In the era of high-quality social development, the situation of population aging is becoming more and more severe, and residents" active health awareness is constantly improving. Various countries have proposed different forms of health management to meet the health needs of the elderly. However, the aging health management in China is still in the preliminary exploration stage, and the construction experience is still insufficient.Based on this, this paper takes foreign practical experience as a reference to summarize and analyze the current situation of domestic chronic disease management service under the background of aging from various aspects. It is found that the current domestic health management of the elderly is mainly based on community work,with a chronic disease service orientation,with medical care, traditional Chinese and Western medicine and smart old-age care as the main service content. And there are deficiencies in supporting policies, community health service capacity and construction, and active health awareness of the population. It is still necessary to focus on medical insurance and medical policies, health personnel construction, community performance appraisal mechanism, information platform construction and Health education and other aspects will improve the health service system and improve the quality of health management.
