    目的:探讨肺炎支原体(Mycoplasma pneumoniae,MP)快速培养法在诊断儿童MP感染中的临床价值,并分析其影响因素-方法:MP快速培养鉴定患儿咽试子标本,被动凝集法检测血清MP抗体(MP-Ab),分析MP感染的相关临床因素-结果:MP快速培养与MP-Ab检测诊断MP感染的阳性率基本一致,差异无统计学意义(P=0.556)-女性儿童MP培养的阳性率(46.26%)略高于男性(42.16%)-3~5岁幼儿的MP培养阳性率(54.05%)明显高于其他各组(P < 0.001)-冬-春季MP培养阳性率分别为48.05%和49.03%,显著高于夏秋两季(26.26%和36.97%,P < 0.001)-MP感染患儿体温增高-咳嗽-咳痰等症状与MP培养结果无明显相关性,血液常规分析中性粒细胞比值显著上升,与其他呼吸道感染相似,尚不能作为诊断MP感染的独立指证-结论:MP感染冬春易发,儿童易感-MP感染缺乏特异的临床症状和体征,MP快速培养对早期快速诊断儿童MP感染具有重要临床价值和意义-

    Abstract:

    Objective:To evaluate the clinical value of rapid culture of Mycoplasma pneumoniae (MP) in diagnosis of MP for children and explore the potential influence factors related to the MP rapid culture. Methods:The rapid MP culture method was used for throat swabs of acute respiratory infection,and passive agglutination assay was used to detect serum MP antibody(Ab). Meanwhile the multiple related factors which can influence the MP infection were analyzed. Results:Female children patients’ positive rate of MP cultivation was 46.26%,higher than male children’s (42.16%). The positive rate of rapid MP cultivation for children with 3~5 years old were 54.05%,highest among all groups with different years old(P < 0.001). The results of rapid MP cultivation was different among 4 seasons. The positive rates of rapid MP cultivation in winter and spring were 48.05% and 49.03%,respectively,obviously higher than those in summer and autumn (26.26% and 36.97%,respectively)(P < 0.001). Moreover,the positive rate of rapid culture for MP was in line with the results of serum MP-Ab detection (P=0.556). No evidence was found between the common clinical symptoms such as fever,cough,expectoration and infection of MP. The increasing percentage of blood neutrophile granulocyte was similar to the respiratory infection resulting from the other pathogenic microorganisms,suggesting this can not be viewed as an independent index for diagnosis of MP inflection. Conclusion:MP inflection is common in winter and spring. Children and old people is easily infected. Children with 3~5 years old is susceptible to MP infection. The patients with MP infection is lack of special clinical symptoms. The rapid MP cultivation method plays a significant role in early rapid diagnosis of MP infection.
