    • Effects of inhaled CDDO ⁃NO on monocrotaline ⁃induced pulmonary arterial hypertension in rats

      Abstract:Objective:This study aims to investigate the effects of a novel hybrid compound CDDO -NO from bardoxolone methyl (CDDO-Me)and NO donor isosorbide 5-mononitrate on monocrotaline(MCT)-induced pulmonary arterial hypertension(PAH)in rats. Methods:Male Sprague-Dawley rats were randomly divided into 6 groups:control(Con)group,MCT group,MCT+10 μg/kg CDDO-NO (MCT+10NO)group,MCT+30 μg/kg CDDO-NO(MCT+30NO)group,MCT+7 μg/kg CDDO-Me(MCT+7Me)group and MCT+21 μg/kg CDDO-Me(MCT+21Me)group. PAH in rat was induced by a single dose injection of 60 mg/kg MCT intraperitoneally. The rats in each treatment groups were treated for daily aerosol CDDO -NO or CDDO -Me inhalation for 28 days. Right ventricular systolic pressure (RVSP)by right heart catheter,and right ventricular hypertrophy index(RVHI)as well as RV -to - body weight ratio(RV/BW)were measured. Hematoxylin-eosin(HE)staining was used to detect the pulmonary artery medial thickness(PAMT)and the morphological changes of the right ventricle. And α-smooth muscle actin(α-SMA)immunohistochemistry was used to investigate the muscularization of distal vessels. Masson’s trichrome staining(MTS)was performed to evaluate the fibrosis of arterioles. Results:Right heart catheter and right ventricular remodeling indicators showed that inhalation of either CDDO-NO or CDDO-Me suppressed elevations of RVSP, RVHI and RV/BW induced by MCT in PAH rats. CDDO-NO(30 μg/kg)showed stronger inhibitory effect in RVHI and RV/BW than that of the equal dose of parent compound CDDO-Me(21 μg/kg). Moreover,pathological studies indicated that 30 μg/kg CDDO-NO had better effects in alleviating right ventricular myocardial hypertrophy and PAMT,and reducing the proportion of fully muscularized vessels and the area of collagen deposition than those of CDDO-Me(21 μg/kg). Conclusion:Inhalation of CDDO-NO can ameliorate MCT-induced PAH in rats by inhibiting pulmonary vascular remodeling,and the effect of CDDO-NO is superior to parent compound CDDO-Me. These results suggest that CDDO-NO could be a promising drug for the treatment of PAH.

    • Antinociceptive effect and mechanism of growth differentiation factor 15 on formalin ⁃ induced pain in mice

      Abstract:Objective:The current study intended to examine the antinociceptive effect of growth differentiation factor 15(GDF-15) and to elucidate its possible mechanism of action. Methods:Mice formalin-induced pain model with intrathecal administration of GDF - 15(50 ng)and intraperitoneal administration of GDF - 15(10,30,100 μg/kg)was used in this study to examine the antinociceptive effect of GDF-15. The expression of glial cell-line derived neurotrophic factor receptor alpha-like(GFRAL)which is considered to be the specific receptor of GDF -15 was assessed using Western blot and immunofluorescence techniques. The expression of interleukin (IL)-6,IL-1β,tumor necrosis factor(TNF)-α,prodynorphin(PDYN)and proenkephalin(PENK)in the formalin micespinal cords was also evaluated. Results:Intrathecal administration of GDF -15(50 ng)showed a significant antinociceptive effect informalin-induced tonic pain(p<0.05)but not in formalin-induced acute pain. Intraperitoneal administration of GDF-15(10,30,100 μg/kg)produced a dose - dependent antinociceptive effect in mice formalin -induced tonic pain. GFRAL was expressed in brain,cerebellum and spinal cord,and only in neurons of the spinal cord dorsal horn. Moreover,intrathecal injection of GDF -15 inhibited the expression of pro - inflammatory mediators IL -6(p<0.05)and TNF -α(p<0.05)in the spinal cord of formalin mice. Conclusion:GDF -15 exhibits asignificant antinociceptive effect in formalin -induced inflammatory hyperalgesia and its possible mechanism may be by reducing the expression of pro-inflammatory factors IL-6 and TNF-α.

