                  Issues and thoughts on community health management from the

                 Abstract：Health ⁃ centered community health management is committed to improve the health status of
             community residents and raise the health level of community people. On the premise of Healthy China and
             comprehensive health，based on the concept and theoretical framework of community health management，
             combined with the current common community chronic disease health management model and the life cycle
             health management model，this paper analyzed the key problems of community health management services in
             the aspects of department coordination ability，talent construction，service standards and self management
             concepts，and put forward targeted，overall and constructive suggestions，so as to provide reference for the
             development of community health management.
