Th22细胞联合循环肿瘤细胞对胃癌腹膜转移的预测价值

［摘 要］ 目的：探讨辅助性 T 细胞 22（T helper 22 cell，Th22）联合循环肿瘤细胞（circulating tumor cell，CTC）对胃癌腹膜转
移的预测价值。方法：连续性纳入 2022 年 1 月—2025 年 1 月淮安市淮安医院收治的 120 例胃癌患者，根据腹膜转移情况分
为转移组38例和未转移组82例，检测并比较两组Th22细胞比例和CTC计数。使用Logistic回归分析筛选胃癌患者腹膜转移
的危险因素，并通过受试者工作特征（receiver operating characteristic，ROC）曲线评估Th22细胞比例联合CTC计数对胃癌患者
腹膜转移的预测价值。结果：胃癌患者中，腹膜转移组低分化、浸润深度 T2、淋巴管侵犯比例及淋巴结转移数量、癌胚抗原
（carcinoembryonic antigen，CEA）、癌抗原 19⁃9（carbohydrate antigen 19⁃9，CA199）水平、Th22 细胞比例、CTC 计数均高于未转移
组，差异有统计学意义（P < 0.05）。Logistic回归分析显示，Th22细胞比例、CTC计数是胃癌腹膜转移的危险因素，而在校正了
低分化、浸润深度T2、淋巴管侵犯、淋巴结转移数量、CEA、CA199后，Th22细胞比例、CTC计数仍是胃癌腹膜转移的独立危险
因素（P < 0.05）。相关性分析显示，Th22细胞比例、CTC计数与胃癌低分化、浸润深度T2、淋巴管侵犯、血清CEA、CA199之间
存在明显的正相关性，且Th22细胞比例和CTC计数之间也存在明显的正相关性（P < 0.05）。ROC曲线分析结果显示，Th22细
胞比例和 CTC 计数预测胃癌腹膜转移的曲线下面积（area under curve，AUC）分别为 0.800、0.831，两项联合预测的 AUC 为
0.943，均大于各指标单独预测的AUC（Z=3.014、2.937，P均 < 0.05）。结论：Th22细胞比例与CTC计数均为胃癌腹膜转移的独
立危险因素，且二者与胃癌恶性病理特征及血清肿瘤标志物水平呈正相关。二者联合应用显示出对胃癌腹膜转移良好的预测
价值。

［Abstract］ Objective：To explore the predictive value of T helper 22 cell（Th22）combined with circulating tumor cells（CTCs）for
peritoneal metastasis of gastric cancer. Methods：A total of 120 patients with gastric cancer admitted to Huai’an Hospital of Huai’an
City January 2022 to January 2025 were consecutively enrolled. According to the presence of peritoneal metastasis，the patients were
divided into the metastasis group（38 cases）and the non⁃metastasis group（82 cases）. The percentage of Th22 cells and the count of
CTCs were detected and compared between the two groups. Logistic regression analysis was used to screen the risk factors for
peritoneal metastasis of gastric cancer，and the receiver operating characteristic（ROC）curve was used to evaluate the predictive value
of Th22 cells percentage combined with CTCs count for peritoneal metastasis of gastric cancer. Results：Among the gastric cancer
patients，the proportion of poorly differentiated tumors，depth of invasion（T2），lymphatic vessel invasion，number of lymph node
metastases，levels of carcinoembryonic antigen（CEA）and carbohydrate antigen 19⁃9（CA199），as well as the percentage of Th22 cells