    • Preliminary study of a BMP ⁃ 2 releasing scaffold comprised of PLGA microspheres for guiding bone regeneration

      Abstract:Objective:This study invented a coaxial dual-channel micro-injection technique to prepare the poly(lactic-co-glycolic acid)(PLGA)microspheres for the first time,which can be used for guiding bone regeneration. Methods:A dual-channel microsyringe was used to prepare PLGA microspheres encapsulated bone morphogenetic protein 2(BMP-2). An optical microscope,a fluorescence microscopy,a scanning electron microscopy(SEM)were used to observe morphology and structure of the microspheres. The changes of morphology and mass loss of the microspheres soaked in PBS solution were recorded for degradation. Cytocompatibility of the microspheres was detected by CCK -8 and cell Calcein/PI staining kit. The effect of PLGA microspheres loading with BMP - 2 on the chemotaxis of MC3T3-E1 cell in 24 hours was detected by Transwell cell migration assay. Results:The microspheres in this study showed regular mono cavity. The mean diameter of the microspheres was 167.58 μm. Two different mean thickness of microsphere shell [(23.82±7.10)μm and(35.31±5.55)μm]can be acquired. The microspheres degraded gradually in 2 months in PBS solution. The results of CCK -8 and ceu Calcein/PI staining test showed cells remained activity with PLGA microspheres. Transwell cell migration assay indicated PLGA microspheres loading with BMP-2 had chemotactic effect on cells in 24 hours. Conclusion:The microspheres in this study with releasing rate of BMP-2 controlled by the thickness of microspheres shell and its surface adhered by cells easily,have apotential of guiding bone regeneration.

      Abstract:Objective:This study aims to investigate the diagnostic efficacy of targeted magnetic resonance SPIO - PLA - P53 molecular probe in rat glioma. Methods:Superparamagnetic iron oxide nanoparticles SPIO -PLA and glioma targeting nanomolecular probe SPIO-PLA-P53 were synthesized. The physical properties of molecular probes were detected by transmission electron microscopy and dynamic light scattering. Cell activity was detected by Cell Counting Kit - 8(CCK - 8)method. The penetration efficiency of molecular probe through blood-brain barrier in vitro was detected. The distribution of nanoprobes in glioma was detected by 7.0 T small animal - special magnetic resonance instrument and matching coil. Pathological staining of tumor - bearing mouse brain tissue was performed to clarify the accumulation of targeted molecular probes in the tumor tissue. Statistical analysis of the results was performed using GraphPad Prism software. Results:Glioma targeted SPIO - PLA - P53 nanoparticles have uniform size,good dispersion,good biocompatibility and targeted aggregation effect. Magnetic resonance scanning showed that the tumor T2WI signal was decreased after injection of targeted SPIO - PLA - P53 probe in the tumor - bearing rats,and there was no significant change in the tumor signal after injection of SPIO-PLA probe solution. The pathological results confirmed that a large amount of iron ions accumulated in the glioma of rat injected with SPIO -PLA -P53. Conclusion:SPIO -PLA -P53 can be used as an magnetic resonance molecular probe for specific imaging of rat gliomas.

