    目的:通过Meta分析评估血清间皮素对卵巢癌的诊断价值。方法:全面检索PubMed、Embase、Web of Science、CBMdisc、万方和中国知网等数据库应用间皮素诊断卵巢癌的文献,采用RevMan 5.3、Stata 12.0和Meta-disc 1.4等统计软件进行分析。结果:有9篇文献共计1 068例患者纳入本次Meta分析,其中卵巢癌患者451例,非卵巢癌患者617例,合并灵敏度为0.80(95%CI:0.76~0.83),合并特异度为0.81(95%CI:0.78~0.84),合并阳性似然比为6.93(95%CI:3.32~14.46),合并阴性似然比为0.24(95%CI:0.16~0.35),合并诊断比值比为45.69(95%CI:17.49~119.35),SROC曲线下面积为0.90,Q=0.83。结论:血清间皮素对于卵巢癌诊断有较高的敏感度及特异度,与其他肿瘤标志物进行联合检测,可作为临床辅助诊断卵巢癌的有效方法。

    Abstract:

    Objective:To evaluate the diagnostic value of serum mesothelin level on ovarian cancer. Methods:Relevant studies were identified by systematically searching PubMed,Embase,Web of Science,CBMdisc,WANFANG,CNKI and so on,articles related to the diagnostic value of serum mesothelin on ovarian cancer were selected. Data analysis was conducted by RevMan 5.3,Stata 12.0 and Meta-Disc1.4. Results:Nine studies with a total of 451 ovarian cancer cases and 617 controls were included. The results of meta-analysis showed that sensitivity and specificity were 0.80 (95%CI:0.76~0.83) and 0.81 95%CI:0.78~0.84),respectively;positive likelihood ratio and negative likelihood ratio were 6.93 (95%CI:3.32~14.46) and 0.24 (95%CI:0.16~0.35),respectively;diagnostic odds ratio was 45.69 (95%CI:17.49~119.35);the area under curve of the summary receiver operating characteristic(SROC) was 0.90,Q=0.83. Conclusion:Mesothelin shows a high specificity and a low sensitivity in the diagnosis of ovarian cancer,and it can be used as an effective way for diagnosis of ovarian cancer combined detection with other tumor markers.
