    • Construction and characterization of nanomicelle carriers composed of fluorinated polyethyleneimine derivatives and their role in delivery across the blood⁃brain barrier

      Abstract:Objective:To investigate the construction,characterization,and role of nanomicelles composed of fluorinated polyethyleneimine(PEI)derivatives in gene delivery across the blood-brain barrier(BBB). Methods:PEI-heptafluorobutyric anhydride (HFAA)was synthesized through a chemical reaction between PEI and HFAA,followed by amide reaction with sinapic acid(SA)to obtain PEI-HFAA-SA(referred as SPF). Finally,PEI-HFAA-SA@PS80(referred as SPFT)was obtained by encapsulating polysorbitol 80(PS80)within SPF. The molecular bonds and elemental composition of SPFT were analyzed using Fourier transformation infrared absorption spectroscopy,fluorine nuclear magnetic resonance(NMR),and hydrogen NMR spectroscopy. The hydrodynamic particle size,plasmid adsorption and protection capacity,stability and morphology of the carrier-plasmid complex were further characterized by dynamic light scattering experiments,agarose coagulation experiments and scanning electron microscopy observations,respectively. The gene transfection efficiency and cytotoxicity of SPFT were investigated in mouse glioma cell line Neuro 2a. SPFT carrying green fluoresant protein expression plasmid was injected into C57BL/6J mice by tail vein to observe its distribution in brain tissues and the effect of gene transfection within the BBB. Results:SA and HFAA were modified to synthesize SFPT,which was then wrapped in PS80. SPFT had a hydrodynamic particle size of 100 to 200 nm while exhibiting significant loading capacity for plasmids along with effective protection against degradation. In vitro experiments revealed that SPFT possessed excellent transfection ability and biocompatibility. In vivo experiments showed that SPFT accumulated in the brain successfully and crossed the BBB to deliver the gene effectively after tail vein injection into mice. Conclusion:SPFT exhibits a good biocompatibility and demonstrates an efficient gene delivery across the BBB,presenting a novel approach for drug administration in neurological disorders.

      Abstract:Objective:To explore whether atorvastatin(Ator)can induce ferroptosis in pancreatic β- cell line MIN6 cells and its possible mechanism. Methods:MIN6 cells were divided into control group,Ator group,Ator+apoptosis inhibitor(Z-VAD-FMK)group, Ator+necrostatin-1(Nec-1)group and Ator+ferrostatin-1(Fer-1)group. Cell viability was detected by cell counting kit-8(CCK-8) method. The ultrastructure of cells was observed by transmission electron microscopy. The levels of reactive oxygen species(ROS)and Fe2+ were observed by fluorescence microscopy. The contents of malondialdehyde(MDA)and glutathione(GSH)were detected by enzyme-linked immunosorbent assay(ELISA)method. Real-time quantitative PCR was used to detect the mRNA levels of caspase-3, receptor-interacting protein kinase 3(Ripk3),acyl-CoA synthetase long-chain family member 4(Acsl4),prostaglandin endoperoxidase synthase 2(Ptgs2)and glutathione peroxidase 4(Gpx4). Western blot was used to detect the proteins levels of 4-hydroxynonenal (4-HNE)and GPX4. Results:Compared with the Ator group,cell viability of MIN6 was higher in Ator+Z-VAD-FMK group and Ator+ Fer -1 group(P < 0.01). MIN6 cells,which were treated with Ator,exhibited the characteristic morphologic features associated with apoptosis,ferroptosis and autophagy under transmission electron microscopy. Compared with the control group,the levels of the intracellular Fe2+ ,MDA and ROS in the Ator group were increased and GSH was decreased. The mRNA relative expression levels of caspase-3,Acsl4 and Ptgs2 were increased,as well as the protein relative expression level of 4-HNE(all P < 0.05). The mRNA and protein relative expression levels of GPX4 were decreased(P < 0.05). Compared with the Ator group,the levels of the intracellular Fe2+ ,MDA and ROS in the Ator+Fer-1 group were decreased and GSH was increased. The mRNA relative expression level of Acsl4 was decreased and Gpx4 was increased(all P < 0.05). The protein relative expression levels of 4-HNE was decreased and GPX4 was increased,though the changes were not statistically significant. Conclusion:Atorvastatin may induce ferroptosis in MIN6 cells by down-regulating GPX4 expression through inhibiting mevalonate pathway.