    • Effects of scopoplamine on boundary coding of border cells in mice

      Abstract:Objective:This study aims to investigate how scopolamine affects on boundary coding of border cells and spatial memory of mice. Methods:Healthy male 2-month-old C57BL/6 mice were randomly divided into saline control group and scopolamine group. Morris water maze was used to evaluate spatial memory ability. Then 32 channel microwire electrodes were implanted into retrosplenial cortex(RSC). After 7 days of recovery,the open field experiment was carried out and the electrical signals were recorded at the same time. Border cells were isolated after analysis. Then 25 μg/kg scopolamine or equivalent volume of normal saline was intraperitoneally injected. After 24 hours,the open field experiment was carried out and the electrical signals were recorded at the same time. The changes of border cells recorded before and after treatment were analyzed. Results:There were significant differences in the learning curve of water maze experiment between saline control group and scopolamine group(P < 0.05),the time in the target quadrant(P < 0.05)and the number of crossing platform(P < 0.05). After intraperitoneal injection of scopolamine or normal saline for 24 hours,the total distance of mice in open field had no significant difference(P > 0.05). Injection of scopolamine had no significant effect on the firing frequency of border cells in mice(P > 0.05). The earth mover’s distance(EMD)was significantly increased after scopolamine injection(P < 0.05),while normal saline injection had no significant effect on EMD(P > 0.05). Conclusion:Scopolamine may affect the ability of spatial memory by affecting boundary coding of border cells in mice.

      Abstract:Objective:This study aims to to develop rapid detection methods and establish a new method for a common foodborne pathogenic microorganism enterohemorrhagic E.coli(EHEC)O157:H7 detection. Methods:Two EHEC O157:H7 detection methods were established and optimized based on the conserved region sequence of EHEC O157:H7 combined with recombinase-aided amplification(RAA)and fluorescence quantitative method or lateral flow dipstick(LFD),then the sensitivity and specificity of the two methods(RAA fluorescence method and RAA-LFD method)were evaluated. Results:Both detection methods can be completed within 16 min at 39 ℃,and can detect 1 × 104 copies,showing high sensitivity. The two methods did not cross react with plasmid templates of other common intestinal pathogens(Staphylococcus aureus,Salmonella,Campylobacter,Yersinia,Shigella)and showed good specificity. Conclusion:The two EHEC O157:H7 detection methods established in this study have short detection time,good specificity,sensitivity and repeatability,and can be used for rapid detection of EHEC O157:H7.

      Abstract:Objective:A high performance liquid chromatography mass spectrometry (LC-MS) method was established to simultaneously quantify the lenalidomide,vorinostat and dexamethasone in rat plasma. The method was successfully applied to the plasma pharmacokinetics of lenalidomide,vorinostat and dexamethasone after oral administration to rats. Methods:The LC column was Agilent Zorbax Eclipse Plus C18(100 mm ×2.1 mm,1.8 μm). The mobile phase was comprised of acetonitrile and water containing 0.1% formic acid+5 mmol/L ammonium acetate. The gradient profile was used. Flow rate was 0.6 mL/min. Column temperature was 35 ℃. Sample injection volume was 5 μL. The quantification was obtained by scanning with multiple reaction monitoring(MRM)via the positive electrospray ionization(ESI + ) source. Results:The correlation coefficients of the lenalidomide,vorinostat and dexamethasone,respectively were over 0.995. The relative standard deviation(RSD)values of the inter- and intra- precisions were less than 7.4%. The accuracy ranged from 94.7% to 106.7%. The matrix effect values were sufficiently minimized to 92.3%~109.7%. The average recoveries from rat plasma were 87.4% to 103.6%. Conclusion:The validated method is further applied to the determination of lenalidomide,vorinostat and dexamethasone in plasma. The method is rapid and economical,and can provide helpful information for pharmaceutical analysis.

    • Changes and clinical significance of concomitant gene mutation/amplification in patients with lung cancer

