    认知障碍是老龄化社会的重大公共卫生挑战,膳食营养被视为可干预的重要靶点。中国化地中海-得舒饮食干预神经退行性延迟(China-adapted Mediterranean-DASH Intervention for Neurodegenerative Delay, cMIND)饮食已初步建立,但缺乏纵向证据支持其与老年认知障碍的关联。本研究聚焦≥65岁中国老年人,基于中国老年健康影响因素跟踪调查(Chinese Longitudinal Healthy Longevity Survey, CLHLS)2008-2018年四个波次数据,采用潜类别增长分析识别cMIND饮食长期轨迹,并探讨其与认知障碍的关系。结果显示,老年人群cMIND饮食呈现持续高依从、稳定中等、中等-下降及持续低依从四种轨迹。与持续高依从型相比,中等-下降型认知障碍风险升高35%,持续低依从型升高48%(均P<0.05)。高龄、文盲、农村居住、缺乏规律锻炼及抑郁是低依从轨迹的独立风险因素。长期维持高cMIND饮食依从性而非仅关注单一时点摄入,是预防老年认知障碍的重要可干预靶点。社区营养干预宜尽早启动并长期坚持,并重点关注农村及低教育水平等脆弱群体。

    Abstract:

    Cognitive impairment poses a major public health challenge in aging societies, and dietary nutrition is considered a key modifiable target. The China-adapted MIND (cMIND) diet has been preliminarily established, but longitudinal evidence supporting its association with cognitive impairment in older adults is lacking. This study focused on Chinese older adults aged ≥65 years and used four waves of data (2008–2018) from the Chinese Longitudinal Healthy Longevity Survey (CLHLS) to identify long-term trajectories of cMIND diet adherence via latent class growth analysis and to examine their relationship with cognitive impairment. Results showed that cMIND diet adherence in older adults followed four distinct trajectories: persistently high, stable moderate, moderate-declining, and persistently low. Compared with the persistently high adherence group, the moderate-declining group had a 35% higher risk of cognitive impairment, and the persistently low group had a 48% higher risk (both P < 0.05). Advanced age, illiteracy, rural residence, lack of regular physical exercise, and depression were independent risk factors for low-adherence trajectories. Maintaining long?term high adherence to the cMIND diet, rather than focusing solely on single?time?point intake, is an important modifiable target for preventing cognitive impairment in older adults. Community-based nutritional interventions should be initiated early and sustained, with particular attention to vulnerable populations such as those in rural areas and with low educational levels.
