    目的:检测糖尿病肾病患者血清中高迁移率族蛋白B1(HMGB1)及相关炎症因子和氧化应激指标,探讨HMGB1在糖尿病肾病发生发展中的作用-方法:收集120例临床糖尿病肾病及60例单纯糖尿病患者的临床资料及血液标本-临床糖尿病肾病患者按24 h尿白蛋白定量分为微量白蛋白尿组(n=60)和大量白蛋白尿组(n=60)-检测血液中糖化血红蛋白(HbA1c)-总胆固醇(TC)-低密度脂蛋白胆固醇(LDL-C)-高密度脂蛋白胆固醇(HDL-C)-甘油三酯(TG)-肌酐等水平,并采用酶联免疫吸附(ELISA)法检测血清中HMGB1-晚期糖基化终末产物受体(RAGE)-丙二醛(MDA)-肿瘤坏死因子α(TNF-α)浓度-变量之间相关性分析采用Pearson相关性分析-结果:①临床糖尿病肾病组血清中HMGB1-RAGE-TNF-α-MDA水平高于单纯糖尿病组,且差异具有统计学意义(P < 0.01);②HMGB1-RAGE-TNF-α-MDA在大量白蛋白尿组中表达量最高;③HMGB1与RAGE-TNF-α-MDA呈正相关-结论:HMGB1在临床糖尿病肾病患者血清中水平显著升高,且随着蛋白尿加重,其血清中的水平亦升高-HMGB1可能通过促进炎症反应及氧化应激等机制参与糖尿病肾病发生发展-

    Abstract:

    Objective:To elucidate the role of HMGB1 in the pathogenesis of diabetic nephropathy by detecting serum concentrations of HMGB1 and related inflammatory factors and oxidative stress makers in patients suffer with diabetic nephropathy. Methods:Collected clinical data and blood samples of 120 cases of diabetic nephropathy and 60 cases of simple diabetic patients. The patients with diabetic nephropathy were divided into two groups according to the 24 hours urinary albumin quantitative,microalbuminuria group (n=60) and macroalbuminuria group (n=60). HbA1c,TC,LDL-C,HDL-C,TG,creatinine of the samples were detected. Enzyme-linked immunosorbent (ELISA) was used to detect the concentration of HMGB1,RAGE,TNF-α and MDA of three groups. Results:①Compared with simple diabetes group,the serum concentration of HMGB1,RAGE,TNF-α and MDA significantly increased in two groups of diabetic nephropathy,and the difference is statistically significant (P < 0.01). ② The serum concentration of HMGB1,RAGE,TNF-α and MDA was highest in macroalbuminuria group of diabetic nephropathy. ③The expression of HMGB1 was positively correlated with RAGE,TNF-α and MDA. Conclusion:The serum concentration of HMGB1 was significantly increased in patients with clinical diabetic nephropathy.With the deterioration of albuminuria,the expression of HMGB1 gradually increased. HMGB1 may participate in the development of diabetic nephropathy by promoting inflammation and oxidative stress.