    • Effects of CPO ⁃ PCL microparticles on the proliferation and osteogenic differentiation of adipose⁃derived mesenchymal stem cells in vitro under hypoxia

      Abstract:Objective:To investigate the effects of calcium peroxide(CPO)-loaded polycaprolactone(PCL)microparticles(CPO-PCL)on the proliferation and bone differentiation of adipose - derived mesenchymal stem cells(ADMSCs)in vitro under hypoxia. Methods:Rat ADMSCs were extracted and added with the prepared CPO-PCL particles for normal or osteogenic differentiation culture under hypoxia/normoxia environment. On the 7th and 14th days,cell proliferation,alkaline phosphatase(ALP)level,the calcification of nodulation,and the fluorescence intensity of Runt-related transcription factor 2(RUNX2),Osteocalcin and Osteopontin were examined by the MTT assay,alkaline phosphatase(ALP)kit,alizarine red staining and immunofluorescence staining,respectively. Results: Under hypoxia and osteogenic differentiation culture conditions,1.00% CPO - PCL microparticles significantly promoted ADMSCs proliferation(P <0.05),and CPO-PCL microparticles of 0.50% and 1.00% concentrations notably increased the production of ALP and calcium nodules(P <0.001),while enhancing the expressions of RUNX2,Osteocalcin and Osteopontin proteins(P <0.05). Under hypoxia and normal culture conditions,1.00% CPO - PCL microparticles elevated ALP levels,increased the expression levels of RUNX2,Osteocalcin and Osteopontin(P <0.05). Under normoxia and differentiation culture conditions,1.00% CPO - PCL microparticles increased the production of ALP and calcium nodules,and CPO-PCL microparticles of 0.50% and 1.00% concentrations promoted the expression levels of RUNX2 and Osteocalcin proteins(P <0.001). Under normoxia and normal culture conditions,0.50% and 1.00% CPO-PCL microparticles up-regulated RUNX2,Osteocalcin and Osteopontin expression levels(P <0.05),but rarely affect osteoblast differentiation. Conclusion:CPO - PCL microparticles of 1.00% promote the proliferation and bone differentiation of ADMSCs in vitro under hypoxia and osteogenic differentiation culture.

      Abstract:Objective:To analyze the expression of malic enzyme 3(ME3)in hepatocellular carcinoma(HCC)tissues and celllines, and explore the effects of ME3 on the proliferation,migration,and invasion of HCC cells,as well as its underlying mechanism. Methods:The Cancer Genome Atlas(TCGA)database was used to study the expression of ME3 in HCC tissues. RT - qPCR and Western blot were performed to detect the mRNA and protein expression levels of ME3 in HCC cells. After knocking down or over expressing ME3 in HCC cell lines,the effects of ME3 on the proliferation,migration and invasion of HCC cells were detected by CCK-8 assay,colony formation assay,transwell assay,and invasion assay. Western blot was used to detect the expression of proteins related to the PI3K/AKT signaling pathway. Results:Compared with adjacent normal tissues and normal liver celline,ME3 was significantly up-regulated in HCC tissues and cell lines. Knockdown of ME3 markedly inhibited the ability of proliferation,migration and invasion of HCC cells,while overexpression of ME3 resulted in the opposite effects. Furthermore,we found that the activity of PI3K/AKT signaling pathway was significantly activated by ME3. Conclusion:ME3 is highly expressed in HCC tissues,promoting the proliferation, migration and invasion of HCC cells,and may function as an oncogene in HCC by activating the PI3K/AKT signaling pathway.

