    目的:探究伴中央颞区尖峰的儿童自限性癫痫(self-limited epilepsy with centrotemporal spikes,SeLECTS)患儿全脑神经磁网络的改变,并探索其与认知功能之间的关系。方法:收集63例未服药的SeLECTS组患儿和30名健康对照(health control, HC)组脑磁图(magnetoencephalography,MEG)数据。校正振幅包络相关性(corrected amplitude envelope correlation,AEC-c)分析估算全脑振荡功能连接(functional connectivity,FC)。采用第4版韦氏儿童智力量表(the fourth edition of the Wechsler intelli- gence scale,WISC-Ⅳ)对所有患儿进行智力评估。Spearman相关分析法计算AEC-c值与WISC-Ⅳ指数之间的相关性。结果:与 HC组比较,SeLECTS患儿在beta频段右外侧眶额与右距状旁回之间的FC强度下降;在theta频段,左侧嗅皮质与左侧前扣带回头部FC网络强度下降;在gamma2频段,右侧梭状回与左侧顶叶下回之间的FC网络强度下降(P均< 0.05)。相反,在theta频段,左侧前扣带回头部与左侧岛回之间及左侧海马旁回和右侧梭状回之间的FC强度,以及在gamma1频段,左侧中央后回和右侧楔叶之间的FC网络增强(P < 0.05)。在theta频段,工作记忆指数(working memory index,WMI)得分与右侧梭状回和左侧海马旁回之间的 AEC-c 值呈正相关(r=0.255,P < 0.05);同样在theta波段,加工速度指数(processing speed index,PSI)得分与左侧岛回和左侧前扣带回头部之间的AEC-c值呈正相关(r=0.258,P < 0.05);在gamma1频段,直觉推理指数(perceptual reasoning index,PRI)得分与左侧中央后回和右侧楔叶之间的AEC-c值呈负相关(r=-0.266,P < 0.05)。结论:与HC相比,SeLECTS患儿的FC网络在theta、beta和gamma频段上表现出显著差异。此外,这些FC网络表现出差异的脑区大多位于认知相关网络的关键节点上,这可能有助于理解SeLECTS患儿认知功能早期改变的机制。其次,FC增强的网络连接值与认知相关指标表现出线性相关,体现了神经影像学技术在评估认知功能损害方面的潜力。

    Abstract:

    Objective:To examine changes in whole-brain neuromagnetic networks in children with self-limited epilepsy with centrotemporal spikes(SeLECTS)and assess the association between these network alterations and cognitive performance. Methods: Magnetoencephalography(MEG)data were collected from 63 unmedicated children with SeLECTS and 30 healthy controls(HC). Corrected amplitude envelope correlation(AEC-c)analysis was performed to estimate the whole-brain oscillatory functional connectivity(FC). Cognitive function was assessed in all children using the fourth edition of the Wechsler Intelligence Scale for Children(WISC-Ⅳ). Spearman’s correlation analysis was applied to determine the relationship between AEC-c values and WISC-Ⅳ indices. Results:Compared to the HC group,children with SeLECTS exhibited reduced FC in several networks,including the left entorhinal cortex and the left rostral anterior cingulate in the theta band,the right lateral orbitofrontal cortex and the right pericalcarine in the beta band,and the right fusiform gyrus and the left inferior parietal gyrus in the gamma2 band(all P < 0.05). Conversely, increased FC was observed between the right fusiform gyrus and the left parahippocampal gyrus,the left insular gyrus and the left rostral anterior cingulate in the theta band,and the left postcentral gyrus and the right cuneus in the gamma1 band(P < 0.05)Regarding the relationship between FC and cognitive performance,in the theta band,WMI scores showed a positive correlation with the AEC-c value between the right fusiform gyrus and the left parahippocampal gyrus(r=0.255,P < 0.05). Additionally,in the theta band, PSI scores were positively correlated with the AEC-c value between the left insular gyrus and the left rostral anterior cingulate(r= 0.258;P < 0.05). In the gamma1 band,a negative correlation was found between PRI scores and the AEC-c value of the left postcentral gyrus and the right cuneus(r=- 0.266,P < 0.05). Conclusion:The FC networks of children with SeLECTS exhibited significant differences in theta,beta,and gamma bands compared to the HC group. These altered brain regions were located in key nodes of cognition-related networks,suggesting early mechanistic insights into cognitive dysfunction. The AEC-c value of enhanced FC network showed linear correlation with cognitive-related metrics,highlighting MEG’s potential for assessing cognitive impairment.
