical manifestations were more serious. ②Laboratory indexes:plasma albumin,serum total cholesterol,lactate dehydrogenase,urea nitrogen and creatinine levels increased significantly in the moderate and severe groups;platelet count decreased significantly in the severe group compared with the mild group,while glutamic oxaloacetic transaminase,triglyceride and D dimer levels increased significantly in the severe group. ③Maternal adverse pregnancy outcome:the incidence of pleural effusion,HELLP syndrome and peritoneal effusion in moderate and severe group increased significantly. ④Fetal adverse pregnancy outcome:iatrogenic preterm birth,fetal growth restriction and neonatal asphyxia increased significantly in moderate and severe groups. ⑤ROC curve analysis showed that 24-hour urinary protein quantitative analysis had certain predictive value for HELLP syndrome,pleural effusions and hydroceliac effusion,iatrogenic premature delivery and the occurrence of FGR in pregnant women. Conclusion:24 h urine protein quantitative degree is associated with adverse pregnancy outcomes such as HELLP syndrome,pleural and peritoneal effusion,FGR,iatrogenic preterm birth,and can be used as an observation index for the development of preeclampsia.

      Abstract:Objective:This study aims to improve the clinicians’ levels of respiratory management and treatment of children with spinal muscular atrophy(SMA)with pulmonary infection. Methods:A retrospective analysis of the clinical data of 49 children with SMA complicated withpulmonary infection admitted from January 1,2013 to July 31,2020. Results:Among the 49 cases,35 cases were type Ⅰ,14 cases were type Ⅱ,and there was no type Ⅲ patient. Compared with type Ⅱ,SMAⅠ children with lung infection had an earlier age of onset,a longer average hospital stays(P < 0.05). The rate of cardiac insufficiency and respiratory failure in children with SMAⅠ type was significantly higher than that of patients with SMAⅡtype(P < 0.05). The cough rate of SMAⅡ type was significantly higher than that of SMAⅠ type patients(P < 0.05). The detection of respiratory pathogens of the SMAⅠ type is mainly multi-drug resistant bacteria and conditional pathogens,and the SMAⅡ type is mainly infected by Mycoplasma and Streptococcus pneumoniae. The rate of invasive ventilation and the use of carbapenem antibioticsin children with SMAⅠ were significantly higher than those in the SMAⅡgroup(P<0.05). Conclusion:Children with SMA are prone to pulmonary infection. Actively promoting the discharge of airway secretions,appropriate respiratory support and reasonable anti-infection are crucial for the treatment of SMA complicated with lung infection.

    • Effectiveness evaluation of artificial intelligence system in the diagnosis of fresh rib fractures

      Abstract:Objective:This study aims to evaluate the effectiveness of using an artificial intelligence system to assist radiologists in the diagnosis of fresh rib fractures with computed tomography(CT). Methods:A dataset of 508 cases with fresh rib fracture CT images were collected and analyzed by 6 radiologists independently with the PACS system,the radiologists were divided into two groups:low seniority group(≤5 years)and high seniority group(>5 years). After a washout period of four weeks,the CT images were evaluated again by the 6 radiologists with the assistance of AI system. The fracture type,site,and time of diagnosis were recorded for further analysis. The paired chi-square test was used to compare the sensitivity and specificity in diagnosis of fresh fracture with and without AI assistance. The receiver operating characteristic curve(ROC)and the area under curve(AUC)was calculated. Cohen’s kappa coefficient was used to analyze the consistency between low and high seniority radiologists in diagnosing rib fractures. Paired sample t test was used to compare the difference in diagnosis time. Results:A total of 2 883 fresh rib fractures were found in the 508 CT images. With the help of AI-assisted diagnosis,the diagnostic sensitivity of low and high seniority radiologists increased significantly from 77.95%,83.96% to 88.52%,90.98%(P < 0.001);The average AUC increased from 0.902 to 0.948(P < 0.001);The Cohen’s kappa coefficient between low and high seniority radiologists increased from 0.832 to 0.900. The average diagnosis time of each case decreased 28.43s,P < 0.001. Conclusion:With the AI assistance in the diagnosis of fresh rib fracture,the detection efficiency of rib fracture is improved and the diagnosis time is reduced.

    • Genetic polymorphisms in HNF1A and HNF1B are associated with breast cancer susceptibility

      Abstract:Objective:We investigated whether genetic variants of HNF1A and HNF1B were associated with the risk of breast cancer in a Chinese Han population. Methods:A case-control study of 1 032 breast cancer cases and 1 063 cancer-free female controls was performed,and six single nucleotide polymorphisms(SNPs)in HNF1A and HNF1B were genotyped in Illumina genotyping platform. Logistic regression was used to calculate the odds ratios(ORs)and 95% confidence intervals(CIs)for the association between a single locus and breast cancer risk with adjustment for age,age at menarche,and menopausal status. Results:Multiple logistic regression analysis illustrated that three SNPs within HNF1A gene were associated with significantly decreased risk of breast cancer(rs2464196,OR=0.80,95%CI:0.71~0.91,P=7.45×10-4;rs1183910,OR=0.87,95%CI:0.77~0.99,P=0.039;rs7310409,OR=0.86,95% CI:0.76~0.98,P=0.023). After correction for multiple comparisons(FDR<0.05),rs2464196 was still significantly associated with susceptibility to breast cancer(P=4.47×10-3). Conclusion:The SNP rs2464196 within HNF1A gene may be a candidate marker for susceptibility to breast cancer in Chinese Han population.