      Abstract:Objective:To analyze the clinical efficacy of low temperature plasma radiofrequency ablation of Krouse T2 and T3 nasal inverted papilloma(NIP)under nasal endoscope. Methods:Clinical data of 141 cases with Krouse T2 and T3 NIP treated with endoscopic surgery at the Department of Otolaryngology Head and Neck Surgery,the Affiliated Nanjing Hospital of Nanjing Medical University(Nanjing First Hospital)from October 2012 to October 2022,were retrospectively analyzed,and the tumor was resected and treated with or wthout low temperature plasma radiofrequency ablation during the operation. They were divided into observation group using low temperature plasma radiofrequency ablation and control group without low temperature plasma radiofrequency ablation. The operative time,intraoperative blood loss,average length of hospital stay,postoperative visual analogue scale(VAS)score of pain, postoperative complications and postoperative recurrence rate were compared between the two groups. Results:The average operation time of NIP patients with T2+T3 and T3 stage in the observation group was less than that of the corresponding control group,but the difference was not statistically significant(P > 0.05);the average operation time of NIP patients with T2 stage in the observation group was less than that of the corresponding control group,and the difference was statistically significant(P < 0.05). The average intraoperative blood loss,the average length of hospitalization,and postoperative pain VAS scores of patients with T2+T3,T2 and T3 stage in the observation group were lower than those in the corresponding control group,and the difference was statistically significant (P < 0.05). There was no statistical significance in the postoperative complications of patients with T2+T3,T2 and T3 stage in the observation group compared with the corresponding control group(P > 0.05). The postoperative recurrence rate of patients with T2+T3 and T3 stage in the observation group was lower than that of the control group,with statistical significance(P < 0.05),while the postoperative recurrence rate of patients with T2 stage in the observation group was lower than that of the control group,with no statistical significance(P > 0.05). Conclusion:Low temperature plasma radiofrequency ablation under nasal endoscope can treat Krouse T2 and T3 stage NIP and its base under a clear surgical field of view. It has the advantages of less intraoperative blood loss, shorter length of hospitalization,less pain and lower recurrence rate,compared with routine operation. And it has satisfactory clinical efficacy,which is worthy of popularization and application.

    • A comparative study of ultrasound and genetic prenatal diagnosis in 106 NT⁃thickened fetuses

      Abstract:Objective:To explore the clinical value of prenatal ultrasound in suggesting thickening of the nuchal translucency(NT) in fetuses and to compare it with the results of genetic prenatal diagnosis. Methods:The results of ultrasound and genetic prenatal diagnosis of 106 fetuses with thickening of the NT were compared and analyzed(all fetuses were subjected to chromosomal microarray analysis,with whole - exome sequencing in four cases and SMN1 gene testing in one case),and pregnancy outcomes were followed. Results:A total of 44 cases(41.51%)of genetic abnormalities and 2 cases of copy number variants of unknown clinical significance were identified. These genetic abnormalities included 39 cases of chromosomal abnormalities(34 cases of aneuploidy abnormalities and 5 cases of pathogenic copy number variants)and 5 cases of genetic abnormalities(all pathogenic or suspected pathogenic variants). The incidence of genetic abnormalities increased significantly with increasing thickness of the NT. In addition,38(86.36%)of 44 fetuses with genetic abnormalities were combined with other ultrasound abnormalities,with the highest percentage of nasal bone dysplasia. Conclusion:When thickened NT is detected in fetuses,chromosomal abnormalities,especially aneuploidy,should be considered first. The thickened NT is also associated with copy number variations and certain monogenic inherited diseases. For fetuses with thickened NT and negative chromosome microarray analysis,comprehensive consideration including ultrasound and family history may warrant the whole-exome sequencing.