      Abstract:Objective:This study aims to observe the changes of the concomitant gene mutation/amplification in patients with lung cancer based on next generation sequencing(NGS)technology and analyze its clinical significance. Methods:Retrospective analysis was performed on 71 lung cancer patients(132 samples)detected by NGS technology in the Second Affiliated Hospital of Soochow University from December 2015 to July 2020,including 67 blood samples,48 tumor tissues,15 pleural effusion and 2 cerebrospinal fluid. GraphPad prism7.0 statistical software was used for statistical analysis of concomitant gene mutation/amplification in different clinical samples. Results:In addition to EGFR mutation,mutations/amplification of the first 5 genes were:tumor suppressor protein p53(TP53),Kirsten rat sarcoma viral oncogene(KRAS),human epithelial growth factor receptor 2(ERBB2),retinoblastoma1(RB1)and hepatocyte growth factor receptor(MET). A total of 50 newly diagnosed patients’blood samples were detected by NGS technology. The results showed that EGFR positive rate was 34%(17/50). Except EGFR mutation,there were 79 kinds of gene mutations/ amplifications detected,with an average of 2.46 times/example,but without statistical difference in concomitant gene mutation/ amplification among clinical stage,smoking status,age,gender and pathological classification groups in blood samples. A total of 46 newly diagnosed patients’tissue samples were detected by NGS technology,of which EGFR positive rate was 50%(23/46). Except EGFR mutation,there were 160 kinds of gene mutations/amplifications detected,with an average of 6.20 times/example. There was no statistical difference in concomitant gene mutation/amplification between clinical stage and age groups in tissue samples(p>0.05). But in terms of smoking status,gender and pathological classification groups,there were statistically differences(U=74.000,p<0.001; U=130.5,P=0.003;F=8.968,P=0.011). At the same time,TP53 mutation or not had no statistical significance on survival rate(the blood group:χ2 =0.321,P=0.571;the tissue group:χ2 =0.309,P=0.579). For the same patients,the frequency of gene mutation/ amplification in the tissue group([3(1,8)])was higher than that in the blood group[3(0,1)],and the differences were statistically significant(W=-150,P =0.001). In the 17 patients with dynamic monitoring,there was no statistically significant difference in the frequency of concomitant gene mutation/amplification between the two groups before and after anti-tumor treatment(W=-3,P=0.916). Conclusion:There may be multiple parallel gene mutations/amplifications before the first - line treatment for lung cancer. Previous smoking and male patients are associated with higher levels of mutation/amplification status. Changes in the frequency of gene mutation/amplification cannot indicate disease progression,and TP53 mutation has no significant effect on the survival rate of patients.

    • Correlation between peripheral blood D ⁃dimer and deep vein thrombosis in patients with early gastric cancer and premalignant lesions underwent ESD

      Abstract:Objective:This study aims to investigate the relationship between D-dimer in different periods and postoperative deep venous thrombosis(DVT)in patients with early gastric cancer and premalignant lesions underwent endoscopic submucosal dissection (ESD). Methods:A retrospective cohort study was carried out,176 patients with early gastric cancer were collected,including 117 men with an average age of(62.9±9.7)years. According to the results of vascular ultrasound examination of the lower extremities at 3 days after surgery,the patients were divided into two groups:venous thrombosis group(12 people)and non -venous thrombosis group (164 people). General clinical data of patients were collected. Postoperative DVT risk score was calculated by Padua scale. The blood D -dimer level of all patients were measured 1 day before operation,immediately after operation and 1 day after operation. The ROC curve and area under curve(AUC)of multiple variables were analyzed to predict DVT after ESD. The relationship between D -dimer level and DVT after ESD was analyzed by multiple regression model. Results:There were 176 patients underwent vascular ultrasound examination of lower extremities 3 days after surgery,and 12 patients(6.82%)had lower extremity DVT. There was statistical significance in D-dimer levels between the two groups before ESD and 1 day after ESD(all P < 0.05). The sensitivity,specificity and AUC of D - dimer 1 day after ESD to predicted DVT were 91.7%,67.1%,0.841(95% CI:0.778~0.892). Multivariate regression analysis showed that the level of D-dimer 1 day after ESD was significantly correlated with the increased risk of DVT after ESD(OR:82.756,95% CI:6.000~1 141.349,P < 0.001). Conclusion:The increased level of D - dimer 1 day after ESD is associated with an increased risk of DVT in patients with early gastric cancer and premalignant lesions underwent ESD.

