In the context of the rising prevalence of chronic non?communicable diseases in China，chronic disease management and the construction of medical alliances work together to comprehensively improve the chronic disease management capacity on the basis of the linkage mechanism of medical alliances and guide the graded diagnosis and treatment of common chronic diseases in order to accelerate the construction of Healthy China. Based on the construction frame of the graded diagnosis and treatment system of medical alliances，this paper summarized and analyzed the main structure，practice characteristics and the existing problems of chronic disease management modes in Xiamen，Shanghai，Langfang，Beijing and Foshan. It was found that domestic chronic disease management had shortcomings in supporting policies，internal mechanism of medical alliances and residents’health awareness. It was suggested we should take the medical alliance system as a platform to improve the chronic disease management mode from the aspects of system and policy，talent team and salary assessment，so as to promote the high?quality development of chronic disease management.