      Abstract:Objective:To study the characteristics and correlative factors of pulse oxygen saturation(SpO2)and exercise tolerance during the 6-min walk test(6MWT)in the stable chronic obstructive pulmonary disease(COPD)patients,aiming to provide a simple and effective objective assessment method for COPD patients. Methods:A total of 58 stable COPD patients and 21 control subjects were enrolled. General data,modified Medical Research Council dyspnoea scale(mMRC)score,COPD assessment test(CAT)score, pulmonary function parameters(such as FEV1%Pred,FVC%Pred,DLCO%Pred,DLCO/VA%Pred),and 6MWT-related parameters were collected. The study analyzed the differences in SpO2 and exercise tolerance indicators between groups,and their correlation with commonly used COPD assessment indicators. Results:COPD patients exhibited a decline in exercise SpO2 during the 6MWT, characterized by the occurrence of exercise-induced desaturation(EID),a decline in minimum SpO2(SpO2min)and mean SpO2(SpO2mean), an increase in the percentage of time with SpO2 below 88%,90%,and 92%(T88,T90,T92)and desaturation area(DA). There were differences in the 6 - minute walk distance(6 MWD)as a percentage of the predicted value(6MWD% Pred)and the desaturation distance ratio(DDR)between the two groups. SpO2mean,DA,DDR and 6MWD%Pred were correlated with mMRC scores,FEV1%Pred, FVC%Pred,and resting SpO2(SpO2rest). DDR was also correlated with DLCO%Pred and DLCO/VA%Pred. However,multivariable linear regression analysis showed that only mMRC scores,DLCO%Pred,and SpO2rest were independently associated with DDR. Conclusion: Continuous SpO2 monitoring during the 6MWT,with parameters such as SpO2mean,DA,DDR,and 6MWD%Pred,can effectively reflect pulmonary function and symptoms of stable COPD patients,suggesting its efficacy in assessment and management of COPD patients.

      Abstract:Objective:To develop a predictive model for significant liver fibrosis based on serological markers in the population with non - alcoholic fatty liver disease(NAFLD). Methods:A total of 2 543 NAFLD patients from the he National Health and Nutrition Examination Survey(NHANES)database in the United States were selected. These patients were randomly divided into a training set and an internal validation set in a 7∶3 ratio. Chi-square tests,univariate analysis,and binary logistic regression analysis(stepwise)were performed on both sets using SPSS 26.0,followed by model evaluation and validation in R 4.3.1. Results:Age,gender,hip circumference,ferritin,aspartate aminotransferase(AST),gamma-glutamyl transferase(GGT),and cardiometabolic index(CMI)were identified as independent risk factor for significant liver fibrosis in NAFLD patients,respectively the predictive model aCHANGE demonstrated good performance in both the training and validation sets,with an area under the curve(AUC)of the receiver operating characteristic(ROC)curve of 0.775 in both sets. The calibration curves and decision curve analysis(DCA)also showed good results. Conclusion:The aCHANGE model is useful for predicting the risk of significant liver fibrosis in NAFLD patients.

    • Effect of tracheal tube cuff pressure control on intraoperative hemodynamics and postoperative sore throat during laparoscopic colorectal surgery

      Abstract:Objective:To investigate the effects of tracheal tube cuff pressure control on intraoperative hemodynamics and postoperative sore throat during laparoscopic colorectal surgery. Methods:Ninety -four patients who underwent laparoscopic radical surgery for colorectal cancer were selected. Patients were randomly divided into Group H(tracheal tube cuff pressure control group,n= 48)and Group L(finger sensation method group,n=46). The pressure of the tracheal tube cuff in Group H was controlled at 25-30 cmH2O,while patients in Group L were only be monitored for cuff pressure. Heart rate(HR),mean arterial pressure(MAP),cuff pressure,and airway pressure were continuously monitored and recorded at seven time points including pre -induction of anesthesia (T0),after intubation(T1),before establishment of pneumoperitoneum(T2),after establishment of pneumoperitoneum(T3),after Trendelenburg position(T4),before tracheal extubation(T5),and after tracheal extubation(T6). Additionally,the incidence and severity of sore throat and hoarseness were investigated in patients at 2 h,12 h and 24 h after surgery. Results:The HR,MAP,and cuff pressure in Group L were significantly higher than that in Group H at time points T1- T5(P < 0.05). The airway pressure in both groups significantly increased at T3(P < 0.05)and further increased at T4(P < 0.05),but without significant difference between the two groups. The incidence of postoperative sore throat at 2 h and 12 h was lower in group H than that in group L(P < 0.05). There was no statistical difference in the incidence and severity of postoperative hoarseness between the two groups(P > 0.05). Conclusion:In patients undergoing laparoscopic colorectal surgery,tracheal tube cuff pressure control can reduce the incidence of postoperative sore throat and maintain the relative stability of intraoperative hemodynamics.