    • The value of combined detection of lactate dehydrogenase and albumin ratio in peripheral blood before and after operation in evaluating the prognosis of colon cancer

      Abstract:Objective:This study aims to investigate the value of preoperative and postoperative changes in lactate dehydrogenase (LDH)to albumin ratio(LDH/albumin ratio,LAR)in assessing the prognosis of colon cancer. Methods:The clinical data of 134 colon cancer patients who underwent gastroenterology surgery at Jiangnan University Hospital from Jan. 2013 to Dec. 2016 were collected. Patients’clinical pathological characteristics were collected,including gender,age,TNM staging,etc. to calculate the LAR values before and within one week after surgery and to determine the optimal cut-off value by using the receiver operating characteristic curve (ROC). Kaplan-meier method of survival analysis was used to draw the survival curve,Log-rank test to perform univariate analysis and Cox multivariate regression analysis to conduct multivariate analysis. Results:According to the optimal cut-off values of preoperative and postoperative LAR,134 patients were divided into preoperative high-LAR group,preoperative low-LAR group,postoperative high- LAR group and postoperative low-LAR group. The 5-year overall survival(OS)rate was 42.9% in the preoperative high-LAR group and 84.9% in the preoperative low -LAR group,and the 5-year progression -free survival(PFS)rate was respectively 45.5% and 82.6%;while the 5-year OS rate was 62.5% in the postoperative high-LAR group and 81.9% in the postoperative low LAR group,and the 5- year PFS rate was 70.0% and 77.3%,respectively. Based on the joint analysis of preoperative and postoperative LAR,134 patients were further divided into 3 groups:LAR1 group(both preoperative and postoperative high LAR),LAR2 group(either preoperative or postoperative high LAR),and LAR3 group(both preoperative and postoperative low LAR). Univariate and multivariate analysis suggested that the 5- year OS rate of LAR1 group,LAR2 group and LAR3 group was 33.3%,78.1% and 84.5%,respectively,with statistically significant differences(P < 0.01). Lymph node metastasis was significantly correlated with patients’OS and PFS(P < 0.01),and clinical staging was significantly correlated with PFS(P < 0.01). Conclusion:Joint detection of dynamic changes of preoperative and postoperative LAR can help determine the prognosis of colon cancer and guide further individualized treatment. Preoperative and postoperative high LAR is an independent factor of poor prognosis for OS in colon cancer patients.

      Abstract:Objective:This study aims to explore the association between neutrophil - to - lymphocyte ratio(NLR),platelet - to - lymphocyte ratio(PLR)and progress after androgen deprivation therapy in patients with prostate cancer(PC). Methods:Total 170 PC patients treated in Sanya Central Hospital(the Third People’s Hospital of Hainan Province)from May 2017 to May 2020 were employed as study subjects. All patients received androgen deprivation therapy and follow -up for 1 year. Patients who progressed to castration - resistant prostate cancer(CRPC)during follow - up were included in the progress group,and the rest were included in the stable group. Examination results such as general data,NLR,PLR and monocyte to lymphocyte ratio(MLR)of two groups were comparatively analyzed,analyzing the factors associated with progressing to CRPC by multivariate logistic regression analysis, constructing receiver operating characteristic(ROC)curve to assess prediction efficiency of NLR and PLR for predicting CRPC within 1 year. Results:At the end of follow-up,14 patients who were lost to follow-up and 2 patients who died of other causes were excluded,44 of the remaining 154 cases were included in the progress group,and 110 cases were included in the stable group. There were statistical differences between two groups in the distribution of the number of Gleason score,T stage and N stage(P < 0.05). NLR and PLR in the progress group were higher than those in the stable group(P < 0.05). There were statistical differences between two groups in the distribution of the number of prostate specific antigen(PSA)level and its nadir(PSA nadir)(P < 0.05). The 1-year mortality was lower in the low NLR group than in the high NLR group(log rank χ2 =5.094,P=0.024);1-year mortality was lower in the low PLR group than in the high PLR group(log rank χ2 =9.931,P=0.002);Gleason score≥8 points(OR=3.998,P=0.034),PLR(OR=1.156,P < 0.001),NLR(OR=13.844,P < 0.001)and T4 stage(OR=16.252,P=0.048)were the risk factors of progressing to CRPC. AUC of NLR and PLR for predicting CRPC within 1 year were 0.845 and 0.865. There was no statistically significant difference between the area under the curve(AUC)of both(P > 0.05). Conclusion:Baseline NLR and PLR in patients with PC correlate with the occurrence of CRPC,and both may be effective measures to predict CRPC in the future.

