In the context of high-quality social development，population aging is becoming more and more severe，and residents’active health awareness is constantly improving. Various countries have proposed different forms of health management for the growing demand of chronic disease services for the elderly population. However，China’s management of chronic diseases among the elderly is still in the preliminary exploration stage with insufficient construction experience. Based on this，this paper used foreign practical experience as a reference to summarize and analyze the current situation of domestic chronic disease management service under the background of aging from various aspects. This paper found that the current domestic health management of the elderly is mainly based on community work， with medical care， coordination of traditional Chinese and western medicine and smart senior care as the primary service content. There are still deficiencies in supporting policies，community health service capacity and construction，and active health awareness of the population. It is still necessary to focus on medical insurance and medical policies， health talent construction， community performance assessment mechanism， information platform construction，health education and other aspects to improve the health service system and quality of health management.