    • Construction and analysis of an olfactory prognosis model for postoperative outcomes in diffuse chronic rhinosinusitis

      Abstract:Objective:To explore the relationship between clinical characteristics and olfactory prognosis after the first endoscopic sinus surgery in patients with diffuse chronic rhinosinusitis with nasal polyps(CRSwNP). Additionally,to establish and verify a nomogram prediction model for poor olfactory improvement after the first endoscopic sinus surgery in these patients. Methods:Clinical data were collected and analyzed from 67 patients with diffuse CRSwNP and olfactory dysfunction who underwent their first endoscopic sinus surgery at the Department of Otolaryngology,the First Affiliated Hospital of Nanjing Medical University. Based on the postoperative improvement in olfactory dysfunction,patients were categorized into an improvement group(50 cases)and a non - improvement group(17 cases). Univariate and multivariate logistic analyses were conducted to identify factors influencing olfactory improvement. Based on these factors,a nomogram model was established and its predictive performance was evaluated. Results: Multivariate analysis indicated that the length of the disease course(β=0.527,OR=1.693,95%CI:1.139-2.517),Lund-Mackay sinus CT score(β=0.704,OR=2.021,95%CI:1.208-3.382),peripheral blood eosinophil ratio(β=0.311,OR=1.365,95%CI:1.026-1.815), and the ratio of ethmoid to maxillary sinus CT scores(β=0.954,OR=2.597,95%CI:1.263-5.340)were independent predictive factors for diffuse CRSwNP patients with poor olfactory improvement after their first endoscopic surgery. The nomogram model constructed based on these factors showed an area under the receiver operating characteristic(ROC)curve is 0.770(95%CI:0.658-0.881,P < 0.01),a C-index of 0.856(0.817-0.894),and a mean absolute error of 0.034 in the calibration curve. Conclusion:The nomogram model based on the disease course,Lund-Mackay sinus CT score,peripheral blood eosinophil ratio,and the ratio of ethmoid to maxillary sinus CT scores can predict olfactory improvement after the first endoscopic sinus surgery in diffuse CRSwNP patients,providing valuable clinical guidance.

      Abstract:Objective:To investigate the clinical efficacy of intra-articular injection of chitosan combined with low-dose corticosteroids in the treatment of knee osteoarthritis(KOA)in the early and middle stages. Methods:A retrospective analysis was conducted on 164 patients with KOA who visited and were followed up at the outpatient department of orthopedics at the First Affiliated Hospital of Nanjing Medical University from June 2021 to December 2022. Based on the treatment methods,patients were divided into three groups:Group A(control group,oral administration)consisted of 55 patients,Group B(intra -articular injection of chitosan+oral administration)had 53 patients,Group C(intra-articular injection of chitosan combined with low-dose glucocorticoids) included 56 patients. Variations in pain visual analogue scale(VAS),Lysholm score,Hospital for Special Surgery(HSS)score,and the levels of inflammatory cytokines in the synovial fluid(interleukin-1,IL-1;interleukin-6,IL-6;tumor necrosis factor-alpha,TNF-α) were compared before and after treatment. Clinical efficacy was also evaluated. Results:All cases were followed up for 7-12 months and no cases were lost. Post-treatment VAS scores,Lysholm scores,HSS scores,and the levels of synovial fluid inflammatory cytokines (IL-1,IL-6,and TNF-α)were all significantly improved compared to pre-treatment levels in each group(P <0.05). There were no significant differences in pre-treatment VAS scores,Lysholm scores,HSS scores and the levels of IL-1,IL-6,and TNF-α among the three groups(P > 0.05);however,post-treatment comparisons showed significant differences(P <0.05). Compared with each other, Group C showed better outcomes than Groups A and B,and Group B was superior to Group A,with significant statistical differences (P <0.05).The comparison of clinical efficacy among the three groups also showed statistically significant differences(P <0.05). Conclusion:Intra-articular injection of chitosan combined with low-dose corticosteroids shows significant clinical efficacy in the treatment of KOA in early and middle stages,with the advantages of minimal trauma,rapid onset,lasting efficacy and avoids the adverse reactions of external application or oral administration of non-steroidal anti-inflammatory drugs. As a first-line treatment option,its clinical promotion and use are recommended.