    • Prognostic value of CCL2 level in bronchoalveolar lavage fluid of patients with idiopathic pulmonary fibrosis

      Abstract:Objective:This study aims to explore the prognostic value of chemokine CCL2(C -C motif)in bronchoalveolar lavage fluid(BALF)and gender-age-physiological index(GAP)scores for idiopathic pulmonary fibrosis(IPF)in clinical practice and the correlation between them. Methods:Total 35 patients with IPF hospitalized in Nanjing Gulou Hospital from January 2017 to December 2019 were analyzed retrospectively. In the same period,10 outpatients with chronic cough(CC)but no other lung diseases or other organ diseases were collected. The differences of gender,age,percentage of predicted forced vital capacity(FVC%),percentage of predicted diffusing capacity for carbon monoxide(DLCO%)(except CC group),blood gas analysis and other indicators between the two groups were compared. At the same time,the level of CCL2 in serum and BALF was detected,and the differences of CCL2 level and survival time between different GAP groups and the correlation between CCL2 and GAP score were analyzed. Results:Compared with CC group,CCL2 in serum and BALF increased in IPF group. The comparison between IPF group(phase Ⅰ,Ⅱ and Ⅲ)and CC group showed that there were differences in oxygenation index,FVC%,CCL2 level in serum and BALF,and there were significant differences in DLCO% among IPF group(phase Ⅰ,Ⅱ and Ⅲ). In terms of correlation,CCL2 in BALF was negatively correlated with FVC% or DLCO%,and positively correlated with GAP score. The survival time of IPF group(stage Ⅰ,Ⅱ and Ⅲ)was different among the three groups. Conclusion:It is further verified in this stndy that CCL2 and GAP scores in BALF can predict the prognosis of IPF. At the same time,it is the first time to explore the differences of CCL2 level in BALF in different GAP stages,and there is a correlation between CCL2 and GAP scores in BALF.

      Abstract:Objective:This study aims to evaluate the diagnostic value of peripheral blood circ_0070934/miR-199a-5p/MGAT3 in combined allergic rhinitis and asthma syndrome(CARAS). Methods:Peripheral venous blood was collected from 38 CARAS patients and 43 healthy controls. Plasma MGAT3 protein level was detected by ELISA,and the expression level of circ_0070934/miR-199a-5p/ MGAT3 in peripheral blood was detected by quantitative real - time PCR(qRT - PCR). Diagnostic sensitivity and specificity were determined by ROC curve. Results:ELISA results showed that MGAT3 concentration in CARAS patients was significantly lower than that in control group(P=0.035). The circ_0070934(P=0.001)and MGAT3 mRNA expressions(P < 0.001)in CARAS patients were down-regulated when compared with control group,while miR-199a-5p was up-regulated(P=0.013). Correlation analysis showed that MGAT3 was positively correlated with the count of eosinophils and the percentage of eosinophils,while circ_0070934 showed a negative correlation. ROC curve analysis indicated that MGAT3 was the best parameter. When the cut - off value was 0.208,the sensitivity and specificity were 73.7% and 100.0%,respectively,and the AUC was 0.933(95% CI:0.882~0.984,P < 0.001). The combination of circ_0070934,miR -199a -5p and MGAT3 had the highest diagnostic value for CARAS. When the cut -off value was 0.539,the sensitivity was 92.1%,the specificity was 93.0%,and the AUC was 0.959(95%CI:0.916~1.000,P < 0.001). Conclusion: Altered expressions of circ_0070934/miR-199a-5p/MGAT3 were confirmed in the peripheral blood of CARAS patients,which can be used as a biomarker for the diagnosis of CARAS. Further researches on the specific role and mechanism of circ_0070934/miR-199a-5p/ MGAT3 in CARAS will help to discover new diagnostic and therapeutic targets of CARAS.

