Correlation of serum chemerin and non⁃HDL⁃C/HDL⁃C ratio with microalbuminuria for patients with type 2 diabetes

目的:探讨2型糖尿病患者血清趋化素(chemerin)、非高密度脂蛋白胆固醇和高密度脂蛋白胆固醇比值(non-high-density lipoprotein cholesterol to high-density lipoprotein cholesterol ratio,non-HDL-C/HDL-C)与微量白蛋白尿的相关性。方法:选择2型糖尿病患者共254例,按照尿白蛋白/肌酐比率(urinary albumin to creatinine ratio,UACR)分为正常白蛋白尿组(UACR<30 μg/mg,129例)和微量白蛋白尿组(UACR 30~<300 μg/mg,125例)。采用酶联免疫吸附试验法检测血清chemerin水平,同时记录糖尿病病程、身高、体重、腰围等临床一般资料,测定血脂、空腹血糖、空腹胰岛素、糖化血红蛋白等生化指标。结果:微量白蛋白尿组的病程、腰围、血清chemerin、non-HDL-C、non-HDL-C/HDL-C、糖化血红蛋白、胰岛素抵抗指数均高于正常白蛋白尿组,差异均有统计学意义(P < 0.05);Pearson相关分析显示UACR与病程、腰围、血清chemerin、non-HDL-C、non-HDL-C/HDL-C、糖化血红蛋白、胰岛素抵抗指数均呈正相关(P < 0.05);以微量白蛋白尿为因变量的二分类非条件Logistic回归分析结果显示血清chemerin、non-HDL-C/HDL-C作为独立影响因素影响2型糖尿病患者微量白蛋白尿的发生。结论:2型糖尿病患者血清chemerin、non-HDL-C/HDL-C与UACR呈正相关,且均为发生微量白蛋白尿的独立危险因素,两者可作为预测早期糖尿病肾病的重要参考指标。