    • Construction and evaluation of Bandavirus davieense virus mRNA vaccine

      Abstract:Objective:To construct a Bandavirus davieense virus(DBV)vaccine based on the strategy of mRNA vaccine,immunize BALB/c mice,and evaluate its immunologic characteristics. Methods:The coding sequence of DBV glycoprotein Gn was optimized and synthesized,then inserted into the pGEM -3Zf(+)plasmid. The linearization plasmids were enzyme digested by BamH Ⅰ,cap and polyA tailing were added with polymerase to complete in vitro transcription. Western blotting verified the protein expression by transient transfection of the mRNA into eukaryotic 293FT cells. The mRNA was delivered by lipid nanoparticles,and immunized BALB/c mice by intramuscular injection of low(2 μg/mouse),medium(5 μg/mouse)and high(20 μg/mouse)dose once every two weeks. The antibody titer in mice serum was detected by ELISA,and the ability of antibody to neutralize DBV in vitro was detected at the cellular level by virus neutralization assay. Results:The prepared DBV mRNA vaccine could be transcribed and expressed in vitro. All three immunization doses induced high levels of specific antibodies in mice,with the high-dose immunization group inducing stable antibody expression for over 10 weeks. The virus neutralization assay showed that immune sera could bind to the virus and block virus infection of cells. Conclusion:A DBV mRNA vaccine capable of inducing specific neutralizing antibodies in mice is obtained,laying the foundation for research on the prevention of DBV infection.

      Abstract:Objective:To study the diagnostic efficacy of thromboelastography combined with ultrasound quantitative parameters on fetal growth restriction(FGR). Methods:A total of 50 cases of pregnant women with FGR admitted to Zhengzhou Maternal and Child Health Hospital from July 2021 to May 2023 were collected as the FGR group,and 50 pregnant women with normal fetal development in the same period and age group were selected as controls(the control group). The thromboelastogram indicators[maximum amplitude (MA),Angle,coagulation time(K),coagulation response time(R)],umbilical and middle cerebral artery resistance index(RI), pulsation index(PI),end-systolic/end-diastolic peak value(S/D),cerebroplacental rate(CPR),weight,and abdominal circumference were compared between the two groups. Binary correlation and partial correlation analyses were performed to explore the correlation of thromboelastogram and ultrasound quantitative parameters with weight or abdominal circumference. The receiver operating characteristic(ROC)curve was performed to analyze the diagnostic value of thromboelastogram,ultrasound quantitative parameters, and the combined diagnosis of FGR. Results:Second-trimester,late-pregnancy,and before delivery the value of R and K were lower in the FGR group than in the control group,while the value of Angle and MA were higher(P < 0.05). The value of Umbilical artery S/D, umbilical artery PI,and umbilical artery RI in the FGR group were higher than those in the control group during the second-trimester, late-pregnancy,and before delivery,while the value of middle cerebral artery S/D,middle cerebral artery PI,middle cerebral artery RI,and CRP were lower than those in the control group(P < 0.05). The weight and abdominal circumference of the FGR group were lower than those of the control group in the second-trimester,late-pregnancy,and before delivery(P < 0.05). The binary correlation analysis showed that the thromboelastography and ultrasound quantitative parameters before delivery were more strongly correlated with both body weight and abdominal circumference(P < 0.05). Partial correlation analysis indicated that the value of R,K,middle cerebral artery RI,and CRP were positively correlated with body weight and abdominal circumference before delivery,while the value of Angle, MA,umbilical artery S/D,umbilical artery PI and umbilical artery RI remained negatively correlated with body weight and abdominal circumference(P < 0.05). The combined diagnosis of FGR by using thromboelastography and ultrasound quantitative parameters before delivery had a higher area under curve(AUC)than each single index(P < 0.05). Conclusion:The quantitative detection of thromboelastography combined with ultrasound in pregnant women with FGR can effectively monitor changes in coagulation function and fetal hemodynamics at different stages of pregnancy,which has a high value in the diagnosis of FGR.