    • Detection rate of thyroid nodules and analysis of influencing factors in 6 096 cases of teacher population in healthy examination

      Abstract:Objective:This study aims to investigate the detection rate and influencing factors of thyroid nodules(TN)among healthy teachers. Methods:The physical examination items(including gender,age,occupation,height,weight,etc.),laboratory examination(fasting blood glucose,blood lipid,liver function,uric acid,13C breath test,etc.)and thyroid color ultrasound examination of health teachers in Sir Run Run Hospital Affiliated to Nanjing Medical University from October 2018 to June 2021 were retrospectively analyzed. Logistic regression was used to analyze the influencing factors of TN. Results:①Thyroid nodules were found in 2 539 of 6 096 patients,with a total detection rate of 41.65%,including 38.20% male and 43.03% female. There were 1 340 single nodules(21.98%)and 1 199 multiple nodules(19.67%). For TN classification according to TI-RADS,grade 3 was the most,accounting for 22.74%. The older the age,the higher the detection rate was,and there were significant differences in the detection rate of TN among different age groups(P < 0.05). In addition,thyroid nodules were detected in bilateral lobe(17.27%)> right lobe(13.27%)> left lobe(10.89%)> thyroid isthmus(0.21%). ② The detection rate of female teachers in the middle school group was the highest(49.44%),and there were significant differences in the detection rate of different types of teachers in the three age groups of < 30 years old,30~39 years old and ≥60 years old(P < 0.05);③Compared with the non - TN group,the incidence of hypertension,abnormal fasting glucose,hyperlipidemia,overweight and obesity was higher in the TN group,and the differences were statistically significant between two groups(P < 0.05). ④Logistic multifactor regression analysis showed that female was an independent risk factor for TN (OR=1.913,P < 0.05),and teachers aged 40-49,50-59 and ≥60 had a higher risk of TN than those aged less than 30(OR=1.402, 2.923,4.637,All P < 0.05),and compared with preschool education group,college teachers had a lower risk of TN(OR=0.718,P < 0.05). Conclusion:The detection rate of TN among teachers who underwent physical examination is high,but the malignancy is not high. Overweight,obesity and“three high”are closely related to TN. Female,age and middle school teachers are risk factors for TN,so comprehensive attention should be paid to the above indicators to improve health management awareness.

    • Accuracy of 3° external rotation of femoral component to the posterior condylar axis in TKA

      Abstract:Objective:This study aims to provide references on the rotation of the femoral component during total knee arthroplasty (TKA)by measuring the rotation degree of the surgical transepicondylar axis and the posterior condylar axis of the femoral condyles on CT images. Methods:The X - ray and lower extremity CT imaging data of 283 patients who underwent TKA in the Department of Bone and Joint Surgery,the First Affiliated Hospital of Nanjing Medical University from October 2018 to March 2022 were retrospected in our study. One resident and one senior chief physician of joint surgery measured the angle between the surgical transepicondylar axis (sTEA)and the posterior condylar axis(PCA)by the imaging system,respectively. The angle between sTEA and PCA and the angle between femoral component and sTEA after routine placement of the femoral component with reference to 3° external to the PCA was calculated. Results:The sTEA was on average 1.63° externally rotated to the PCA(range 3.79° internal to 7.87° external). When the femoral component was routinely placed at 3°external to PCA,only 163(57.6%)patients had femoral component rotation within ± 2 ° of sTEA after TKA,of which 68(24.0%)patients had femoral component internal rotation to sTEA. Conclusion:There were extensive differences in the rotation of the distal femur in patients with osteoarthritis. Routinely setting femoral rotation 3° external to the PCA would cause significant position errors in femoral component,affecting the prognosis of TKA. Joint surgeons should individualize the rotational alignment of the femoral component during TKA based on preoperative imaging data.