      Abstract:Objective:This study aims to analyze the epidemiological characteristics of the outbreak caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)Delta variant in Jiangsu Province and to provide scientific basis for epidemic prevention and control. Methods:The coronavirus disease 2019(COVID-19) data,reported-from-July-20-to-August-28-in-Jiangsu Province,such as basic information,epidemiological history,date of diagnosis,was collected for statistical descriptive analysis. Results:During periods-of-investigation-time,there were 820 cases in Jiangsu Province,and the fatality rate was of all cases 0. Of all cases 0.341 (41.6%) were male. The median age was 48 years. At the time of diagnosis,differences of the median age between mild,moderate and severe cases were statistically significant(P < 0.05). The median incubation period was 9.5(7.5,11)days. The median serial interval was 3(2,5)days. Differences in serial intervals between 1st to 2nd and 2nd to 3rd generation were statistically significant(P < 0.001),2nd to 3rd and 3rd to 4th generation were statistically significant(P < 0.05). The sensitivity of the first nucleic acid testing was 77.7%. Thee were 3 super communicators in the epidemic. Conclusion:The SARS-CoV-2 Delta variant is with high transmissibility and spreads rapidly. All age groups are susceptible the Delta variant,and the age was trending younger. Severe cases are more likely to occur in the elderly. Multiple nucleic acid tests are recommended for patients with fever,cough and other symptoms in medium-high risk areas.

      Abstract:Objective:This study aims to compare the performance of seasonal autoregressive integrated moving average(SARIMA)model and long-short term memory(LSTM)model in predicting the epidemics of scarlet fever in Jiangsu Province,China,which may provide decision basis for prevention and control of scarlet fever. Methods:SARIMA model and LSTM model were fitted with data of monthly scarlet fever cases as well as meteorological variables during 2005-2017 in Jiangsu Province,China. Performance of the models were evaluated with data during 2018-2019. Results:The epidemics of scarlet fever in Jiangsu Province showed obvious seasonality,higherincidences were observed from April to June and from November to next January. SARIMA(3,1,2)(1,1,1)12 had the best perfor mance of all the alternative SARIMA models. The optimal LSTM model had 4 LSTM layers and 1 full connected layer,and each LSTM layer contains 32 memory cells. The mean absolute percentage error(MAPE)of SARIMA model and LSTM model in testingset were 22.47% and 16.94%,respectively,and the root mean squared error(RMSE)were 227.85 and 152.46,respectively. Conclusion:LSTM model performed well in predicting the incidence of scarlet fever in Jiangsu province. This model can be used to investigate the prevalence trends and assess the epidemic risk of scarlet fever,thus providing basis for optimizing and adjusting monitoring,prevention and control strategies and measures of this disease.

      Abstract:Objective:The co-existence of fatty liver and chronic hepatitis B(CHB) is common among patients. The study was aimed to explore the prognosis of CHB patients complicated with fatty liver. Methods:A retrospective study of 915 patients with CHB treated in the department of infectious disease of Jiangsu Province Hospital between 2016 and 2017 was conducted. Liver steatosis assessment was performed using Fibroscan. HBV DNA viral load and the degree of liver fibrosis were compared between the two groups with or without fatty liver. Results:Among the 915 CHB patients,447 of whom were diagnosed with fatty liver(48.8%). Controlled attenuation parameter(CAP) value was negatively correlated with HBV DNA level. Compared with the simple CHB patients,CHB with fatty liver had a lower degree of liver fibrosis(OR=0.79,95%CI:0.67-0.92,P=0.003). CAP values were U-shaped correlated with alanine transaminase. Conclusion:This finding indicates that the severity of liver inflammation is aggravated in CHB patients with severe fatty liver.

      Abstract:Objective:The structural equation model(SEM)was used to analyze the data of psychological scales to explore the effects of these on willingness to use and adherence to HIV pre-exposure prophylaxis(PrEP)among men who have sex with men(MSM),and to provide the basis for effective implementation of PrEP intervention strategy among MSM in China. Methods:Online recruitment and time-location sampling(TLS)were used to recruit MSM in Nanjing. Questionnaire survey was filled by Questionnaire Star software. Three psychological factors of anticipated HIV stigma,community homophobia and depressive symptoms were included in the SEM to explore the mechanism of these interaction with willingness to use and adherence to PrEP among MSM. Results:There were 75.73% of participants willing to use PrEP and 55.34% of participants self-reported high intention to adhere to PrEP. Participants had high anticipated HIV stigma and community homophobia,with median 20(17~23)and 27(22~30),respectively,but had few depression symptoms,with median 1(0~2). Anticipated HIV stigma had a direct positive effect on willingness to use and adherence to PrEP,with effect sizes of 0.13(P < 0.05) and 0.14(P < 0.05),respectively. Community homophobia negatively predicted PrEP adherence(β=-0.15,P < 0.05). Depression symptoms played a partially mediating role between community homophobia and PrEP adherence,with an effect size of 0.03(P < 0.01). Conclusion:High willingness to use PrEP,but low drug adherence were indicated among MSM. Psychological factors had significant effects on the willingness to use and adherence to PrEP. Psychological services should be in conjunction with PrEP prevention and intervention strategies to achieve better benefits of PrEP.