      Abstract:Objective:To analyze the correlation between peripheral blood nucleotide binding oligomerization domain like receptor protein 3(NLRP3)inflammasome-cysteine protease 1(Caspase-1)-interleukin 1β(IL-1β)signaling axis and lung cancer. Methods: From June 2021 to August 2023,72 patients with lung cancer in our hospital(lung cancer group)and 72 healthy subjects(healthy control group)were selected. The levels of NLRP3,Caspase-1,IL-1β in peripheral blood and peripheral blood of lung cancer control and healthy group subjects were compared and analyzed. The levels of pulmonary function[forced expiratory volume in one second (FEV1),forced vital capacity(FVC),and ratio of forced expiratory volume in the first second to forced vital capacity(FEV1/FVC)] were compared. The correlation between peripheral blood NLRP3 and IL - 1β levels and FEV1,FVC and FEV1/FVC was analyzed. Results:The levels of FEV1,FVC and FEV1/FVC in lung cancer group were significantly lower than those in healthy control group(P < 0.05). The relative mRNA expression levels of Nlrp3Caspase-1 and IL-1β,as well as the secretion levels of Caspase-1 and IL-1β in peripheral blood of lung cancer group were significantly higher than those of healthy control group(P < 0.05). Spearman correlation analysis showed that Nlrp3 gene mRNA relative expression levels in peripheral blood were significantly negatively correlated with FEV1/FVC in lung cancer group(P < 0.05). The peripheral blood IL -1β secretion levels were significantly positively correlated with FEV1 and FEV1/FVC in lung cancer group(P < 0.05). Conclusion:Nlrp3 gene mRNA relative expression level in peripheral blood monocytes and peripheral blood IL-1β secretion level are positively correlated with the progression of lung cancer and can reflect the progression of lung cancer to a certain extent,which has potential reference value for the screening and diagnosis of lung cancer.

    • Clinical comparative study of dental floss traction and IT knife assisted endoscopic submucosal dissection in the treatment of gastric horn lesions

      Abstract:Objective:To study the clinical effects of dental floss traction assisted submucosal dissection and IT knife assisted disposable mucosal dissection knife in the treatment of early gastric horn cancer and precancerous lesions. Methods:One hundred and forty-six patients with early gastric horn cancer and precancerous lesions admitted from December 2015 to August 2023 were included as study subjects. They were divided into two groups according to the treatment plan:dental floss traction group(n=73)and IT knife group(n=73). The time of operation and lesion dissection,muscularis injury,surgical effect,and postoperative complications in the two groups were evaluated and compared. Results:The number of muscularis injury and the use of hemostatic clips were significantly reduced in the dental floss traction group compared to the IT knife group(P < 0.05). There was no significant difference in the whole lesion resection rate,R0 resection rate,curative resection rate,margin involvement rate,and perioperative complication rate (postoperative bleeding and perforation)between the two groups(P > 0.05). Univariate analysis showed that floss traction - assisted submucosal dissection with disposable mucosal dissection knife was effective in reducing muscularis injury(OR=0.37,95%CI:0.18- 0.74,P=0.006). Conclusion:The use of dental floss assisted endoscopic submucosal dissection for early gastric horn cancer and precancerous lesions is particularly effective. It can significantly reduce the damage to the muscularis propria and ensure the safety and effectiveness of the procedure.