    • Trend analysis of tuberculosis incidence and mortality in Jiangsu Province from 2005 to 2020

      Abstract:Objective:This study aims to analyze the trends of tuberculosis incidence and mortality in Jiangsu Province from 2005 to 2020,and to explore the effects of age,period and cohort on them,so as to provide a scientific basis for better control of tuberculosis incidence and mortality in the later period. Methods:Joinpoint regression was used to analyze the incidence and mortality of tuberculosis in Jiangsu province from 2005 to 2020,annual percent change(APC)and average annual percent change(AAPC)were calculated. The data for people aged 0~89 years old were statistically analyzed by using the R-based age-period-cohort model web tool to estimate effect values. Results:There was a monotonic down ward trend in age-adjusted incidence for males,females and the total population(APC=AAPC=-7.3%,P < 0.001;APC=AAPC=-6.8%,P < 0.001;APC=AAPC=-7.1%,P < 0.001)and an overall down ward trend in age-adjusted mortality formales,females and the total population(APC=AAPC=-10.4%,P < 0.001;AAPC=-17.2%,P < 0.001;APC=AAPC=-10.0%,P < 0.001),with the population incidence rate reaching higher values at the age of 20-24 and 70-74,and the mortality rate reaching its highest value at the age of 80~84. With the risk of incidence and mortality decreasing overtime,the risk of incidence and mortality in the population gradually decreased as the birth cohort moved backwards. Conclusion:Focusing on the impact of age on tuberculosis,males,people aged 20-24 years and 70-74 years are populations with risk of tuberculosis incidence and people aged 80-84 years are populations with risk of tuberculosis mortality.

      Abstract:Mitochondrial dysfunction is one of the most important mechanisms in developing cardiovascular diseases. With the deep study of mitochondrial function,it has been found that the function of mitochondrial ion channels are crucial to mitochondria. This review aims to illustrate the mitochondrial ion channels associated with cardiovascular disease,and to elucidate their pathophysiological roles in different cardiovascular diseases,such as ischemia - reperfusion injury,arrhythmia,heart failure,diabetic cardiomyopathy,and pulmonary hypertension. In recent years,the research and development of mitochondria targeting drugs has become an indispensable field in medical pharmacology. Therefore,we summarized widely used mitochondrial ion channel activators or inhibitors with high specificity that can regulate cardiovascular function,providing new ideas for the treatment and prevention of cardiovascular diseases.

      Abstract:Thyroid - associated ophthalmopathy(TAO)is a vision - threatening autoimmune disease. Patients usually suffer from proptosis,diplopia or strabismus. In severe cases,the optic nerve may be compressed,resulting in visual impairment or even blindness. Serum immunoglobulin G4(IgG4)is a specific antibody that is associated with a variety of autoimmune diseases. Current studies have found that some TAO patients have elevated serum IgG4 levels,and these patients tend to have more severe symptoms. In the occurrence and development of ophthalmopathy,IgG4 may induce orbital fibroblasts to generate hyaluronic acid and promote the production of autoantibodies. IgG4 may play a role in the pathogenesis of TAO alone or in combination with thyrotropin receptor antibodies. Studies have shown that serum IgG4 is related to the clinical activity and severity of TAO. High IgG4 may indicate that TAO is in the early stage of inflammatory activity,and timely intervention can slow down the progression of muscle lesions and orbital fibrosis. Therefore,this article reviewed the relevant literature to explore the relationship between serum IgG4 and thyroid-associated ophthalmopathy,with a view to earlier and more effective clinical management of patients with TAO.

      Abstract:Parkinson’s disease(PD)is a neurodegenerative disorder characterized by progressive loss of dopaminergic neurons(DA) in the substantia nigra pars compacta,accompanied by an accumulation of Lewy bodies and activation of neuroglial cells. Current studies on the correlation between lipid metabolism and PD suggest that different kinds of lipid components play an important role in the progression of PD. However,as it was influenced by a variety of factors(age,gender or etiology)in clinical,it is difficult to obtain a definitive association between lipids and the development of PD. This article aims to review the progress of lipid components in PD, deepen the understanding of the pathology and pathogenesis of PD,and provide new potential targets for the treatment of PD.
