    目的:应用组织多普勒成像技术评价不同类型肥厚型心肌病患者室间隔及左室侧壁各节段收缩功能的异常-方法:采集肥厚型心肌病患者83例(其中室间隔肥厚52例,心尖肥厚16例及对称性肥厚15例)和正常对照者30例的动态组织多普勒图像后,分别测量各组室间隔和侧壁的基底段-中段-心尖段的收缩期峰值速度(Vs)-峰值应变(ε)-位移(S)-达峰值应变的时间(Tε),各组结果分别与对照组进行分析比较-结果:ε与相应节段心肌肥厚的部位相关,如心尖肥厚组心尖部应变值降低;对称性肥厚组室间隔-侧壁均降低;室间隔肥厚组室间隔应变显著降低-ε与心肌肥厚的程度有相关性-Vs-S与心肌肥厚的部位和程度没有相关性-各种不同类型肥厚型心肌病患者左室长轴各节段的Tε没有显著性差异-结论:组织多普勒成像技术可评定肥厚型心肌病患者局部心肌收缩功能的异常-

    Abstract:

    Objective:To evaluate the regional myocardial systolic function in the patients with hypertrophic cardiomyopathy(HCM) by TDI imaging. Methods:Doppler myocardial imaging of 83 HCM patients(HCM group) and 30 healthy subjects(control group) was collected,and their regional peak velocity,systolic strain,disaplacement,time to peak systolic strain in the apical,mid and basal segment was measured respectively at septum and lateral wall. The data was compared and analyzed between the two groups. Results:The strain value of HCM group was significantly lower in the place where hypertrophy was happened. There was a significant correlation between midseptal strain and thickness of intraventricular septum. Velocity and displacement was decreased in septum and lateral wall,but have no significant correlation with the extend and site of hypertrophy. No significant difference was found about the time to systolic strain of left ventricle. Conclusion:TDI imaging could detect the global and regional myocardial function of HCM patients,and provide a new quantitative parameter for evaluating the regional myocardial functions.