    • Analysis of the trend of public health emergencies of infectious diseases in Jiangsu Province from 2004 to 2022

      Abstract:Objective:To analyze the epidemic characteristics of public health emergencies of infectious disease in Jiangsu Province,and to provide scientific basis for prevention and control efforts. Methods:Joinpoint regression model was used to analyze the temporal trends,and space -time permutation scan was employed for spatiotemporal cluster detection and exploration. Results:From 2004 to 2022,a total of 3 641 emergencies were reported,with 131 880 cases and 73 deaths. There were 1 868 emergencies involving 25 notifiable infectious diseases,including 76 Class A,391 Class B,and 1 401 Class C diseases. Additionally,1 773 emergencies of 8 other non-notifiable infectious diseases were involved. Joinpoint regression analysis revealed that the number of emergencies in Jiangsu Province showed an increasing trend[average APC(AAPC)=16.28,P < 0.05)]. Class A showed a declining trend since 2006 (AAPC =-18.23,P < 0.05),while Class B,Class C,and others exhibited increasing trends(AAPC>0,P < 0.05). Emergencies in Southern of Jiangsu,Middle of jiangsu,and Northern of Jiangsu all showed increasing trend(AAPC>0,P < 0.05). In Southern of Jiangsu,the trend was characterized by 4 stages:rapid increase,slow increase,rapid increase,and decrease. In Middle of Jiangsu, Emergencies showed a monotonic increase. In Northern of Jiangsu,the trend was characterized by 4 stages:increase,slow decrease,rapid increase,and decrease. 2006,2015,and 2018 were major turning points. 8 space-time clusters were detected,sequentially distributed in midde of Jiangsu + Southern of Jiangsu,Northern of Jiangsu,Southern of Jiangsu,and Southern of Jiangsu+Northern of Jiangsu. Conclusion:In Jiangsu Province,infectious disease emergencies showed increasing trends,with clusters in southern and northern regions and relatively scattered in middle region. Priority areas need to focus on monitoring and controlling key diseases such as chickenpox,influenza,and hand foot and mouth disease(HFMD).

      Abstract:As a kind of gas signaling molecules,nitric oxide(NO)is involved in the regulation of diverse physiological functions. In the central nervous system,physiological concentrations of NO participate in maintaining physiological neuropsychiatric functions, whereas high concentrations of NO are neurotoxic,which promote several neuropsychiatric diseases through a wide variety of pathological processes. This review discusses the metabolism and functions of NO in the central nervous system. Taking Alzheimer’s disease,Parkinson’s disease,multiple sclerosis,depression and autism spectrum disorder as examples,the relationship and pathogenic mechanism of NO with neurodegenerative diseases,neuroinflammatory diseases,mental disorders and neurodevelopmental disorders are described,which provides ideas and impetus for further development of pathogenic mechanism and therapeutic drugs.

      Abstract:Atrial fibrillation(AF),a common supraventricular arrhythmia,is associated with severe complications,including ischemic stroke and heart failure,significantly increasing morbidity and mortality rates. Its pathogenesis encompasses a wide array of electrophysiological and pathophysiological mechanisms. Central to AF’s onset,persistence,and recurrence is atrial structural remodeling,notably characterized by left atrial(LA)myocardial fibrosis,which acts as a crucial predictive indicator. Catheter ablation (CA)is the preferred treatment for symptomatic or drug-resistant AF,despite a notable recurrence rate post-procedure. Contemporary research highlights a significant link between LA fibrosis and AF recurrence following CA. Thus,precise evaluation and quantification of LA fibrosis are imperative for identifying AF patients at high risk,formulating individualized ablation approaches,and estimating the likelihood of post-CA AF recurrence. This paper thoroughly reviews the interconnected mechanisms between LA fibrosis and AF, encompassing aspects such as imaging and serological evaluations,with the aim of informing AF prevention and management strategies.

      Abstract:Aortic dissection(AD)is a critical emergency,where rapid and accurate diagnosis and treatment significantly influence the prognosis of AD patients. However,achieving such precise and swift diagnosis and treatment poses substantial challenges. In recent years,the application of deep learning based on CT images in assisting the diagnosis and treatment of AD has increasingly gained prominence,particularly in the identification,segmentation,and prediction of patient outcomes,thereby offering significant value for clinical decision -making in AD management. To better understand the application of deep learning based on CT images in AD,this review will summarize and discuss the current applications,existing challenges,and potential optimization strategies of deep learning in the diagnosis and treatment of AD.
