Despite the increasingly evident trend of“delayed age”of first childbearing，the advancing onset age of stroke has prevented the full realization of reproductive potential among young stroke groups. The continued decline in fertility intentions of young chronic disease patients，especially those with stroke，requires immediate attention from relevant administrative authorities. This study used purposive sampling to select 15 young stroke patients and spouses receiving treatment in the neurology department of a tertiary hospital in Anhui province for semi-structured，in-depth interviews. Using Colaizzi’s descriptive phenomenological method， four major themes were identified from the original data：generally low fertility intentions，concerns about the negative impact of illness on reproduction，a lack of education related to stroke rehabilitation and fertility safety，and the need for psychological，social，and family support. This study concludes that the fertility intentions of young stroke patients are generally low，with influencing factors mainly including individual factors，psychological and social factors，and economic and environmental factors. To address the identified needs，this study proposed comprehensive intervention strategies，including enhancing medical support and health guidance by promoting health education，establishing specialized medical consultations，and enhancing rehabilitation and follow-up services，as well as improving the social environment and support system.