      Abstract:Traumatic brain injury(TBI)is a structural and physiological disruption of brain function caused by external forces. It is the leading cause of death and disability worldwide. TBI includes both primary and secondary injuries. Excessive iron accumulation and ferroptosis are highly involved in the pathophysiological process of secondary brain injury. Ferroptosis is a form of regulatory cell death,characteristic as increased iron accumulation,lipid peroxidation,reactive oxygen species(ROS)production,mitochondrial dysfunction and neuroinflammatory responses,resulting in cellular and neuronal damage. For this reason,eliminating factors like excessive accumulation of iron and inhibiting lipid peroxidation may be a promising therapy approach. Iron chelators can be used to eliminate excess iron and to alleviate some of the clinical manifestations of TBI. In this review,it was focused on the mechanisms of iron and ferroptosis involved in the manifestations of TBI,broadening our understanding of the use of iron chelators for TBI,better seeking new research directions for further treatment of TBI.

      Abstract:Uremia cardiomyopathy,the most important direct cause of death in end stage kidney disease patients,is one of the important cardiovascular complications of chronic kidney disease. Uremia cardiomyopathy is characterized by left ventricular hypertrophy,fibrosis,systolic and/or diastolic dysfunction. The pathogenesis of uremia cardiomyopathy is complex and has not been fully elucidated. This article reviews the recent advances in the pathogenesis of uremia cardiomyopathy from the perspectives of non-traditional cardiovascular risk factors. In addition,we introduce our latest results,and provide a new possible entry point for the research of pathogenesis of uremia cardiomyopathy.

      Abstract:Bronchopulmonary dysplasia is a common chronic lung disease in very low birth weight infants and lacks effective treatments. Mesenchymal stem cells (MSC) provide a new treatment for bronchopulmonary dysplasia due to their characteristics of immunoregulation,anti-inflammatory,and promotion of regeneration. Animal studies have shown that MSC are effective in the treatment of bronchopulmonary dysplasia,and it has entered the clinical trial stage. Basic research found that mesenchymal stem cells mainly play a role through paracrine,and mesenchymal stem cell-derived exosomes are a physiologically relevant and powerful component of the MSC secretion group. Therefore,this paper reviews the biological functions of mesenchymal stem cell-derived exosomes and their progress in the treatment of bronchopulmonary dysplasia.

      Abstract:Podocyte is an important part of maintaining the glomerular filtration barrier. Genetic,immune,infection and other factors often lead to the destruction of podocyte structural integrity and cell metabolism disorders,resulting in podocyte foot process fusion,podocyte detachment from the glomerular basement membrane,causing proteinuria,and even glomerular sclerosis and other kidney diseases. However,podocytes,similar to permanent cells such as nerve cells and muscle cells,are cells with no proliferative potential. Therefore,podocyte injury is crucial in the pathogenesis of renal disease. The physiological and functional changes caused by podocyte injury are not simple passive processes,but complex interactions among proteins that constitute the anatomy of podocytes. This article reviews the mechanisms of podocyte injury from the aspects of cytoskeleton injury,abnormal transcription factors in the nucleus,abnormal energy metabolism of cytoplasmic mitochondria,and changes in intracellular calcium dynamic stability.

      Abstract:Neuroblastoma is an embryonic tumor of the peripheral sympathetic nervous system. It is the most common extracranial solid tumor of childhood and accounts for up to 15% of all pediatric cancer fatalities. It has a varied prognosis,ranging from spontaneous regression to aggressive metastatic tumors with fatal outcomes despite multimodality therapy. Patients are divided into risk groups on the basis of age,stage,and biologic tumor factors. Multiple clinical and imaging tests are needed for accurate patient assessment. Nuclear medicine has both anatomical and functional imaging and is a useful method for staging and assessment. As the disease progresses,neuroblastoma is often accompanied by increased adrenal medulla function,high expression of somatostatin receptors and a significant increase in glucose metabolism,accompanied by catecholamines and metabolites,and a significant increase in serum chromogranin. Positron emission tomography/computed tomography(PET/CT),as a functional imaging,plays an important role in the diagnosis of neuroblastoma and has received widespread clinical attention. Theragnostic of nuclear medicine have emerged and are demonstrating promising response rates for patients at high risk. This article reviews the research progress of radionuclide theragnostic in children with refractory neuroblastoma.
