In the ever-quickening urbanization process, there emerges a newly-arising disadvantaged urban community，which mainly consists of land-requisitioned farmers and low-pensioned city residents. According to PRECEDE-PROCEED model, this article is in-depth analysis of predisposing factors、reinforcing factors and enabling factors of the elderly in the special community. Aiming at reaching the goal of health intervention，efforts should be pushed forward on the countermeasures，including knowledge、attitude and behavior.
